Coexistence of acromegaly and raised intracranial pressure: insights from a rare case report and literature review.

BACKGROUND: Idiopathic intracranial hypertension (IIH) is a rare condition characterized by increased cerebrospinal fluid pressure without an intracranial mass, mainly affecting overweight women of reproductive age. It commonly presents with headache, visual disturbance, and papilledema. Acromegaly is an uncommon endocrine disorder caused by a growth hormone-secreting pituitary adenoma. CASE PRESENTATION: A 24-year-old female presented to an ophthalmology clinic complaining of blurred vision and generalized headache. Clinical examination revealed coarse facial features and a deep voice, raisi

Secondary chiasmal herniation after pituitary adenoma treatment: a diagnostic pitfall.

A man in his 30s with a prior diagnosis of presumed primary open-angle glaucoma presented with progressive visual deterioration and headaches. Initial optic disc cupping and the established glaucoma diagnosis suggested glaucomatous optic neuropathy; however, visual field defects respecting the vertical meridian and optical coherence tomography changes inconsistent with glaucoma prompted neuroimaging, which revealed a large invasive pituitary macroadenoma compressing the optic chiasm, later confirmed as a mixed prolactin-secreting and growth hormone-secreting tumour. Following medical treatment

French guidelines for the diagnosis and management of neuromyelitis optica spectrum disorder.

Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune disease of the central nervous system, characterized by severe attacks. Optic neuritis is the most common clinical manifestation, leading to a rapid and often severe loss of vision, which is sometimes bilateral, with frequent sequelae and a risk of very low visual acuity in the long-term. Magnetic resonance imaging (MRI) reveals extensive lesions of the optic nerve, mostly involving the posterior part of the nerve and sometimes extending to the optic chiasm. Myelitis manifests as longitudinally extensive and centrally located spin

Congenital aplasia of the optic chiasm presenting as optic nerve hypoplasia with no other ocular or systemic comorbidity.

We introduce a case of an infant girl who presented to our paediatric ophthalmology department with no behavioural response to visual targets and horizontal nystagmus. An ocular assessment revealed isolated bilateral small optic discs prompting further investigation. Her biochemical and endocrinological profiles were normal but a head MRI identified a total absence of the optic chiasm and optic tracts. Electrodiagnostic tests confirmed the diagnosis of aplasia of the optic chiasm. Our patient had no systemic comorbidities and has shown excellent milestone progression. She is now in a mainstrea

Retinopathy of Prematurity Is Associated With Reduced Thalamic Volume and Impaired Neurodevelopment. A Cohort Study.

BACKGROUND: Retinopathy of prematurity (ROP) is a neurovascular disorder that may affect not only the retina but also the brain structures involved in visual processing. The relationship between ROP severity and the volumetric development of these regions remains unclear. METHODS: A total of 157 children aged 6-16 years, born preterm (24-36 weeks gestational age), were assessed. Perinatal data and comorbidities were collected. Brain MRI (3 T) data were analysed using FreeSurfer to obtain the volumes of the thalamus, fusiform gyrus, cuneus, lingual gyrus, lateral occipital gyrus, optic chiasm

Visual field loss in pituitary and hypothalamic disorders.

Pituitary and hypothalamic tumours and disorders cause visual impairment due to proximity to structures critical for vision: intracranial optic nerves, chiasm and optic tracts. Visual dysfunction arising from compression of these structures typically has a profound impact upon quality of life. Visual fields should not be interpreted in isolation but in the context of a complete neuro-ophthalmic evaluation. Optical coherence tomography (OCT), specifically peripapillary retinal nerve fibre layer (pRNFL) and macular ganglion cell layer (mGCL), quantifies the degree of structural damage of the ant

Complicated allergic fungal rhinosinusitis: diagnostic complexities and management strategies.

A man in his 60s presented with diplopia, headache and facial numbness. Examination demonstrated a unilateral right abducens nerve palsy which rapidly progressed to a bilateral abducens palsy, partial right third nerve palsy and bilateral facial numbness. Nasendoscopy and imaging findings suggested an erosive fungal lesion centred on the sphenoid sinus. He underwent aggressive clearance with functional endoscopic sinus surgery, which revealed pan-sinus allergic fungal disease with marked bony erosion of the sphenoid, exposing the dura and optic chiasm. Corticosteroids and systemic antifungals

Pituitary Stalk Enlargement With Panhypopituitarism and Arginine Vasopressin Deficiency in Lung Adenocarcinoma: Metastatic Infundibular Disease or Paraneoplastic Hypophysitis?

Paraneoplastic endocrine syndromes are well-recognised complications of lung malignancies, specifically small-cell lung cancer. The majority of patients with panhypopituitarism associated with lung cancer result from pituitary metastasis. In those with an enlarged pituitary stalk without obvious pituitary metastasis presenting with a combination of panhypopituitarism and arginine vasopressin deficiency (AVP-D), metastatic pituitary stalk infiltration should be considered rather than a paraneoplastic immune-mediated hypophysitis. We report a case of a 67-year-old gentleman with newly diagnosed

Disentangling crossing fibers with advanced dMRI methods reveals bundle-specific degeneration across the visual system in asymmetric glaucoma.

Diffusion magnetic resonance imaging (dMRI) is a non-invasive neuroimaging technique that enables in vivo assessment of white matter microstructure and is highly sensitive to tissue alterations associated with disease. Although substantial evidence links diffusion-derived metrics to underlying white matter tissue properties, the presence of complex within-voxel axonal configurations complicates their biological interpretation. Several methods have been proposed to assess diffusion properties of individual crossing axonal populations, but their validation and clinical applicability remain limit

Adrenocorticotropin-secreting pituitary macroadenomas: expanding the clinical spectrum.

Cushing disease (CD) secondary to adrenocorticotropin (ACTH)-secreting pituitary macroadenomas is uncommon, accounting for 10% to 15% of cases. We describe 3 patients from South Africa who presented with overt clinical features of hypercortisolism and considerable systemic complications, including hypertension, diabetes mellitus, and osteoporosis. Biochemical testing confirmed ACTH-dependent hypercortisolism with hypopituitarism and mild hyperprolactinemia consistent with stalk effect. Magnetic resonance imaging demonstrated pituitary macroadenomas, measuring 10 mm or greater, with parasellar

Unravelling Cholera's Footprints in India: A Data-Driven Investigation of its Burden, Risk, and Strain Variability.

INTRODUCTION: Binge Eating Disorder (BED) has high lifetime prevalence rates, low treatment success rates, and high rates of treatment dissatisfaction, early discontinuation of care, and recurrence. Complementary and integrative health (CIH) interventions (non-mainstream practices used with conventional approaches for whole-person treatment) hold potential to overcome many treatment barriers and improve BED treatment outcomes. Some CIH interventions have empirical support for use in eating disorders. However, little is known about the current state of CIH use in BED. METHODS: This mixed-metho

Comparative immunopathology and clinical features of MOGAD and AQP4-IgG NMOSD in children: A scoping review.

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is a distinct immune-mediated demyelinating central nervous system disorder, frequently presenting as optic neuritis in children and adolescents. Conversely, aquaporin-4 antibody-positive neuromyelitis optica spectrum disorder (AQP4-NMOSD) is an astrocytopathy characterized by more severe relapses and greater residual visual impairment. This scoping review, conducted according to PRISMA-ScR and JBI methodology, mapped evidence from 23 studies identified across PubMed and Scopus over the last decade. The objective was to ex

Pituitary involvement in ANCA-associated vasculitis: Case report and literature review.

The antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is an rare autoimmune condition characterized by inflammatory cell infiltration causing necrosis of small blood vessels. Pituitary involvement is uncommon in AAV and it can occur at any point during AAV. The main clinical manifestations are central diabetes insipidus and panhypopituitarism. In this article, we describe 2 cases of pituitary involvement of AAV, pituitary dysfunction presented as initial symptom in 1 patient and developed over the course of the diseases in the other patient. Both of them had positive ANCA

Adapting the Daily Inventory of Stressful Events for SMS Delivery: Recommendations for Research.

Vernal keratoconjunctivitis (VKC) is a chronic and potentially sight-threatening disease. Topical corticosteroids (Cs) seem to be the only effective treatment for this condition, although severe side effects may occur owing to their prolonged use. More recently, cyclosporine (Cyc) eye drops have been reported as a valid alternative, but so far such treatment has only been successfully experimented for a short time and in small numbers of patients. The aim of our study is to evaluate the long term safety and efficacy of topical cyclosporine eye drops in children suffering from VKC. Over a perio

A Rare Case of Oligosecretory Multiple Myeloma With Intracranial Plasmacytoma.

Multiple myeloma (MM) is a hematologic malignancy characterized by monoclonal plasma cell proliferation within the bone marrow. According to current International Myeloma Working Group (IMWG) criteria, true non-secretory multiple myeloma (NSMM) is defined by the absence of detectable monoclonal protein even on serum free light chain (FLC) assay, unlike oligosecretory multiple myeloma (OSMM), in which electrophoresis and immunofixation are negative but the FLC κ/λ ratio is pathological - making diagnosis particularly challenging by conventional techniques. The association of OSMM with an intrac

Iturin A disrupts Cryptococcus neoformans membrane integrity, attenuates virulence, and synergizes with amphotericin B.

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

Optical coherence tomography angiography reveals insights into complementary vascular and neurodegenerative mechanisms in multiple sclerosis.

Optical coherence tomography angiography quantifies retinal microvasculature biomarkers, offering insights into neurovascular mechanisms underlying brain damage in multiple sclerosis. This study evaluated these potential mechanisms of neurodegeneration by examining associations between optical coherence tomography and optical coherence tomography angiography metrics, brain volumes and clinical outcomes in people with multiple sclerosis. This cross-sectional study included multiple sclerosis patients from a prospective cohort. Participants underwent optical coherence tomography/optical coherenc
