Reframing neural decussation: A vision-based theory for contralateral control.

Neural decussation, the systematic crossing of sensory and motor pathways at the midline, is one of the most prominent features of the vertebrate nervous system, yet its functional rationale has remained unresolved. This work proposes the Visual-Origin Theory of Neuronal Decussation, which argues that contralateral organization arises as a direct consequence of visual field optical geometry. Light rays from the right visual hemifield physically cross the midsagittal plane before reaching the temporal retina, and so do rays from the left hemifield in the opposite direction. The decussation of n

Surgical management and visual outcomes of optic-pathway cavernous malformations: A comprehensive literature review of 80 reported cases (1979 - 2025).

OBJECTIVE: Cavernous malformations (CMs) of the optic chiasm, optic tract, and hypothalamus are exceedingly rare. Most patients present with acute or progressive visual loss, and surgery aims to maximize visual recovery while minimizing neural manipulation. We describe an updated review of all reported operated optic-pathway cavernomas to clarify surgical strategies and visual outcomes. METHODOLOGY: PubMed, MEDLINE, and Google Scholar were searched with the terms optic pathway cavernoma, optic chiasm cavernoma, hypothalamic cavernoma, and surgical outcomes. Eighty published cases (1979 - May

Ophthalmoplegia and vision loss in extrapulmonary tuberculosis with bilateral cavernous sinus involvement.

We report the case of a 41-year-old woman who initially presented with headache, right hemifacial hypoesthesia, and a right abducens palsy. Initial external investigations failed to identify an etiology, and cerebral MRI was considered normal, although later review revealed subtle meningeal thickening. Approximately two months later, she developed a right pupil-involving oculomotor nerve palsy, followed by progressive visual loss in the right eye, a right inferior quadrantanopia, and a left abducens palsy. Repeat MRI demonstrated a new contrast-enhancing lesion in the right cavernous sinus, va

Adaptive Treatment Strategy: Adjuvant Chemoradiotherapy for a Complex SMARCB1 (INI-1)-Deficient Sinonasal Carcinoma With Intracranial Extension.

SWI/SNF-related matrix-associated actin-dependent regulator of chromatin subfamily B member 1 (SMARCB1) (integrase interactor 1)-deficient sinonasal carcinoma (SDSNC) is a rare and aggressive malignancy of the head and neck. It is characterised by the absence of nuclear SMARCB1 expression and typically presents at advanced stages due to non-specific symptoms resembling benign conditions such as nasal polyps or sinusitis. This report describes a 64-year-old male who presented with a sphenoidal sinus mass causing bony erosion and intracranial extension, associated with headaches and diplopia. In

Unmasking Photophobia and Blepharospasm Mechanisms: The Role of Optic Chiasm and Trigeminal Nerve Compression in a Giant Pituitary Adenoma Case.

Blepharospasm, characterized by abnormal blinking and sensory hypersensitivity such as photophobia and ocular pain, is thought to arise from pathological sensorimotor integration. We report a rare case of a male teenager with a growth hormone-secreting giant pituitary adenoma compressing both the optic chiasm and bilateral trigeminal nerves. Initially, the patient presented with visual disturbances and bitemporal hemianopia, without photophobia or ocular pain. Following partial tumor resection, which relieved compression of the optic chiasm but not the trigeminal nerves, he developed severe ph

Management challenges of postpartum rhinocerebral mucormycosis following spontaneous vaginal delivery in a resource-constraint setting: A case report and review of literature.

INTRODUCTION AND IMPORTANCE: Rhinocerebral mucormycosis (RM) is a rare and severe condition caused by filamentous fungi, characterized by infection of the nose, paranasal sinuses, and brain. It is the most common and fatal clinical form of mucormycosis, accounting for 50 % of reported cases. RM is seldom reported during the postpartum period. This case report presents a rare instance of RM following childbirth and reviews the most recent literature, highlighting the limited cases documented globally and even fewer within Africa. The aim is to contribute to the existing body of knowledge and pr

Pituitary apoplexy in the setting of severe headache and unconsciousness.

Pituitary apoplexy is a rare but life-threatening condition caused by acute ischaemia or haemorrhage within the pituitary gland, often occurring in pre-existing adenomas. We present a case of an unconscious patient with acute headache, initially managed empirically for suspected meningitis. Investigations, such as cerebrospinal fluid analysis, revealed initially xanthochromia, prompting evaluation for subarachnoid haemorrhage. Magnetic resonance imaging and pituitary function testing identified a pituitary macroadenoma with optic chiasm compression and hypopituitarism, leading to successful tr

Gut microbial and functional alterations lead to metagenomic signatures for midgut neuroendocrine tumor patients and for carcinoid syndrome.

SUMMARY: We report the case of a 41-year-old Japanese woman with visual field disturbances during late pregnancy. At 39 weeks of gestation, she was diagnosed with bitemporal hemianopsia at the ophthalmology department. An MRI revealed a symmetrical pituitary gland enlargement, compressing the optic chiasm. An emergency cesarean section was performed immediately, resulting in the delivery of a male infant weighing 3,112 grams. Laboratory tests indicated low serum free thyroxine (T4), thyroid-stimulating hormone (TSH), cortisol, luteinizing hormone, and follicle-stimulating hormone. The patient

Tucatinib, trastuzumab, and capecitabine with stereotactic radiosurgery in patients with brain metastases from HER-2 positive breast cancer (TUTOR): Study protocol for a multicenter phase 1 clinical trial.

BACKGROUND: Approximately 30-50% of patients with HER-2 + breast cancer develop brain metastases (BM). Stereotactic radiosurgery (SRS) is frequently used for local disease control. Tucatinib, an oral HER-2-selective tyrosine kinase inhibitor, has been demonstrated to be safe and efficacious in HER-2 + breast cancer in combination with capecitabine and trastuzumab. The synergy between these three drugs and radiation may enhance DNA damage and increase apoptosis. We hypothesize that this chemoradiation regimen in patients with HER-2 + breast cancer BM (BCBM), is safe and provides superior long-t

Hypophysitis in a HIV-Positive Patient With a Syphilis Latent Infection.

BACKGROUND: Hypophysitis is a rare inflammatory disorder of the pituitary gland that may result from primary autoimmune disease or secondary causes such as infection. In people living with HIV, coinfection with Treponema pallidum can precipitate pituitary inflammation, leading to panhypopituitarism, visual field compromise, and neurological symptoms. METHODS: Written informed consent was obtained from the patient, with all personal identifiers removed, in compliance with the CARE guidelines. PRESENTATION: A 50-year-old man with known HIV infection and a history of latent syphilis presented t

Tuberculous optochiasmatic arachnoiditis.

A 24-year-old woman developed multiple cerebral tuberculomas during treatment for central nervous system tuberculosis, requiring a protracted course of antituberculous therapy. Four years later, while on a second course of antituberculous therapy for relapsed disease, she developed deteriorating vision and further cerebral tuberculomas with inflammation; this was attributed to optochiasmatic arachnoiditis. She was treated at different stage with intravenous corticosteroids, second-line antituberculous therapy, infliximab and thalidomide. Her visual deficits initially improved but subsequently

Challenges in the Management of Silent Lactotroph Pituitary Adenoma.

Silent lactotroph pituitary adenomas are rare tumors that clinically resemble nonfunctioning macroadenomas but are identified histologically by prolactin immunoreactivity. We report a 29-year-old woman with progressive visual loss caused by a large sella-suprasellar mass compressing the optic chiasm and invading the cavernous sinus. Initial endocrine evaluation was unremarkable, and magnetic resonance imaging (MRI) suggested a nonfunctioning adenoma. She underwent urgent surgical decompression, and histology confirmed a sparsely granulated lactotroph adenoma. Despite significant residual tumor

Secondary xanthogranulomatous hypophysitis mimicking a pituitary macroadenoma: a case report.

Hypophysitis is an extremely rare inflammatory disease that can mimic the clinical and radiological features of a pituitary adenoma. In this case report, we describe a 45-year-old woman with secondary xanthogranulomatous hypophysitis (XGH) who presented with signs of a pituitary macroadenoma. The patient complained of headaches, visual impairment, and amenorrhea-galactorrhea syndrome. Her physical examination was normal. Laboratory investigation revealed corticotropin, thyrotropin, and gonadotropin deficiencies. She also had low visual acuity in her right eye and an altered visual field. Pitui

Is the OCT a predictive tool to assess visual impairment in optic chiasm compressing syndrome in pituitary macroadenoma? A prospective longitudinal study.

Visual dysfunction is a prevalent symptom in patients with non-functioning pituitary macroadenoma (NFPM); the role of OCT in such patients has not been yet determined. This is a prospective longitudinal observational study over a period of 6 years, on 20 patients presenting a radiological compression of the optic chiasma without visual acuity (VA) and visual field (VF) disturbances. The primary endpoint was to evaluate the impact of NFPA on neuro-axonal loss by measuring RNFL thickness using OCT at inclusion (T0), 12 months (T1), 24 months (T2), and 36 months (T3), respectively. The secondary

Optochiasmatic Cavernomas: Updated systematic review and proposal of a novel classification with surgical approaches.

Cavernomas are histologically benign vascular malformations found at different sites in the brain. A rare site for such cavernomas, however, is the anterior optic pathway, comprising the optic nerve, chiasma, and optic tract-called optochiasmatic cavernomas (OCC). These lesions usually present with sudden onset or progressive vision loss, headache, and features mimicking pituitary apoplexy. In this paper, we describe a case of OCC operated at our center. We carry out an updated review of literature depicting cases of OCC, their clinical presentation, management, and postoperative complications

Optic Chiasm Glioma in an Older Adult Patient.

We present a case of an adult patient experiencing progressive visual loss. An initial presentation was concerning for neuromyelitis optica with optic chiasm involvement. However, persistent contrast enhancement observed in follow-up brain and orbit images raised suspicion for optic tract malignant neoplasm. Histopathological evolution of optic nerve biopsy confirmed the diagnosis of an optic chiasm glioma. The patient was then referred to oncology for chemotherapy.
