INTRODUCTION: The abducens nerve (sixth cranial nerve) is a motor nerve that innervates the lateral rectus muscle, playing a key role in ocular abduction. Palsy of this nerve leads to convergent strabismus and diplopia. Common causes include strokes, trauma, inflammation, and infections, though in some cases, the etiology remains undetermined. With the emergence of COVID-19, neurological manifestations such as cranial neuropathies, including abducens nerve palsy, have been reported.
CASE PRESENTATION: We present a case of a previously healthy 48-year-old male diagnosed with SARS-CoV-2 infecti

BACKGROUND Traumatic abducens nerve palsy can be caused by fractures around the Dorello's canal, direct injury at the dura entry point or within the Dorello's canal, or ischemia due to posterior meningeal artery damage. Recent imaging studies have also identified avulsion of the abducens nerve at its cisternal segment as a significant cause. CASE REPORT We present 2 cases of traumatic abducens nerve palsy due to avulsion at the cisternal segment. Preoperative thin-slice T2 magnetic resonance imaging indicated unclear visualization of the nerve at the cisternal segment in both cases. Surgical e

We report a rare case of clival metastasis from a high-grade neuroendocrine carcinoma (NEC) of unknown primary origin. A 71-year-old man, previously cured of prostate adenocarcinoma, presented with abdominal pain leading to the diagnosis of metastatic NEC (Ki67 >90%) involving the liver and bones. After three lines of chemotherapy and immunotherapy, he developed sudden diplopia and right abducens nerve palsy. A brain contrast-enhanced MRI revealed a clival mass compressing the sixth cranial nerve, which was undetectable on prior imaging. The patient received palliative radiotherapy and cortico

Vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic (VEXAS) syndrome is a late-onset autoinflammatory disorder caused by somatic mutations in the ubiquitin-attaching protein 1 (UBA1) gene. It primarily affects men aged over 50. We report a middle-aged man presenting with bilateral orbital inflammation, sixth nerve palsy, relapsing polychondritis, sensorineural hearing loss, possible vestibulopathy and a papulovesicular rash. He had macrocytic anaemia and elevated inflammatory markers, but normal autoimmune and infective screens. Imaging identified features of orbital pseudotumour. Genetic

INTRODUCTION: Alzheimer's disease, the most common form of dementia, is characterized by age-dependent amyloid beta (Ab) aggregation and accumulation, neuroinflammation, and cognitive deficits. Significantly, there are prominent sex differences in the risk, onset, progression, and severity of AD, as well as response to therapies, with disease burden disproportionately affecting women. Although menopause onset (i.e., perimenopause) may be a critical transition stage for AD susceptibility in women, the role of early ovarian decline in initial disease pathology, particularly key neuroinflammatory

Paralysis of the extraocular muscles can lead to complications such as strabismus, diplopia, and loss of stereopsis. Current surgical treatments aim to mitigate these issues by resecting the paralyzed muscle or transposing the other recti muscles to the paralyzed muscle, but they do not fully improve the patient's quality of life. Electrical stimulation shows promise, while requiring further in vivo experiments and research on various stimulation parameters. In this study, we conducted experiments on rabbits to stimulate the superior rectus (SR) muscles using different parameters and stimulati

Background: Vertebrobasilar artery occlusion (VBAO) is a life-threatening condition with often nonspecific symptoms, making early diagnosis challenging. Timely intervention is crucial, especially in cases involving distal vertebral artery stenosis. Case Report: A 65-year-old male presented with acute vertigo, dizziness, and visual disturbances, along with ipsilateral sixth cranial nerve palsy. His medical history included a treated abdominal aortic aneurysm, hypercholesterolemia, and hypertension. CT angiography (CTA) revealed an occlusion in the V4 segment of the right vertebral artery. OBJECTIVE: The hypoglossal-facial and cervical-hypoglossal nerve anastomoses (HFCHA) technique addresses severe facial palsy after cerebellopontine angle (CPA) tumor resection. However, postoperative facial function recovery and tongue dysfunction vary significantly among patients. This study identifies prognostic factors influencing these outcomes.
METHODS: We retrospectively followed up on 43 patients with facial palsy who underwent HFCHA for CPA tumor resection (July 2019-December 2022). The patients were divided into two groups based on their recovery of facial function or tongue function

UNLABELLED: Sixth nerve palsy in children is a rare yet clinically significant condition that can result in strabismus and developmental complications. The acute onset of sixth nerve palsy can lead to esotropia, a sudden inward deviation of the eye due to impaired abduction. While most cases resolve spontaneously, botulinum toxin injection is gaining ground as a therapeutic intervention, particularly in pediatric patients with persistent or severe misalignment.
OBJECTIVE: To evaluate the effectiveness of botulinum toxin injection in managing acute sixth nerve palsy in a 1-year-old child with

OBJECTIVE: Clival chordomas with brainstem invasion are extremely rare; only 5 cases have been reported in the literature. In such cases total tumor resection is associated with high intraoperative risks and is a notable challenge for the neurosurgeon. However, it is the quality of surgical treatment that influences the future prognosis, despite the efficacy of proton therapy.
METHODS: We describe the clinical case of a 39-year-old male patient with the sudden onset of double vision and left trochlear nerve paresis, who was diagnosed with a clival chordoma with brainstem invasion.
RESULTS: T

OBJECTIVE: To assess whether glucagon-like-peptide-1 receptor (GLP-1R) agonists could serve as a novel prophylactic treatment for migraine in patients with obesity.
BACKGROUND: Increased intracranial pressure (ICP) is speculated to play a role in migraine mechanisms, as chronic migraine and idiopathic intracranial hypertension without papilledema (IIWHOP) are often clinically indistinguishable. These striking similarities suggest a deep pathogenetic link between the two conditions posing the hypothesis that control of ICP may be helpful in migraine treatment. The GLP-1R agonists have been sho

The regulated secretion of von Willebrand factor (VWF) from Weibel-Palade bodies (WPBs) in endothelial cells is fundamental to hemostasis. This process relies on recruiting Rab GTPases and their effectors to the WPB membrane, with the guanine nucleotide exchange factor MAPK-activating death domain (MADD) playing a central role. Biallelic variants in MADD lead to a pleiotropic neurological and developmental disorder that can include bleeding abnormalities. This study investigates the impact of pathogenic MADD variants on VWF secretion using patient-derived endothelial cells. We isolated endothe

Strabismus surgery is required for some cases with abducens nerve palsy (ANP) without spontaneous resolution; however, whether the surgical corrective effect for ANP is similar to that of concomitant esotropia (ET) remains unclear. This study compared the 1-year surgical outcomes of recession and resection (RR) between patients with ANP (ANP group, 11 patients) and those with concomitant ET (ET group, 33 patients). The target angle of RR for ANP was added by 5 prism diopters (Δ) compared to concomitant ET. The 1-year corrective effect was relatively lower in the ANP group than in the ET group
