Exploring the Symptoms and Treatment of a Pituitary Macroadenoma: A Case Report.

Pituitary macroadenoma (PMA) are significant intracranial tumors that can cause local mass effects and systemic endocrine disruptions. This case report illustrates an unusual case of PMA in a patient presenting with an altered level of consciousness subsequently coupled with headache and throat pain. A 78-year-old man reported to the emergency room with a history of an altered level of consciousness followed by headache and throat pain for two days. He had a past medical history of diabetes mellitus (DM), hypertension (HTN), severe hypothyroidism, ischemic heart disease, cholelithiasis, and ri

Microsurgical Management of Large Superior Hypophyseal Artery Aneurysm Presented with Visual Impairment.

Superior hypophyseal artery aneurysms are rare. Symptomatic superior hypophyseal artery aneurysm commonly presents with subarachnoid hemorrhage, uncommonly with cranial nerve deficits, and very rarely with visual impairment. Microsurgical management of such aneurysms is quite complex considering the anatomical structures in the paraclinoid region. A 43-year male presented with decreased visual acuity in both eyes. The patient was diagnosed with left supraclinoid partially thrombosed unruptured aneurysm with mass effect over optic chiasm and optic nerves on magnetic resonance imaging of the bra

Diagnostic Challenge: The Case of a 70-Year-Old Man Who Presents With Progressive Vision Loss and Headaches.

A 70-year-old presents with 6 weeks of progressive bilateral vision loss and headaches. He was also recently treated for presumed sciatica. His exam on admission is notable for poor visual acuity (no light perception), optic nerve atrophy bilaterally with swelling and disc hemorrhage on the left eye and left foot drop. MRI orbits reveals diffusion restriction and enhancement of bilateral optic nerves extending into the optic chiasm. He undergoes bilateral temporal artery biopsy due to concern for giant cell arteritis and high-dose steroids with no improvement in vision and both biopsies are no

Spatiotemporal Characterization of Changes in the Respiratory Tract and the Nervous System, Including the Eyes in SARS-CoV-2-Infected K18-hACE2 Mice.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the causative agent of coronavirus disease 2019 (COVID-19), is known to affect multiple organ systems, including the respiratory tract and nervous and ocular systems. This retrospective study aimed to characterize the spatiotemporal distribution of viral antigen and associated pathological changes in the nose, lungs, brain, and eyes of K18-hACE2 mice intranasally infected with SARS-CoV-2. Using histology and immunohistochemistry, tissues were examined at 3, 6, and 7/8 days post-infection (dpi). In addition, lung and brain tissues we

Stereotactic Radiosurgery to the Nucleus Accumbens: A Dosimetric Feasibility Study.

INTRODUCTION: Addiction remains a critical issue in the United States, and options for individuals suffering from treatment-refractory addiction are limited. The nucleus accumbens (NAc) is a region of interest for both invasive and non-invasive interventions aimed at treating addiction. Stereotactic radiosurgery (SRS) is an established modality for managing benign conditions. This study seeks to establish the technical feasibility of targeting the NAc with linear accelerator-based SRS. METHODS: A single de-identified image set from a patient previously treated with linac-based SRS for a benig

Brain iron accumulation in neurodegenerative disorders: Does air pollution play a role?

BACKGROUND: Both excess brain Fe and air pollution (AP) exposures are associated with increased risk for multiple neurodegenerative disorders. Fe is a redox-active metal that is abundant in AP and even further elevated in U.S. subway systems. Exposures to AP and associated contaminants, such as Fe, are lifelong and could therefore contribute to elevated brain Fe observed in neurodegenerative diseases, particularly via nasal olfactory uptake of ultrafine particle AP. These studies tested the hypotheses that exogenously generated Fe oxide nanoparticles could reach the brain following inhalationa

Stereotactic Ablative Radiotherapy for Delayed Retrobulbar Metastasis of Renal Cell Carcinoma: Therapeutic Outcomes and Practical Insights.

We present a rare case of delayed retrobulbar and adrenal metastases from renal cell carcinoma (RCC), diagnosed 5.5 years after radical nephrectomy. The patient exhibited symptomatic orbital involvement, with imaging revealing a hypervascular retrobulbar mass and an incidental right adrenal lesion, indicative of an oligometastatic state. Owing to the patient's refusal of surgical resection, stereotactic ablative radiotherapy (SABR) was delivered to the retrobulbar lesion at a total dose of 40 Gy in five fractions, concurrently with immune checkpoint inhibitor therapy. Treatment planning priori

Can optic nerve morphology in children with autism spectrum disorder be associated with atypical visual-sensory behaviors?

INTRODUCTION: The aim of this study is to investigate, using magnetic resonance imaging (MRI), the optic nerve diameter, morphometric characteristics of the optic chiasm (OC), volumes of the lateral, third, and fourth ventricles, as well as the volumes of the corpus callosum (CC) and choroid plexus (CP) in children with autism spectrum disorder (ASD), and to compare these findings with those of a typically developing (TD) control group. Additionally, the study seeks to evaluate the impact of these neuroanatomical parameters on autism symptom severity and sensory sensitivity. METHODS: This stu

Axonal degeneration and retinal neurovascular dysfunction in the TgF344-AD rat model of Alzheimer's disease.

BackgroundRecent research suggests that retinal changes occur at early stages of Alzheimer's disease (AD). However, gaps remain in understanding the detailed pathophysiological processes and their molecular underpinnings.ObjectiveThis study investigates alterations in the retinal neurovascular unit (NVU) and associated proteomic changes in the TgF344-AD rat model to determine how these changes relate to brain and known AD pathology.MethodsTransgenic rats and age-matched wild-type rats were studied. Proteomic analysis was conducted to identify changes in critical signaling pathways across the r

Skin and Macular Carotenoids in Relation to Brain Structure and Function in School-Aged Children.

BACKGROUND: Carotenoids are anti-inflammatory plant pigments that, when consumed, deposit in human tissues (e.g., macula, skin, brain). Although limited data indicate associations between carotenoids and brain structure and function in adults, this relationship has not been previously examined in children. OBJECTIVES: This research aimed to study skin and macular carotenoids in relation to brain volume, global functional connectivity, and task-based brain function among school-aged children. METHODS: Cross-sectional data were collected from 47 children aged 7-13 y (X¯ = 10.84 ± 1.88). Skin c

First report of caruncular invasive apocrine adenocarcinoma with intracranial extension treated with a multimodal approach: a case report and literature review.

A 51-year-old male presented with a rapidly growing right caruncular nodular lesion. Excisional biopsy performed elsewhere showed an apocrine adenocarcinoma of the caruncle with positive margins. Clinically, there was no obvious residual disease, mapping biopsies were negative, and cryotherapy of the surgical base was performed. The tumor recurred both anteriorly in the lower fornix and deeper in the anterior orbit a year later. These were surgically excised with adjuvant radiotherapy (60 Gy in 30 fractions). Unfortunately, the tumor continued to recur posteriorly to progressively first involve

Intensity-Modulated Proton Therapy for an Unresectable Giant Non-functioning Pituitary Adenoma: A Case Report and Literature Review.

Giant non-functioning pituitary adenomas (NFPAs) often extend into or compress critical structures, such as the optic nerves, brainstem, and cavernous sinus, frequently making complete surgical resection difficult or unfeasible and posing significant therapeutic challenges. Radiotherapy (RT) is crucial for unresectable or residual disease, yet conventional techniques may struggle to deliver adequate doses while sparing organs at risk (OARs). Proton beam therapy (PBT), particularly intensity-modulated proton therapy (IMPT), offers potential dosimetric advantages, such as superior conformity and

Coexistence Tumor of Craniopharyngioma and Pituitary Neuroendocrine Tumor: A Case Report and Literature Review.

Acromegaly most commonly results from excess growth hormone (GH) produced by pituitary neuroendocrine tumors (PitNETs). Craniopharyngioma (CP) is an uncommon suprasellar tumor characterized by cystic change and calcification. The true coexistence of PitNET and CP is rare and poses diagnostic and operative challenges, particularly when lesions are spatially separated. A 72-year-old man was referred for a pituitary mass and clinical features suggestive of acromegaly (enlarged extremities, prominent supraorbital ridge, and jaw enlargement) without headache or visual complaints. MRI demonstrated t

Craniopharyngioma.

Introduction Adamantinomatous craniopharyngioma (ACP) is a significant source of morbidity in the pediatric brain tumor population. It predominantly arises from the parasellar space. The tumors proximity to key vital structures often makes gross total surgical resection challenging and sometimes clinically inadvisable. Compounding the problem, adjunct therapies are not yet capable of providing a definitive cure. Recent preclinical research has made significant progress towards elucidating the mutagenic drivers of ACP, however much work remains to translate this into safe and effective treatmen

Regional Position and Axonal Environment Shape Astrocyte Morphology in the Mouse Optic Projection.

Optic neuropathies encompass a diverse group of disorders that result in damage to the optic nerve and lead to vision loss. Despite the initial site of damage often being injury specific, we have an incomplete understanding of how differential regional susceptibility arises. Recent studies have implicated astrocytes in this phenomenon in mice, showing regional transcriptional heterogeneity in optic nerve astrocytes in response to glaucoma and optic nerve crush. Because astrocytes provide structural support and protection from tissue deformation, we examined the length of the optic nerve for as

Atypical Teratoid/Rhabdoid Tumor of the Sellar Region in an Elderly Patient: A Case Report and Literature Review.

Atypical teratoid/rhabdoid tumor (ATRT) is a rare and highly malignant central nervous system (CNS) neoplasm, most commonly diagnosed in children and only infrequently reported in older adults. Classified as a World Health Organization (WHO) grade IV tumor, ATRT carries a poor prognosis and typically requires aggressive, multimodal treatment. Management in elderly patients is particularly challenging due to comorbidities, a lack of evidence-based guidelines, and heightened risk of treatment-related toxicity. We report the case of a 75-year-old woman with hypertension and hyperlipidemia who pre

Exploring the Shared Genetic Architectures Between Primary Open-Angle Glaucoma and Visual Pathway Regions in the Brain.

PURPOSE: To investigate the genetic relationships between primary open-angle glaucoma (POAG) and major visual pathways in the brain to better understand the neurological biology of glaucoma, which may facilitate the discovery of neuroprotective drug targets. METHODS: We assessed the relationship between POAG and the volumes of five visual pathway regions using genetic correlation and polygenic risk score (PRS). We further used Mendelian randomization (MR) to investigate the causal relationships. In addition, we used genome-wide association study (GWAS)-pairwise analysis to identify genomic se

Altered intra- and inter-network functional connectivity in pituitary adenomas with chiasmal compression.

BACKGROUND: Pituitary adenomas (PA) frequently compress the optic chiasm, leading to visual field defects (VFDs) and potentially affecting the function of brain networks. This cross-sectional study aimed to investigate alterations in brain networks in PA patients with chiasmal compression using resting-state functional magnetic resonance imaging (rs-fMRI). METHODS: In this study, 35 PA patients with chiasmal compression and 33 healthy controls (HCs) were enrolled and underwent rs-fMRI scanning. Network-Based Statistic (NBS) and large-scale network analyses were performed. Additionally, correl
