Chromatic and luminosity processing in retinal disease.

Color vision loss can be an early sign of eye disease; in many retinal disorders the loss precedes any change in visual acuity. Noninvasive psychophysical methods allow factoring out of preretinal, receptoral, and postreceptoral (neural) components of the color vision change. A loss of chromatic but not achromatic sensitivity occurs for diabetics; the loss is selective for pathways subserving blue-sensitive photoreceptors. Both chromatic and achromatic pathways are altered in glaucoma and senile macular degeneration; the most marked change in central serous choroidopathy is loss of sensitivity

Vibrotactile stimulation for intersystemic reorganization in the treatment of apraxia of speech.

Intersystemic reorganization in the treatment of apraxia of speech involves pairing fragments of a speech motor program with internal cues generated by some other, more intact system. Gestural reorganization, pairing speech with limb gestures, somehow provides an organizational framework for the proper sequencing of motor speech movements. This paper briefly develops some hypotheses suggesting that afference resulting from limb movements rather than a central movement program is involved in gestural reorganization, and that vibrotactile stimulation may have advantages over visual or auditory m

Sensory and non-sensory visual disorders in man and monkey.

The posterior third of the cerebral cortex in monkeys consists of a patchwork of visual areas in each of which there is a 'map' of the retina. The details of the 'map' vary considerably from one area to another and one notable variation concerns the optimal visual feature to which the cells respond. Orientation, disparity, colour and movement are emphasized in separate areas that appear to be concerned with sensory analysis. Their existence and the possibility that brain damage is occasionally restricted chiefly to one such area may explain the rare highly selective visual sensory impairments

Visual handicap in children.

Vision is the primary mode of learning and if it is absent or impaired, other pathways must be stimulated. The range of appropriate services available to visually handicapped children is explained in relation to the school run by the Royal Victorian Institution for the Blind in Melbourne. A period of residential education may be needed for children with multiple impairment. Those whose handicap is primarily visual can usually live at home and have their education integrated with that of their normal peers. Home-based services and parent support systems for preschool and kindergarten age childr

Meningioma and the ophthalmologist: diagnostic pitfalls.

In seven patients with intracranial meningioma whose presenting signs and symptoms were ophthalmologic the underlying problem was initially misdiagnosed. Three patients had sphenoidal meningiomas with compression of the anterior visual pathways, but the initial diagnoses were acute optic neuritis, chronic optic neuritis and glaucoma. Two other patients had large frontal meningiomas causing in one case unilateral pain and swelling of the upper lid plus ptosis and hypotropia, and in the other case bilateral frontal morning headaches and intermittent blurring of vision in one eye; they were thoug

The deafferentation syndrome in genetically blind rats: a model of the painful phantom limb.

The hypothesis was tested which states that the somatic deafferentation syndrome is a visually prompted response to sensorimotor loss. The dorsal roots, C5-T2, were bilaterally cut in a strain of rats known to be genetically blind. These complete dorsal rhizotomies left the forelimbs totally anesthetic, analgesic and paretic. Contact and visual placing reactions were absent, and responses to pinprick or pinch were absent. Self-mutilation limited to the distal digits appeared on the first or second postoperative days and then progressed proximally. The forelimbs were symmetrically affected, and

[Diagnosis and differential diagnosis of hysteric amblyopia using psychophysical and electro-ophthalmological examination methods (author's transl)].

In three patients with psychogenic amblyopia the computerized visual field, the ERG, the flash light (H-EP) and pattern reversal stimulated (M-EP) cortical potentials were recorded. Differential diagnosis between psychogenic amblyopia on the one hand, and functional amblyopia and organic lesions of the retrobulbar visual pathways on the other was shown to be possible. In the cases with organic retrobulbar lesions of visual pathways, visual field defects and pathologic H-EP's and M-EP's were found. In functional amblyopia only the M-EP responses are diminished in amplitude. In psychogenic ambly

Eye movement abnormalities in rod monochromacy.

Eye movements were studied quantitatively using electro-oculography in seven patients with rod monochromacy. Attention was given to eye movement abnormalities that could help to differentiate rod monochromacy from other forms of congenital nystagmus. Horizontal nystagmus in center gaze had pendular and jerk waveforms with much lower amplitude than that in patients with other forms of congenital nystagmus and visual acuity of 20/200-20/400. Slow build-up of slow component velocity over many seconds and directional asymmetry of optokinetic nystagmus (OKN) were observed during monocular optokinet

Detection of visual defects using the contrast sensitivity function.

In this chapter the theoretical reasons were outlined and clinical data summarized as to why spatial contrast measurements can reveal visual losses that are not uncovered by testing visual acuity, no matter which optotype one uses or how carefully the measurement is made. Spatial contrast sensitivity measurements may disclose different types of contrast losses in patients with different lesions but with identical visual acuity. The relationship of different types of spatial contrast sensitivity losses (plotted as visuograms) to specific location or cause of lesions is not yet clear. Patients w

Visual evoked potentials in two forms of hereditary spastic paraplegia.

Visual evoked potentials (VEPs) were used to study two forms of hereditary spastic paraplegia. We recorded the VEP in 6 homozygotes and 5 obligate heterozygotes in one family with recessively transmitted disease and in 5 persons with dominantly transmitted hereditary spastic paraplegia. The VEPs in the affected persons from both families were similar; both had delayed conduction times and changes in the wave form. The VEPs of the obligate heterozygotes were indistinguishable from those of the unaffected control subjects. Comparison of our findings with those of other disease processes that aff

Sensory-specific and fractional disorders of recent memory in man. II. Unilateral loss of tactile recent memory.

Three patients were found to have unilateral tactile recent memory loss on examination. In two patients the loss occurred in the right hand and was accompanied by a disorder of verbal recent memory; in the third patient the loss occurred in the left hand and was not accompanied by any other disturbance in recent memory. An acute unilateral lesion, either structural or functional, in the medial temporal lobe contralateral to the tactile recent memory disturbance was the common denominator in all three cases. The proper methods for diagnosing a unilateral tactile disturbance of recent memory and

The fiber connections of the temporal lobe with emphasis on the rhesus monkey.

The temporal lobe has three functionally distinct areas which have minimal interconnections within the lobe, but extensive connections with other parts of the brain. The superior temporal gyrus contains primary and association auditory areas. Inferotemporal cortex is exclusively a visual association area. The temporal pole is involved in social behavior. In addition, the superior temporal sulcus functions as a sensory integration area. From a review of the literature, a schema is proposed for organizing the reciprocal connections of the three functional areas into a series of loops which follo

Annual review of section on ocular disease: diseases of the optic nerve, tracts, and visual cortex.

The ophthalmic literature dealing with diseases of the optic nerve, tracts, and visual cortex was reviewed for the period November 1977 to December 1978. Fifty-nine papers of interest to optometrists have been abstracted. Main areas of interest include: role of axoplasmic transport in production of disc edema and cotton-wool spots; ocular effects of local and systemic vascular disturbances; congenital abnormalities of the eye alone and in association with other bodily malformation; field testing, including an ingenious ophthalmoscopic method which can lead to detection of very small scotomas;

Visual evoked potentials and the visuogram in multiple sclerosis.

Visual evoked potential (VEP) latency to a sinusoidal grating pattern was measured in each eye of 103 patients with multiple sclerosis (MS) and compared with results in a control group of 56 patients hospitalized for other neurological conditions. Of the 50 patients classified as having definite MS, 90% showed prolonged latency (over 131 msec) in one or both eyes. In each eye of 24 of the MS patients, psychophysical measurement of the detectability of grating patterns was obtained. This test was abnormal in 11 of 13 patients with definite MS, 3 of 4 with probable MS, and 5 of 7 with possible M

Opto-chiasmatic arachnoiditis in brothers.

Six patients (3 couples of brothers) with adhesive arachnoiditis of the opto-chiasmatic cistern, surgically verified, are presented. Only one couple of brothers offered a possible family history of a similar condition in one maternal uncle. The visual damage had almost constantly a sudden onset and was predominantly of the axial type. No gross pathology, other than the arachnoiditic involvement of the chiasmal cistern and alterations of the anterior optic pathways, was found at surgery. Pneumocisternoencephalographic findings did not prove constantly reliable for a preoperative diagnosis of th

A colour-dependent abnormality in human visual detection of stimulus motion and spatial structure.

We have studied the wavelength dependence of visual responses in a single human subject, M.W., who has an unusual visual defect. It is shown that for red stimuli, both spatial resolution for grating patterns and detection of target movement are grossly abnormal. On the basis of these and other [8] experimental data, we argue that M.W.'s response pattern implies interdependent central visual processing of colour, movement and spatial structure. We examine this conclusion in relation to recent anatomical and electrophysiological findings.

Abnormal retino-geniculate and geniculo-cortical pathways in several genetically distinct color phases of the mink (Mustela vison).

Several genetically distinct color phases of mink, which all show an abnormal reduction of pigment in the retinal pigment epithelium and which also show abnormalities of the retinofugal pathways, have been studied. Autoradiographic methods have been used to demonstrate the retino-geniculate pathways, and retrograde degeneration or the retrograde transport of horseradish peroxidase has been used for the geniculo-cortical pathways. The retino-geniculate abnormality is mild in some of the color phases and extremely severe in others, but within any one color phase the variability is relatively low

The afferent pupillary defect in acute optic neuritis.

Twenty-two patients with acute optic neuritis were studied by the techniques of infrared pupillometry and visual evoked responses (VER) to pattern reversal. A relative afferent pupillary defect was found in all cases and the magnitude of this defect was found to be related to the amplitude, but not to the latency, of the VER. During follow-up the afferent defect was found to remain persistently abnormal while other methods of clinical evaluation could not demonstrate abnormality reliably. The amplitude of the VER also remained low.

Pattern shift visual evoked responses. Two hundred patients with optic neuritis and/or multiple sclerosis.

Fifty-one patients with clinically pure optic neuritis (ON) and 149 with possible, probable, or definite multiple sclerosis (MS) were tested with pattern shift visual evoked responses (PSVER) and compared with a group of 43 normal subjects. Attention was paid to response latency, intereye latency difference, as well as differences in amplitude or duration of the major positive peak (P100). Abnormal PSVER cannot be recorded from everyone with confirmed ON. Abnormal responses were recorded from 91% of all patients (including those with MS) who had a history of ON, 57% of all MS patients, and 36%

Visual evoked responses and visual symptoms in multiple sclerosis.

Absolute latency, interocular difference in latency, and waveform of visual evoked responses (VER) to checkerboard reversal stimuli recorded from the midline of the skull were studied in 104 multiple sclerosis patients, 25 to 50 years of age, classified according to visual symptomatology. Group 1 had strong evidence of past or present optic neuritis. Patients with blurring of vision, diplopia, and undefined visual complaints were assigned to group 2, while group 3 contained patients with no visual symptoms but suspected diagnosis of multiple sclerosis on other grounds.
