Isolated Sixth Cranial Nerve Palsy as the First Presenting Sign of MOG Antibody-Associated Disease in a Three-Year-Old Child.

BACKGROUND: Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is a neurological, autoimmune, demyelinating disorder. Clinical phenotypes differ between adults and children, and MOGAD is more commonly observed in the paediatric population compared to the adult population. Cases of isolated cranial nerve palsies as initial clinical phenotypes of MOGAD are rarely reported in the literature. Here, we report a child who presented with an isolated left sixth cranial nerve palsy as the initial sign of anti-myelin oligodendrocyte glycoprotein antibody associated disease (MOGAD).

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Aberrant regeneration of third nerve combined with sixth nerve palsy in the setting of trauma: surgical results.

Introduction: Aberrant regeneration of the third nerve is a known entity after trauma. It is important to recognize signs of aberrant regeneration and keep lid aperture disparity in mind before choosing the surgical procedure in such cases. Surgical procedure in these cases is often customized on case-to-case basis. Methods: Two cases with combined third and sixth nerve palsy are described. Both the patients had synergistic adduction on elevation and were treated by a single muscle transposition. Results: Both patients were orthotropic in the primary position with no diplopia and a reduction i

Isolated third cranial nerve palsy in benign idiopathic intracranial hypertension.

Idiopathic intracranial hypertension (IIH) is marked by increased intracranial pressure without any accompanying evidence of clinical, imaging or laboratory findings of intracranial pathology. In addition to headache, nausea and vomiting, typical symptoms might also include diplopia, photophobia and blurred vision. Third nerve palsy is rarely linked to IIH, although sixth nerve palsy is reported in the majority of individuals with IIH. This paper describes the unique case of a woman patient in her 50s with solitary IIH-related oculomotor nerve palsy. Dilated pupils, ptosis and medial rectus pa

Skull base chordomas presenting with abducens nerve deficits: clinical characteristics and predictive factors for deficit improvement or resolution.

OBJECTIVE: Skull base chordomas (SBCs) often present with cranial nerve (CN) VI deficits. Studies have not assessed the prognosis and predictive factors for CN VI recovery among patients presenting with CN VI deficits. METHODS: The medical records of patients who underwent resection for primary chordoma from 2001 to 2020 were reviewed. Those presenting with CN VI palsy were identified. The extent of CN VI deficit was determined to be partial or complete based on the Scott-Kraft score. The change in deficit from baseline was recorded within 3 days of surgery and at the 6-month follow-up. The p

Is There an Association between Bell Palsy in Pediatric Patients and COVID-19?

Introduction  Bell palsy (BP) is an acquired, idiopathic facial palsy linked to lower motor neuron malfunction of the seventh cranial nerve. Several studies have identified BP as one of the many neuropathies that coronavirus disease 2019 (COVID-19) patients have developed, while other studies disagree. Objective  To study if there is an association between BP in pediatric patients and COVID-19, and to examine the pattern of recovery in all pediatric cases of BP during the COVID-19 pandemic. Methods  We performed a prospective cohort study on pediatric patients with acute onset unilateral facia

Carotid cavernous fistula complicated by contralateral abducens palsy and optic neuropathy.

Carotid-cavernous fistulas (CCFs) occur rarely in children. We present the second known case of a pediatric CCF complicated by contralateral cranial nerve palsies. A 9-year-old boy developed a left CCF after falling onto a plastic stick. He underwent stenting of the internal carotid artery and transvenous coil embolization of the left superior ophthalmic vein and left sides of the cavernous and circular sinuses. Six weeks after repair, he demonstrated right periorbital edema, proptosis, and engorged conjunctival vessels, a right abducens nerve palsy, and a right relative afferent pupillary def

Nicorandil-induced Isolated Abducens Nerve Palsy.

Nicorandil is commonly used to treat angina pectoris. However, the adverse neurological effects of nicorandil remain unclear. Herein, we report the case of a patient who presented with isolated abducens nerve palsy associated with the administration of nicorandil. A 74-year-old man presented to our hospital with abducens nerve palsy and headaches. The patient had started nicorandil treatment one week prior. No abnormalities were observed in the laboratory tests or neuroradiological findings. The patient recovered immediately after the discontinuation of nicorandil treatment. In cases of crania

Vascular Loop Syndrome As the Etiology of Abducens Nerve Palsy: A Case Report.

Vascular loop syndrome, characterized by arterial loops or kinks causing compression of cranial nerves, can lead to a range of secondary side effects conditional on vascular morphology. This etiology has made standardization of treatment difficult, as confounding factors may take precedence over more interventional treatment. This case seeks to illustrate how vascular loop syndrome, specifically a dolichoectatic vertebrobasilar artery, can lead to left abducens nerve palsy. Here we present an 82-year-old female with reports of diplopia, who was seen previously for sixth nerve palsy. Imaging re

Fractionated robotic radiosurgery for unfavorable nonfunctioning pituitary macroadenoma: 5-year outcomes from a single institution protocol.

OBJECTIVE: Nonfunctioning macroadenoma is a commonly diagnosed pituitary tumor. Resection is the favored treatment, with radiosurgery often utilized for residual or progressing disease. Long-term outcomes are established in the literature for single-fraction frame-based radiosurgery, but mature outcomes are lacking for fractionated frameless radiosurgery. We report our institution's 5-year efficacy and toxicity results for unfavorable nonfunctioning pituitary macroadenoma patients treated with 5-fraction robotic radiosurgery. METHODS: Between 2010 and 2020, patients who completed 5-fraction r

[Benign recurrent 6th nerve palsy].

Benign recurrent sixth nerve palsy is a rare phenomenon, and the majority of reported cases suffer from their first episode in early childhood. In this case report, we present a benign recurrent isolated left 6th nerve palsy in a nine-year-old boy. All episodes were precipitated by an upper respiratory tract infection and resolved within 2-3 weeks. Extensive investigations with two MRI scans, two lumbar punctures and several blood tests showed no underlying cause. This diagnosis of exclusion is worth considering in an otherwise well-appearing child, especially if precipitated by febrile illnes
