Measurements of visual evoked potentials in Parkinson's disease.

Visual evoked potentials elicited by reversing grating patterns were recorded in 35 patients with Parkinson's disease and in 26 controls. The average latency of evoked potentials in patients with Parkinson's disease exceeded by two standard deviations the average of the age-matched control group of other neurological patients. Over two-thirds of all patients with Parkinson's disease had abnormal latency. In these patients latency did not correlate with age. In 5 patients the latency became less prolonged on levodopa therapy, suggesting that catecholaminergic pathways have either indirect or di

Neurophysiology of movement disorders.

The relation of the cortex, cerebellum, and basal ganglia and their participation in movement disorders is presented. Studies have shown that lesions to the cortex will produce contralateral paralysis while damage to the basal ganglia or cerebellum will result in movement abnormalities. These data provide support for the view that subcortical but not cortical structures initiate the control movement activity. It would appear that the basal ganglia and cerebellum receive signals from the auditory, visual, and somatosensory cortex integrate this information, and relay signals back to the motor c

[Chiasmatic syndrome--clinical value of the visual evoked responses (VER) to the diagnosis (author's transl)].

The VER is compared with that of other neuroophthalmological examinations in 25 patients presenting lesions of the anterior optic pathways (of which 18 were of an expansive nature). The authors believe that visual field plotting and VER are indispensable complementary examinations, giving results which orientate subsequent investigations towards a chiasmatic or prechiasmatic syndrome. In 15 cases of chiasmatic syndrome of tumoral origin (thus in 30 optic nerves), visual fields and VER showed early pathological changes. In originally unilateral forms, VER changes are usually bilateral even when

ENG analysis of 150 cases of posterior fossa disease.

Analysis of the ENG records of 150 patients with well substantiated neurologic disease involving the structure of the posterior fossa revealed a high incidence of diminished optic fixation inhibition (DOFI) of caloric induced nystagmus. DOFI is defined as less than 50 percent decrease of speed of the slow phase when eyes are open and fixated. The whole group was subdivided into seven subgroups according to type and location of disease and the incidence of DOFI was found to vary significantly in the different subgroups. The highest incidence was found in the group of cerebellar mass lesions and

Hemispheric disconnection syndrome with a "crossed avoiding" reaction in a case of Marchiafava-Bignami disease.

A clinicopathological study is presented of a case of Marchiafava-Bignami disease with a hemispheric disconnection syndrome, an association that does not appear to have been reported previously. Gross and microscopic examination of the brain revealed necrosis of the corpus callosum (sparing a small area in front of the splenium) and of the anterior commissure, cortical and subcortical infarction of the right lingual gyrus, diffuse cortical lesions of the laminar sclerosis type, and lacunae in the basal ganglia and the pons. The patient was unable to grasp objects presented to the right visual

Unilateral afferent pupillary defects in asymmetric glaucoma.

Unilateral relative afferent pupillary defects are associated with unilateral prechiasmal lesions affecting the retinomesencephalic pathway. Five patients with unilateral or asymmetric glaucoma and relative afferent pupillary defects are presented. The lack of descriptions of relative afferent pupillary defects in unilateral glaucoma may be explained by the frequency with which these patients are treated with miotics, the association of trauma, and the presence of anatomic alterations of the iris. It is important to distinguish unilateral afferent pupillary defects in glaucoma from those assoc

Evoked potential studies in neurological disorders.

Techniques for recording and analysing visual and somatosensory evoked reponses using an on-line PDP 11/40 computer have been developed and applied to a group of subjects with established or suspected multiple sclerosis as well as to patients with a variety of other lesions of the visual pathways, myoclonic epilepsy and functional neurological deficits. The most consistent responses were obtained using the pattern reversal visual evoked response and the spinal somatosensory evoked response, abnormal responses being found with both techniques in a significant number of patients with suspected d

Tumors of the optic nerve.

This is a review of primary and secondary tumors of the optic nerve. The emphasis is an optic nerve gliomas and meningiomas. Optic nerve gliomas are slowly growing astrocytic neoplasms of the anterior visual pathways, the majority of which occur within the first two decades of life, with equal sex incidence in about 1 of 200,000 patients presenting with eye complaints. The incidence is greater in neurofibromatosis. The typical presentation is visual impairment in a verbal pre-school child with optic canal enlargement and optic atrophy. An intraorbital location of the tumor leads to axial, irre

[Frontal cerebral cortex and photic epilepsy of the baboon Papio papio (author transl)].

It was discovered in 1966 that the senegalese baboon (Papio papio) exhibits a photosensitive epilepsy. This finding has led, among other work, to the neurophysiological study of this epilepsy. Although in some characteristics the baboon's photosensitive epilepsy differs from that of man, it can be considered that this animal presents a real model of essential epilepsy, for the study of the human disease. 2. In the baboon, the EEG disturbances triggered by intermittent light stimulation at 25 Hz appear first at the level of the frontal cortex (area 6). At this level, recordings of single unit d

Neurophysiology of photically induced epilepsy in Papio papio.

This chapter critically reviews arguments supporting the role of the frontal cortex in light-sensitive epilepsy of the baboon Papio papio in the light of the most recent neurophysiological research. In particular, it is known that spontaneous or ILS-induced paroxysmal discharges, as well as generalized seizures, originate in the frontorolandic cortical region. In this region during ILS, neurons behave in the same manner as hyperexcitable neurons in focal epileptogenic lesions of animals and man. Aso, section of the corpus callosum causes deterioration or even destruction of the synchronization

Observations on cortical blindness and on vascular lesions that cause loss of recent memory.

Two long-surviving cases of cortical blindness are described, one total and the other total except for detection of sudden transitions from light to darkness and darkness to light. Both suffered from severe defect of recent memory, which lasted a month in one, and till death after nearly six years in the other. One patient survives. Necropsy findings on the other are given.

Homonymous hemianopia in multiple sclerosis. With report of bilateral case.

A patient with multiple sclerosis and bilateral retrochiasmal visual field defects is reported. Homonymous field defects are rare in multiple sclerosis despite the frequency of pathological involvement of the retrochiasmal visual pathways. A higher incidence might be found with a higher index of suspicion and careful visual field testing with qualitative confrontation technique. Other reasons for the infrequent clinical detection of retrochiasmal lesions are considered. Such lesions may exist without demonstrable defect. This may be explained by anatomical factors, for example, fibre arrangeme

Use of the Pulfrich pendulum for detecting abnormal delay in the visual pathway in multiple sclerosis.

(1) Pulfrich's pendulum has been developed as a method for detecting abnormal delay in the visual pathway of patients with suspected multiple sclerosis. (2) Forty-one normal subjects gave a mean interocular latency difference of 2-4 +/- 1-8 msec on Pulfrich's pendulum. This mean +2 SD (6 msec) was taken as the limit of normal interocular latency difference by this method. (3) The test was assessed on 58 patients. Eighteen of these had definite multiple sclerosis, 9 had probable multiple sclerosis, and 8 had possible multiple sclerosis, according to clinical criteria. The remaining 23 patients

Portal hypertension in a patient with osteopetrosis. A case report with discussion of the mechanism of portal hypertension.

PURPOSE: Somalia faces a substantial burden of avoidable eye disease, while access to primary eye-care services remains uneven and optometry training capacity is limited. This rapid communication reports the pattern of ocular conditions and service needs identified in a large service-based eye-care dataset from Mogadishu and outlines a practical implementation framework for strengthening the optometry workforce in Somalia. METHODS: This study was conducted as a retrospective review of routine service data from 2,890 patients evaluated during community- and hospital-based eye-care programs at
