Anterior segment OCT, Schlemm's canal, and carotid-cavernous fistula: Reflections on a clinical case.

We rerport a 69-year-old man who presented with diplopia. He had a conjunctival injection with corkscrew-vessels, proptosis, and sixth cranial nerve palsy in his right eye (RE), and was diagnosed with a right carotid-cavernous fistula (CCF). Intraocular pressure was within normal limits. Gonioscopic examination revealed a pinkish appearance of the trabecular meshwork, but the Schlemm's canal could not be identified on anterior segment optical coherence tomography (OCT). We discuss the potential usefulness and limitations of OCT in the assessment of the Schlemm's canal in CCF.

Incidence of idiopathic intracranial hypertension (IIH) in adults and children: a 14-year population-based study.

BACKGROUND: Idiopathic intracranial hypertension (IIH) is characterised by raised intracranial pressure in the absence of structural pathology and with normal cerebrospinal fluid composition. IIH is strongly associated with obesity and rising incidence has been reported in northern hemisphere countries. However, paediatric and Southern Hemisphere populations, including Australia, are under-represented in the existing literature. METHODS: We conducted a multicentre retrospective study of adults (≥ 18 years) and children (< 18 years) diagnosed with definite IIH (Revised Friedman criteria 2013)

Chronic oculomotor nerve palsy secondary to a sphenoid-cavernous sinus lesion in an adolescent: a case report and literature review.

BACKGROUND: Oculomotor nerve (cranial nerve III) palsy in children and adolescents is uncommon and typically suggests an underlying structural lesion rather than microvascular ischemia. Sphenoid sinus disease represents a rare cause of ocular cranial neuropathies and most frequently affects the abducens nerve. Consequently, isolated chronic oculomotor nerve palsy secondary to sphenoid sinus pathology is an unusual clinical presentation. CASE PRESENTATION: We report a 16-year-old male who presented with long-standing right ocular deviation and progressive limitation of eye movements beginning

Papilledema and related clinical and paraclinical visual assessment in cerebral venous and sinus thrombosis versus idiopathic intracranial hypertension.

INTRODUCTION: Cerebral venous and sinus thrombosis (CVST) is a life-threatening disorder which can present with papilledema. Idiopathic intracranial hypertension (IIH) also presents with papilledema, but is a relatively benign condition. Despite often presenting in a similar manner, CVST and IIH may lead to markedly different outcomes. We investigated if papilledema severity, related clinical features, and paraclinical visual assessment, can be used to differentiate between CVST and IIH-related papilledema, this way trying to improve a priori probability for both conditions at the bedside. Video-Oculographic Saccadic Fatigability Distinguishes Myasthenia Gravis From Unilateral Ocular Motor Cranial Nerve Palsy in Diplopia: A Prospective Diagnostic Study.

OBJECTIVE: To determine whether repetition-based saccadic fatigability measured with three-dimensional video-oculography (VOG) distinguishes myasthenia gravis (MG) from unilateral ocular motor cranial nerve palsy (CNP) in patients with diplopia. METHODS: In this prospective cross-sectional diagnostic study conducted from August 2022 to July 2024, consecutive patients with MG or unilateral third, fourth, or sixth CNP, and healthy controls underwent repetitive horizontal and vertical saccade testing (±°15 targets; 75 cycles at 0.25 Hz). Prespecified index metrics were percentage change in sacca

Concurrent Complete Spinal Cord Syndrome and Sixth Cranial Nerve Palsy in Advanced Adenoid Cystic Carcinoma: A Case Report and Literature Review.

Adenoid cystic carcinoma (ACC) with intracranial metastasis and sixth cranial nerve (CN VI) palsy, spinal metastases, and malignant spinal cord compression is an uncommon clinical entity. We present a case of a patient exhibiting this constellation of findings secondary to the parotid ACC. A 55-year-old man with no identifiable risk factors for ACC presented with a 10-year history of a left parotid tumor. He underwent two surgical resections and received one cycle of chemotherapy. Subsequently, his disease progressed locoregionally and systemically. He developed diplopia due to left sixth cran

Sixth Cranial Nerve Paresis Secondary to Intracranial Hypotension: Etiology, Clinical Characteristics, and Treatment.

Intracranial hypotension is a typically self-limiting or treatable condition characterized by orthostatic headache and neck stiffness. However, it may be severe and may be associated with neurological complications, particularly sixth cranial nerve palsy. We report two young women who presented to the emergency department with diplopia secondary to acute unilateral sixth cranial nerve paresis in the context of intracranial hypotension following anesthetic procedures. In both cases, clinical and radiological findings consistent with a cerebrospinal fluid (CSF) deficit were documented, including

Idiopathic Intracranial Hypertension in a Child with Marfan Syndrome: Clinical, Neuroimaging, and Biomarker Findings from a Case Report.

Marfan syndrome (MFS) is a connective tissue disorder classically associated with cardiovascular, musculoskeletal, and ocular manifestations. Neurological involvement is increasingly recognized and is most commonly related to spontaneous intracranial hypotension secondary to dural ectasia and cerebrospinal fluid (CSF) leakage. By contrast, idiopathic intracranial hypertension (IIH) is exceptionally rare in pediatric patients with MFS. We report the case of an 8-year-old girl with Marfan syndrome presenting with neck pain, diplopia, bilateral papilledema, and bilateral sixth cranial nerve palsy

Dengue-Associated Encephalopathy With Multiple Cranial Neuropathies and Multisystem Involvement in a Patient With Epilepsy: A Comprehensive Case Report With Longitudinal Multimodality Imaging.

Dengue fever is a common arboviral infection in tropical and subtropical regions. Neurological involvement is uncommon but increasingly recognized, and may mimic stroke, post-ictal deficits, thrombotic thrombocytopenic purpura, or sepsis-associated encephalopathy. We report a 45-year-old Lebanese woman with epilepsy who presented after a short febrile illness with dysuria initially treated as a urinary tract infection. She was later found in a post-ictal state with frothy oral secretions and urinary incontinence, febrile to 40°C and hypotensive to 70/50 mmHg. Neurological examination revealed

Multisystem infection with cavernous sinus thrombosis in seropositive rheumatoid arthritis: a clinico-radiological case report.

INTRODUCTION: Rheumatoid arthritis (RA) is a chronic systemic autoimmune disease primarily affecting synovial joints, but it may also manifest with extra-articular involvement, including neurological and ophthalmic complications. Cavernous sinus thrombosis (CST) is a rare yet life-threatening condition, most often secondary to infection, characterized by painful ophthalmoplegia, proptosis, and cranial nerve dysfunction. The coexistence of RA, disseminated infection, and CST is exceedingly uncommon and presents significant diagnostic and therapeutic challenges. This case highlights the importan

Clinical and Genotypic Spectrum of Twinkle-Related Disorders: Insights From a Multinational Cohort Study.

BACKGROUND AND OBJECTIVES: Twinkle, encoded by the TWNK gene, is a mitochondrial DNA helicase that unwinds the double helix of DNA during replication, playing a pivotal role in mitochondrial function. Twinkle-related disorders encompass a variety of genetic disorders characterized by mitochondrial dysfunction. Although several phenotypes have been described, the full clinical and molecular spectrum remains poorly defined. The aim of this study was to characterize the phenotypic and genotypic variability among multinational patients diagnosed with Twinkle-related disorders. METHODS: A retrospe

Volumetric brain analysis and associated retinal thinning in autosomal dominant optic atrophy patients.

INTRODUCTION: Dominant optic atrophy (DOA) is an inherited mitochondrial disorder characterized by retinal thinning and progressive visual loss. When accompanied by additional neurological or systemic features, such as progressive external ophthalmoplegia, myopathy, or deafness, it is classified as DOA-plus (DOA+). Although central nervous system involvement has been associated with cortical and cerebellar atrophy, specific regional patterns remain unclear. This study aimed to investigate cortical lobe alterations in DOA+ patients and examine the association between retinal thinning and struct

Exploring Outcome Measures for Mitochondrial Myopathies; Insights From a Longitudinal Study on TK2 Deficiency.

Thymidine kinase 2 deficiency (TK2d) is an ultra-rare autosomal recessive mitochondrial myopathy with variable presentations, including late-onset forms beginning after age 12. Unlike early-onset disease, the natural history of late-onset TK2d remains poorly defined. We conducted a prospective, single-centre natural history study of 11 untreated patients with late-onset TK2d over 24 months. The median age at symptom onset was 27.2 years. Clinical phenotypes included progressive myopathy (n = 7), chronic progressive external ophthalmoplegia plus (n = 2), and exercise intolerance (n = 2). Most p

Outcomes of kidney transplantation in three patients with single large-scale mitochondrial DNA deletion syndromes.

Single large-scale mitochondrial DNA deletion syndromes (SLSMDS) are a clinical continuum of three classic discrete clinical syndromes: Pearson marrow-pancreas syndrome, Kearns-Sayre syndrome, and chronic progressive ophthalmoplegia. Kidney manifestations, including chronic kidney disease with progression kidney failure has emerged as significant cause of morbidity and mortality in SLSMDS. Despite this recognition, reports of kidney transplantation in this population are limited. Here, we describe outcomes of kidney transplantation in three patients with SLSMDS and kidney failure over a 1-2.5-

Integrated molecular and clinical profiling of primary mitochondrial oxidative phosphorylation disorders in an Indian cohort: Insights from genetics, neuroimaging, and machine learning.

Primary mitochondrial disorders are clinically and genetically heterogeneous and remain underdiagnosed in resource-limited settings. We performed a retrospective observational study (March 2016- January 2024) at a tertiary neurology center in Eastern India to characterize the clinical, biochemical, neuroimaging, electrophysiological, and molecular features of suspected mitochondrial disease and to explore interpretable machine-learning approaches for syndromic stratification. Forty-eight patients from 42 unrelated families were classified as mitochondrial encephalomyopathy, lactic acidosis, an

Mutations Causative of CPEO Differentially Engage Innate Immunity Sensors.

Chronic Progressive External Ophthalmoplegia (CPEO) is a primary mitochondrial disorder (PMD) caused by mutations in nuclear genes encoding mitochondrial DNA (mtDNA) maintenance proteins. CPEO is characterized by mtDNA depletion and deletions, and patients primarily present with ocular and muscular features (isolated CPEO). However, additional encephalomyopathy, neurological complications, and Parkinsonism can drive a more severe disease form, CPEO-plus. The evolution from isolated CPEO to CPEO-plus remains poorly understood. Inflammatory and innate immune processes are emerging as strong dise
