[Familial paroxysmal ataxia sensitive to acetazolamide. 3 cases in a new European family].

UNLABELLED: This family is the tenth reported world-wide and the second reported in Europe. Two of 3 brothers, their mother and probably their maternal grandmother were affected. CLINICAL FEATURES: paroxysmal bouts of ataxia and dysarthria, nystagmus of permanent gaze, the dominant autosomal transmission, the benign nature of the affection and the remarkable efficacy of acetazolamide were typical of familial paroxysmal ataxia. Specific points emphasized are the presence of a mild pyramidal syndrome as a permanent finding in 2 of these patients, and the anomalies of visual (longer lag period)

Convergent strabismic amblyopia in cats.

Experiments were carried out to determine the effects of different types of experimental strabismus on the acuities of retinal ganglion cells. Six kittens were raised from twenty-one days of age with an esotropia surgically induced by myectomy of the lateral rectus muscle and a large portion of the superior oblique muscle. The results are compared with those, previously reported, from five other cats also made esotropic, but by tenotomy of the lateral rectus. All animals tested behaviourally were amblyopic in the strabismic eye. For square wave gratings, the visual acuities were 1.0 to 2.5 cyc

[Does proprioception of the extrinsic eye muscles participate in equilibrium, vision and oculomotor action?].

Extrinsic eye muscles respond to both proprioceptive and visual impulses. Histology of ocular proprioceptive receptors shows them to be qualitatively and original, neuromuscular bundles are found in primates, and so called "palisade sensory nerve endings" in felines. Golgi's osteotendinous receptors are apparently absent. The number of muscle receptors is high and their proportion in relation to muscle weight even more so. The afferent pathway for this sensory system initially accompanies the motor fibers of oculomotor III, IV, ans VI nerves. Some fibers continue with these nerves to the brain

[Visual field defects in hydrocephalus].

Eight patients representing visual field defects associated with hydrocephalus are reviewed. Seven cases had aqueductal stenosis and one had congenital communicating hydrocephalus. We found five cases of defects in visual field typical of a chiasmal or optic nerve lesion: (1) inferior altitudinal hemianopia with inferior nasal quadrantanopia in the opposite eye; (2) inferior binasal quadrantanopia; (3) unilateral inferior nasal depression; (4) unilateral temporal defect; (5) bilateral central scotoma. In these cases CT demonstrated moderate or marked symmetrical dilatation of the third and lat

The value of visual evoked potential as a screening test in neurofibromatosis.

Pattern reversal visual evoked potential (VEP) test was performed in 30 asymptomatic patients with neurofibromatosis (NF). All patients had normal visual acuity, visual fields, and ophthalmoscopic examination results. Pattern reversal VEP was abnormal in eight patients (26%). Seven of these patients had computed tomographic scans with reformated orbital views. Orbital computed tomographic scans were abnormal in six patients and in each case showed enlargement of the optic nerve on the side of VEP abnormality. Our data indicate that pattern reversal VEP is a valuable screening test in asymptoma

An animal model of myopia.

Myopia develops in macaque monkeys when their lids are surgically fused at birth and kept closed for one year. This experimental refractive error has many features in common with human myopia: It is caused by progressive axial elongation of the eye, is often accompanied by fundus changes, and can only be induced before eye growth has been completed. Myopia does not develop in animals raised in the dark; thus, it is triggered by an alteration of the visual input and is presumably mediated by the nervous system. In Macaca arctoides, atropine administration prevents abnormal eye elongation, and t

Traumatic third nerve palsy.

Twenty patients with a traumatic third nerve palsy had sustained a closed head injury with prolonged loss of consciousness in a high-speed deceleration accident. Sixteen were male, and the average age was 25 years. Seven had skull or facial fractures, 15 damage to the anterior visual pathways, and 16 other permanent neurological damage. Nineteen developed the misdirection/regeneration syndrome. Thirteen had strabismus surgery, and an area of binocular single vision was enlarged or achieved in three.

[Phenomenology of the Klüver-Bucy syndrome in man].

The KBS was first described on the basis of experimental bilateral temporal lobectomy in monkeys. Extrapolating any syndrome from monkey to man is frought with controversy, but the 53 patients with KBS reported here are significant for the completeness of its expression and the opportunity to follow its evolution. The rarity of the human KBS is attested to because less than 200 cases have been reported in the literature. The KBS in man, presented here as a clinical entity, includes the following symptoms: (1) Increased oral activity: a strong tendency to examine all objects orally, putting obj

Fiber-fiber interaction and tectal cues influence the development of the chicken retinotectal projection.

The development of the retinotectal projection has been studied by a new experimental approach combining antibodies against the nerve cell adhesion molecule (NCAM), and techniques for mapping neuronal pathways using rhodamine B isothiocyanate (RITC) crystals. Anti-NCAM Fab', which specifically inhibits neurite fasciculation, was injected into the eye cup of 4-day-old chicken embryos. After 4-6 days of development, a small RITC crystal was placed on the neural retina to stain selectively axons arising from a localized region. One day later the retina, optic nerve, and tectum were examined and t

Visual evoked potential abnormalities in Charcot-Marie-Tooth disease and comparison with Friedreich's ataxia.

Pattern-reversal visual evoked potentials (VEPs), recorded in 15 visually asymptomatic patients fulfilling the clinical and electrophysiological criteria of Charcot-Marie-Tooth disease (CMTD), were abnormal in 5 and possibly abnormal in another 3. Five patients showed a prolongation of P100 latency, one a reduction of amplitude and one a possibly abnormal "scotomatous" waveform. In 9 cases abnormalities were detected on neuro-ophthalmological examination. These were poorly correlated with VEP abnormalities, except for patients with 2 or more clinical eye signs. Relative central scotomata were

Peripheral and central sensory nerve conduction in Charcot-Marie-Tooth disease and comparison with Friedreich's ataxia.

Somatosensory evoked potentials were recorded in response to stimulation of the median nerve at the wrist and the elbow in 14 cases of Charcot-Marie-Tooth disease (CMTD). Cervical and cortical latencies were used to derive conduction times and velocities over peripheral and central segments of the pathway. Sensory conduction velocities between the wrist and the elbow were distributed bimodally (12-27 m/s and 36-70 m/s), but did not correspond with the bimodality of motor conduction velocity values in 4 cases. Three patients had severely slowed sensory conduction in one arm but only moderate sl

Low-contrast letter charts as a test of visual function.

Visual pathway disorders can cause visual loss that is not detected by the Snellen test: visual sensitivity to coarse detail may be depressed, even when visual sensitivity to fine detail is unaffected. Sinewave grating test targets can detect such hidden visual loss. However, electronic apparatus for generating sinewave gratings is expensive, while the inexpensive Arden plates provide no check on the patient's accuracy. We have tested 10 patients and 10 control subjects with a set of five letter charts (including the standard Snellen chart). These letter charts were of different contrasts, nam

Hereditary neuropathy with upper motor-neuron, visual pathway, and autonomic disorders.

A 42-year-old man had progressive distal weakness and muscle atrophy, stocking-type sensory loss, upper motor-neuron and visual pathway lesions, and dysautonomia. Electrodiagnostic tests revealed a generalized sensorimotor peripheral neuropathy that largely involved axons. Low recumbent and upright norepinephrine levels implied a peripheral autonomic defect. Sural nerve biopsy showed mild abnormalities of medium and small size fibers. The patient's mother and two brothers were also affected. Other causes of peripheral motor, sensory, and autonomic failure were eliminated. This kinship does not

The 20/20 eye in multiple sclerosis.

Using clinical and electrophysiologic measures, we evaluated the visual pathway of patients who had multiple sclerosis, 20/20 Snellen acuity, and no history of optic neuritis. Delayed latencies were found in the transient visual evoked potentials (VEPs) of 38% of the patients, and interocular latency differences were abnormal in 67%. Contrast VEPs were abnormal in 46%. Psychophysical determinations of contrast sensitivity were abnormal in 78%. Only 17% of the patients had dyschromatopsia, 36% had afferent pupillary abnormalities, and 59% had optic nerve pallor or nerve fiber layer loss. Psycho

The one-and-a-half syndrome--a unilateral disorder of the pontine tegmentum: a study of 20 cases and review of the literature.

The one-and-a-half syndrome is a clinical disorder of extraocular movements characterized by a conjugate horizontal gaze palsy in one direction plus an internuclear ophthalmoplegia in the other. The syndrome is usually due to a single unilateral lesion of the paramedian pontine reticular formation or the abducens nucleus on one side (causing the conjugate gaze palsy), with interruption of internuclear fibers of the ipsilateral medial longitudinal fasciculus after it has crossed the midline from its site of origin in the contralateral abducens nucleus (causing failure of adduction of the ipsila

Clinical and experimental evidence that the pattern electroretinogram (PERG) is generated in more proximal retinal layers than the focal electroretinogram (FERG).

A TV monitor was used to evoke either a pattern ERG to a contrast-reversing checkerboard (PERG), or a focal ERG to alternate increases and decreases of luminance of the blank screen within a bright surround (FERG). Both responses are small (approx 2 microV) and fast (approx 50 msec to peak) and are similar in several other properties. However, they differ in timing and respond differently to changes in contrast. Each frame of a TV picture evokes a "raster ERG," even though the screen is blank. The response is focal and specific to a small central strip of the screen. It is simpler to record th

Effects of human aging on patterns of local cerebral glucose utilization determined by the [18F]fluorodeoxyglucose method.

The [18F]fluorodeoxyglucose (FDG) scan method with positron emission computed tomography was used to determine patterns of local cerebral glucose utilization (LCMRglu) in 40 normal volunteer subjects aged 18 to 78 years. Throughout all the studies, each subject was quiet, without movement, with eyes open and ears unplugged, exposed only to ambient room light and sound. For the entire group, whole brain mean CMRglu was 26.1 +/- 6.1 mumol 100 g-1 min-1 (mean +/- SD, n = 40). At age 78, mean CMRglu was, on the average, 26% less than at age 18, an alteration of the same order as the variance among

Evoked potentials in the evaluation of visual field defects due to chiasmal or retrochiasmal lesions.

We used monocular full-field and hemifield pattern reversal stimulation to study visual evoked potentials (VEPs) in 20 patients with confirmed defects of the visual pathways. In 10 of 12 patients with homonymous field defects, it was possible to lateralize the brain lesion but not to predict the site of the lesion within the hemisphere. In four of eight patients with bitemporal field defects, VEPs suggested involvement at the chiasmal level. In the remaining cases, VEPs provided no correlation with perimetric and radiologic findings. We found that VEPs usually corroborated other diagnostic stu
