Moxifloxacin-induced Abducens Nerve Palsy: A Rare Neurological Complication During Mycobacterium avium Complex Lung Disease Treatment.

Mycobacterium avium complex (MAC) lung disease requires prolonged multidrug therapy. We report the case of an 80-year-old woman with MAC lung disease who developed an isolated right abducens nerve palsy after approximately nine months of treatment with moxifloxacin, azithromycin, and ethambutol. The patient presented with horizontal diplopia, and ophthalmological evaluation confirmed sixth cranial nerve palsy. Although ethambutol-induced optic neuropathy was initially considered, clinical findings were not consistent with this diagnosis. Given the temporal relationship with moxifloxacin exposu

Mitochondrial Myopathy in the Oculoplastic Practice.

Ptosis is a common ocular complaint in the elderly population. While most cases are often due to involutional changes in the eyelid, neurogenic, mechanical or myogenic diseases should remain as important differential diagnoses. We report a case of a 75-year-old Chinese man who presented with chronic progressive ptosis and was initially managed as presumed seronegative ocular myasthenia gravis (MG). Despite positive bedside findings suggestive of MG, serological and electrophysiological investigations were inconclusive, and there was no clinical response to pyridostigmine. The diagnosis was ach

Progressive Cranial Polyneuropathy and Myelopathy as the Initial Manifestation of Leucine Zipper 4-Positive Paraneoplastic Syndrome in Previously Undiagnosed Metastatic Testicular Seminoma.

A 50-year-old man without a significant medical history developed progressive double vision from a partial right third nerve palsy, as well as right-sided hyperreflexia and decreased temperature sensation in his right hand. Initial contrast MRI of the brain revealed enhancement of the right oculomotor nerve. Cerebrospinal fluid and serologic studies were negative or normal. Over the next 6 weeks, he developed a left sixth nerve palsy and right-sided facial sensory changes, followed by a partial left third nerve palsy and right foot drop. The patient later disclosed a 2-year history of painful

Small molecules rescue pathogenic POLGA aggregation and restore its functions in a mouse model of PEO.

POLGA, the catalytic subunit of the mitochondrial DNA polymerase, is essential for mitochondrial DNA (mtDNA) replication and maintenance. Mutations in POLGA cause progressive external ophthalmoplegia (PEO), but the underlying pathogenic mechanisms remain unclear. Here, we show that the mitochondrial E3 ligase MITOL hyperubiquitylates PEO-associated POLGA mutants at lysine 1060, triggering conformational changes and formation of insoluble aggregates that impair their mitochondrial import and consequently compromise mtDNA replication and mitochondrial homeostasis. To identify compounds that disr

Recurrent Pituitary Neuroendocrine Tumors: Anatomical Predictors of Gross-Total Resection with a Proposed Pituitary Resectability Score.

OBJECTIVE: Revision endoscopic endonasal transsphenoidal surgery (EETA) for recurrent pituitary neuroendocrine tumors (PitNET) is challenging due to altered anatomy and scarring. We prospectively evaluated extent of resection, clinical outcomes, and anatomical predictors of resectability, and assessed a proposed Pituitary Resectability Score (PRS). METHODS: Thirty-two patients with recurrent or residual PitNET underwent EETA between January 2023 and June 2025. Extent of resection (EOR) was classified as gross-total, near-total, subtotal, or partial and correlated with anatomical characteristi

Diplopia in Extramedullary Multiple Myeloma: Clinical Features, Management, and Outcomes at a Tertiary Cancer Center.

BACKGROUND: Extramedullary multiple myeloma (EMM) involving the skull base and orbit is an uncommon cause of diplopia. Prior reports, including our earlier series of 7 patients, are limited to small case series. The aim of this study was to characterize the clinical features, lesion patterns, management, and outcomes of EMM-associated diplopia in a large single-center cohort. METHODS: We identified 250 patients with International Classification of Diseases, 10th Revision codes for multiple myeloma and diplopia who were evaluated at the MD Anderson Cancer Center between 2016 and 2025. Dental Management of a Child With Mitochondrial DNA Deletion Syndrome (Kearns-Sayre Syndrome): A Case Report.

BACKGROUND: Kearns-Sayre syndrome (KSS) is a rare mitochondrial DNA deletion disorder characterized by progressive external ophthalmoplegia, pigmentary retinopathy, and cardiac conduction abnormalities. Dental management may be complicated by developmental enamel defects, high caries burden, limited cooperation, and anesthetic considerations related to multisystem mitochondrial disease. Published dental case reports focused specifically on pediatric KSS remain extremely sparse. CASE DESCRIPTION: We describe the staged dental management of a 10-year-old girl with genetically confirmed KSS and

Lateral Orbitotomy for Trigeminal Schwannomas: Surgical Technique, Outcomes, and Limitations.

BACKGROUND AND OBJECTIVE: Trigeminal schwannomas (TSs) are rare, mostly benign tumors that exhibit an indolent clinical course. The lateral orbitotomy approach (LOA), with or without endoscopic assistance, has emerged as a novel skull base route, particularly for accessing lesions confined to the middle cranial fossa. Herein, we elucidate the advantages and limitations of the LOA for the treatment of TSs. METHODS: All TS cases that underwent LOA through a lateral canthus incision during the past decade in our department were retrospectively reviewed. The operative technique was detailed, and

Cranial Nerve Six Palsy Occurring in PreEclampsia: A Case Report.

Isolated palsy of cranial nerve VI, also known as the abducens nerve, is a condition rarely seen in the obstetric population; however, there is a growing body of literature that suggests an association with pre-eclampsia and hypertensive disorders of pregnancy. We present the sixteenth case documenting this association involving a 30-year-old G1 admitted for induction of labor at 39 weeks gestational age (WGA) subsequently diagnosed with pre-eclampsia with severe features and, after an otherwise uncomplicated vaginal delivery, developed an isolated abducens nerve palsy. While similar cases rev

Under Pressure: Increased Intracranial Pressure in Infants and Children. Presented at the 2024 AOC/AACO/AAO Sunday Symposium.

Intracranial hypertension (ICH) in children refers to increased pressure within the skull. The skull normally contains the brain, blood vessels, meninges, and cerebrospinal fluid (CSF). Abnormalities in any of these intracranial structures may cause ICH. The differential diagnosis for pediatric ICH includes brain tumors; intracranial hemorrhages; intracranial infections; craniosynostosis; disorders of CSF production, clearance, or abnormal CSF contents; and pseudotumor cerebri syndrome (PTCS), which may be primary (also known as idiopathic intracranial hypertension [IIH]) or secondary. Isolated traumatic abducens nerve avulsion in the cisternal segment: mechanism, diagnosis and neurosurgical treatment.

The abducens nerve’s long intracranial course makes it highly vulnerable to injury during traumatic brain injury. While traditionally attributed to skull base fractures, high-resolution MRI has identified avulsion in cistern as a significant, yet underrecognized, mechanism, which can occur even without adjacent fractures. The study retrospectively analyzed 31 patients with traumatic abducens nerve palsy and radiologically suspected or surgically confirmed cisternal segment avulsion. Injuries were classified via MRI as complete or partial/suspected avulsion. Patients received different treatmen

Concurrent Presentation of Idiopathic Intracranial Hypertension and Bilateral Optic Perineuritis in a Young Boy: A Rare Clinical Entity.

Idiopathic intracranial hypertension (IIH) is a rare neurological syndrome characterized by increased intracranial pressure (>28 cm H₂O in children), in the absence of intracranial space-occupying lesions, and with normal cerebrospinal fluid (CSF) composition. Optic perineuritis (OPN) is an orbital inflammatory disease confined to the optic nerve sheath. We report a case of concurrent IIH and bilateral OPN in a previously healthy child. A 13-year-old male, with normal body mass index, presented with diplopia, visual blurriness without ocular pain, and occipital headache. Neurological examinati

The Differential Diagnosis of Diplopia.

BACKGROUND: Diplopia is a common symptom of interdisciplinary significance that can lead to marked impairment of function. Its causes range from harmless to life-threatening conditions that must be precisely distinguished. METHODS: For this CME review article, a PubMed search was conducted in an interdisciplinary collaboration of ophthalmologists and neurologists. RESULTS: Systematic history-taking and a structured clinical examination are the basis for topographic-anatomical diagnosis and the differentiation of peripheral and central causes. It is important to distinguish monocular from bin

Spontaneous rupture of an arachnoid cyst with subdural hygroma formation: a rare case report and literature review.

BACKGROUND: Arachnoid cysts are congenital, cerebrospinal fluid-filled sacs, predominantly asymptomatic and often detected incidentally. Spontaneous rupture of arachnoid cysts is an exceedingly rare complication, particularly in pediatric patients, and may present with acute neurological symptoms, including cranial nerve palsies. CASE PRESENTATION: We report a 7-year-old male with a large, left middle fossa arachnoid cyst, initially asymptomatic and followed conservatively for five years. The patient developed resistant headache, vomiting, and right sixth cranial nerve palsy. Neuroimaging rev
