Nasopharyngeal diffuse large b-cell lymphoma in a teenage presenting with cavernous sinus syndrome: A case report.

Primary lymphoma of the nasopharynx is uncommon and is more commonly seen as an association with other primary pathologies. Nasopharyngeal lymphoma is an aggressive tumor and can affect all age groups but is more commonly seen in middle-aged and older individuals. Majority of these tumors belong to the non-Hodgkin's group. Cavernous sinus syndrome (CSS) is a rare but severe complication indicating advanced aggressive stage of disease with direct extension of tumor into the cavernous sinus. Our patient is a young teenage 18-years old presented to emergency department with the symptoms of left j

Does the stria of Gennari integrity reflect acquired visual field defects?

Visual input reaches the primary visual cortex (V1) at the stria of Gennari, a band of highly myelinated fibers at the level of layer IV. Damage to the visual pathways can cause visual field defects (scotomas) that prevent visual input from reaching V1. Here, we evaluate whether the stria of Gennari degrades in the absence of visual input. Using 7 Tesla MRI, we investigate the integrity of the stria of Gennari in case of acquired homonymous visual field defects, in particular hemianopia: the loss of vision in half of the visual field. These visual field defects allow us to compare stria of Gen

Case Report: Pituitary apoplexy temporally associated with acute alcohol intoxication, presenting as complete oculomotor nerve palsy in an SF-1 lineage gonadotroph PitNET.

Pituitary apoplexy (PA) is an acute, potentially life-threatening neuroendocrine emergency caused by hemorrhage or infarction within the pituitary gland, most often arising in a previously unrecognized pituitary neuroendocrine tumor (PitNET). Reported precipitants include surgery, anticoagulation, dynamic pituitary testing, hypertension, and head trauma; alcohol is rarely implicated. We report a 61-year-old man with long-standing hypertension, type 2 diabetes mellitus and coronary artery disease (on aspirin after coronary stenting) who developed sudden thunderclap headache and complete right o

Bilateral cranial polyneuropathy associated with an intracranial mass in a cat with concurrent tonsillar lymphoma.

A 14-year-old spayed female Siamese cat was presented with signs consistent with a rapidly progressive cranial polyneuropathy. Magnetic resonance imaging of the brain revealed an extra-axial mass at the calvarial floor, extending caudally to involve bilateral trigeminal nerves and rostrally to involve the pituitary gland and optic chiasm. Additional findings included contrast-enhancing content within the bilateral tympanic bullae, mild mandibular and medial retropharyngeal lymphadenopathy, and a left tonsillar mass. Cytologic and histopathologic findings confirmed tonsillar lymphoma. The concu

Radiological and Functional Outcomes of Joshi External Stabilization System (JESS) in Proximal Tibia Fractures: A Case Series.

INTRODUCTION: External fixators have a limited role in the management of tibial plateau fractures (TPF). They are typically used as an initial management strategy. The Joshi External Stabilisation System (JESS) is a Kirschner wire-based fixator and has been used in TPF where plating may not be feasible due to poor skin conditions or delayed due to other soft tissue complications. This retrospective case series aimed to evaluate the radiological and functional outcomes of TPF managed using the JESS fixator. MATERIALS AND METHODS: Records of patients operated on with the JESS fixator for TPF be

Atypical clinical and MRI features in Leber hereditary optic neuropathy: a case series of four patients.

BACKGROUND: Leber's hereditary optic neuropathy (LHON) is a mitochondrial disorder typically presenting with subacute, painless, bilateral vision loss in young individuals. While often recognizable, atypical presentations can complicate diagnosis. Notably, optic nerve lesions on MRI are uncommon in LHON and are typically associated with inflammatory optic neuropathies, increasing the risk of misdiagnosis. METHODS: We retrospectively analyzed four patients who initially presented with suspected optic neuritis but were later diagnosed with LHON. Demographic, genetic, clinical, and multimodal da

Neurosarcoidosis: clinical diagnosis of migratory chiasmitis and bilateral optic neuropathy.

Sarcoidosis is a diagnostically challenging and complex multisystem disease characterised by the formation of non-caseating granulomas. This case describes a young man who developed bitemporal vision loss that rapidly progressed to complete bilateral vision loss. MRI demonstrated chiasmatic and optic neuritis. A history of pulmonary nodules was noted. Via multi-disciplinary team discussion, no safe biopsy targets were identified. A clinical diagnosis of neurosarcoidosis was established. The patient's symptoms responded well to high-dose steroids and plasma exchange. Chiasmitis and optic neurit

Region-resolved proteomic map of the human brain: functional interconnections and neurological implications.

While progress has been made in transcriptomic profiling of the human brain, functional characterization of brain regions and their interactions on the basis of regional protein expression remains limited. Here, we constructed a proteomic map from thirteen anatomical brain regions of eight cadaver donors to elucidate region-specific protein expression patterns and their implications for brain function. The results underscore the interconnectivity of the four cerebral lobes, suggesting facilitated information integration through large-scale neural networks. We propose a three-module framework (

Granular cell tumors of the neurohypophysis with atypical imaging features: a case report and literature review.

BACKGROUND: Granular cell tumors (GCTs) of neurohypophysis are rare, the incidence rate of GCTs is higher in female patients compared to male patients, the enlarged GCTs in neurohypophysis can compress the optic chiasm and cause changes in the patient’s visual acuity and visual field. Complete surgical resection is often regarded as the best treatment and can be accomplished via either endoscopic transsphenoidal or open microsurgical approaches, depending on the configuration and topography of the lesion. We report a case of a tumor originating from the pituitary gland with visual field defect

Pituitary Metastasis as the Initial Manifestation of an Undiagnosed Adenocarcinoma of Mullerian Origin: A Case Report and Literature Review.

Metastasis to the pituitary gland is rare, occurring in approximately 1% of pituitary tumors resected via transsphenoidal surgery. We present the second report of metastatic high-grade serous ovarian carcinoma to the pituitary gland, presenting with diabetes insipidus and bilateral hemianopsia. An 86-year-old female patient presented with complaints of dizziness, vertigo, vision changes (peripheral field vision loss), loss of taste, extreme thirst, and fatigue. Urine studies revealed hypotonic hyponatremia, which was consistent with diabetes insipidus. The pituitary magnetic resonance imaging

Pediatric hemophagocytic lymphohistiocytosis with predominant CNS involvement: MRI features and differential diagnosis.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory disorder resulting from immune dysregulation that leads to uncontrolled macrophage activation. The diagnosis of HLH is often challenging, as its clinical manifestations can mimic various systemic conditions, including sepsis, autoimmune disorders, and malignancies. This overlap, combined with the rapid progression of the disease, may delay treatment and increase morbidity and mortality. Neurological involvement is particularly frequent in pediatric cases. Neuroimaging plays a critical role in rai

Pituitary apoplexy in a macroadenoma with cavernous sinus extension presenting with acute ophthalmoplegia.

A man in his 30s presented with a thunderclap retro-orbital headache, nausea and acute binocular diplopia with right ptosis. This presentation illustrates the 'red flag' combination of thunderclap headache and pupil-involving third-nerve palsy in pituitary apoplexy. A vascular-first, then sellar-focused imaging pathway (CT → MR angiography (MRA) → sellar MRI) expedites diagnosis and protects vision. Brain MRI with sellar protocol revealed a haemorrhagic pituitary macroadenoma with suprasellar extension compressing the optic chiasm and lateral extension abutting the right cavernous sinus. Intra

Expanding the phenotype of Wolfram syndrome: adult presentation with a novel WFS1 variant.

Wolfram syndrome type 1 (WS1) is a rare autosomal recessive disorder involving diabetes mellitus, optic atrophy, and neurodegeneration, caused by biallelic WFS1 mutations. Though typically diagnosed in childhood, adult-onset cases may be missed due to variable symptom onset. We describe a 38-year-old woman with early-onset insulin-requiring, autoantibody-negative diabetes, progressive visual loss due to optic atrophy, bilateral sensorineural hearing loss, secondary amenorrhea with hyperprolactinemia, and arginine-vasopressin (AVP) deficiency. Magnetic resonance imaging (MRI) demonstrated atrop

X-Linked Cerebral Adrenoleukodystrophy Presenting With Acute Encephalitis-Like Illness and Optic Neuropathy in a Child.

An 8-year-old boy was found unconscious after febrile gastroenteritis. Upon arrival to the emergency department, he was unresponsive with fixed dilated pupils, high fever, tachycardia, and severe hypoxia. Brain computed tomography followed by brain MRI revealed diffuse cerebral edema with tonsillar herniation. He was treated with mannitol, hypertonic saline, broad-spectrum antimicrobials, and corticosteroids, resulting in rapid systemic recovery. Four months later, follow-up MRI revealed new nonenhancing hyperintense lesions involving optic nerves, chiasm and tracts, and subtle midbrain lesion

Perimetry and visual psychophysics.

Patterns of visual loss are the key to anatomic localization. Strictly monocular defects map to the prechiasmal sensory visual system. Bitemporal hemianopia is the signature of damage to the optic chiasm. Incongruous homonymous hemianopia points to an optic tract lesion. The closer a lesion gets to the occipital cortex, the more congruous or similar are the patterns of homonymous hemianopia. Understanding these patterns will add an important dimension to the clinician's localization arsenal. Perimetry is the quantitation of the visual field. This is done with a perimeter and usually involves e

Dosimetric evaluation and clinical feasibility of Halcyon-E O-ring linear accelerator for single fraction coplanar stereotactic radiosurgery.

AIM: This study aimed to evaluate the Halcyon-E O-ring gantry linear accelerator, which lacks non-coplanar beam delivery capability, for single-fraction stereotactic radiosurgery (SRS). The validation process consisted of three steps: dosimetric analysis, clinical evaluation, and patient-specific quality assurance, along with a comparative analysis with plans generated on a C-arm linear accelerator. MATERIALS AND METHODS: A total of 96 patients who underwent single-fraction SRS using a Novalis Tx C-arm linear accelerator (linac) were replanned on an O-ring gantry linac. All patients were trea

Hypofractionated stereotactic radiation therapy for optic nerve sheath meningiomas: A single-center experience and preliminary results.

BACKGROUND: Primary optic nerve sheath meningioma (pONSM) causes progressive vision loss. Surgery carries high morbidity (~ 94% visual impairment), and conventional fractionated radiotherapy (CFRT; 50.4-54 Gy) requires an extended course. Given the low α/β ratio of meningiomas (~ 3.76 Gy), we investigated hypofractionated stereotactic radiotherapy (hypo-FSRT; 40 Gy/10 fx) as an alternative. METHODS: This retrospective STROBE-compliant study (2016-2023) included imaging-confirmed pONSM patients (PS 0-2) unsuitable for or refusing surgery/CFRT. Hypo-FSRT delivered via gamma-ray system with ther

Pitfall of fat grafting in pituitary surgery.

Acute visual loss following transsphenoidal pituitary surgery is rare but potentially reversible if promptly identified and treated. Migration of fat graft into the sella is an infrequently recognised but treatable cause. A woman in her early 60s presented with pituitary acromegaly and underwent endoscopic endonasal transsphenoidal surgery for a pituitary macroadenoma. Postoperatively, she developed sudden bilateral visual loss. MRI revealed optic chiasm compression by migrated fat graft used for sellar reconstruction. Emergency re-exploration and decompression led to complete restoration of v

Improved visual outcomes after endoscopic endonasal transsphenoidal surgery for pituitary macroadenomas: a single-arm meta-analysis.

BACKGROUND: Visual impairment due to optic chiasm compression is a frequent complication of pituitary macroadenomas and substantially impacts quality of life. Reported postoperative visual field (VF) results after endoscopic endonasal transsphenoidal surgery (EETS) vary across series. We performed a single-arm meta-analysis to quantify postoperative VF outcomes after EETS for pituitary macroadenomas. METHOD: PubMed/MEDLINE, Scopus, and Web of Science were searched in accordance with PRISMA through September 2025. Eligible studies were prospective or retrospective clinical series of pituitary

Rescue of Suprasellar Metastasis of EGFR-mutant NSCLC by Daily Osimertinib Re-escalation With Filgrastim Support.

BACKGROUND/AIM: Managing central nervous system (CNS) metastases of epidermal growth factor receptor (EGFR)-mutated non-small-cell lung cancer can be challenging if dose-limiting toxicities prevent adequate drug exposure. CASE REPORT: We report the case of a 70-year-old woman with a suprasellar metastasis abutting the optic chiasm for whom radiotherapy was contraindicated. Although treatment with 80 mg osimertinib once daily was begun, recurrent grade 3 neutropenia required a dose reduction to 40 mg every other day. During alternate-day dosing, the suprasellar lesion progressed despite contin
