Ocular Myasthenia Gravis following strabismus surgery and presenting as refractory strabismus.

INTRODUCTION: Ocular myasthenia gravis is a protean disorder and can present with myriad disorders of ocular motility. The diagnosis may not be obvious at presentation and strabismus surgery has occasionally been performed with unexpected outcomes. Strabismus surgery can be performed on patients with ocular myasthenia gravis who have stable ocular deviations, although outcomes may vary. CASE REPORT: We report two patients who were operated for strabismus and were diagnosed as ocular myasthenia gravis later, when classic signs appeared. The first patient underwent right medial rectus recession

An Update on the Effects and Complications of BoNT-A in the Management of Third, Fourth, and Sixth Nerve Palsies: A Narrative Review.

This review article explores the etiology of oculomotor palsies-including third, fourth, and sixth cranial nerve palsies-and addresses the application of botulinum toxin type A (BoNT-A) in the management of these conditions, along with its associated complications and side effects. The objective is to assess BoNT-A's potential efficacy and its role across various types of nerve palsies. A comprehensive analysis of relevant studies reveals that BoNT-A holds promise as a therapeutic option in managing these conditions. BoNT-A injection into the lateral rectus muscle proves to be an effective tre

Causes of Diplopia, Strabismus Patterns, and Ocular Motor Features in Patients With Spinocerebellar Ataxia Type 27B.

BACKGROUND: Spinocerebellar ataxia type 27 B (SCA27B) caused by GAA trinucleotide repeats in the fibroblast growth factor 14 gene is emerging as a common cause of late-onset ataxia. Oscillopsia due to downbeat nystagmus (DBN) and diplopia are common symptoms, yet the causes of diplopia and strabismus patterns are poorly defined. METHODS: Retrospective chart review of 18 patients diagnosed with SCA27B over the past year. RESULTS: Ten of 18 patients had episodic or persistent oscillopsia or diplopia at disease onset, neurologically isolated in 4. Seventeen had detectable DBN, although it was o

Complex strabismus in a patient with KBG syndrome with 16q24.3 microdeletion.

KBG syndrome is characterized by intellectual disability, and craniofacial and skeletal abnormalities. We report the case of a 5-year-old boy with KBG syndrome with 16q24.3 microdeletion who showed complex strabismus. Left esotropia and ptosis was first noticed by parents at 18 months of age. He also showed submucosal cleft palate, velopharyngeal insufficiency, prominent eyebrows, short fifth fingers, and fetal finger pad. Marginal reflex distances were +4 mm in the right eye and +1.5 mm in the left eye. He had 18Δ of left esotropia, and 12Δ of left hypotropia, which increased with head tilt t

Comparison of different positioning techniques for reduction of induced vertical deviation following Nishida procedure for the treatment of sixth nerve palsy.

PURPOSE: Induced vertical deviation is a potential complication following the Nishida procedure for the treatment of sixth nerve palsy. This study aims to compare different positioning techniques for the reduction of this complication. METHODS: We retrospectively examined medical records from consecutive patients who underwent the Nishida procedure, classifying them into three positioning groups: intra-quadrant (IQP), lateral rectus border (LRBP), and horizontal meridian (HMP). Surgical and pre/postoperative data were compared. RESULTS: Among the 27 included patients (8 IQP, 9 LRBP, 10 HMP),

Neuro-Ophthalmic Manifestation Associated With COVID-19 in a Tertiary Eye Center in Nepal.

The coronavirus disease 2019 (COVID-19) primarily involves the respiratory system, but can manifest with a variety of neuro-ophthalmic symptoms. Here, we describe three cases presenting with neuro-ophthalmic manifestations secondary to COVID-19 at a tertiary center in Nepal. The first case was a 42-year-old male with sudden onset painless loss of vision noticed in the right eye (RE) after COVID-19 infection. Examination findings in the RE showed best corrected visual acuity (BCVA) of 6/18 with relative afferent pupillary defect positive and superior sectoral disk edema in the same eye. The cas

Acute Bickerstaff Brainstem Encephalitis in a Middle-Aged Male: Diagnostic Challenges and Therapeutic Responses.

Bickerstaff brainstem encephalitis (BBE) is a rare, post-infectious autoimmune disorder characterized by ophthalmoplegia, ataxia, and altered consciousness. It shares overlapping features with Guillain-Barré syndrome (GBS) and Miller Fisher syndrome (MFS), and diagnosis is often clinical, supported by anti-GQ1b antibodies. A 40-year-old previously healthy male presented with dizziness, diplopia, gait instability, and a recent viral prodrome. Neurologic examination revealed bilateral sixth cranial nerve (CN VI) palsy, partial involvement of the third cranial nerve (CN III), and bilateral dysmet

Bilateral Abducens Nerve Palsy: An Unusual Manifestation of Subdural Collection in Invasive Breast Cancer with Dural Metastasis.

Dural metastases from breast cancer are uncommon, with fewer than 5% of secondary intracranial tumors. As a presentation, bilateral abducens nerve palsy is extremely rare, making this case clinically significant. This case report emphasizes the need to recognize the unusual neurological manifestations in patients with advanced malignancies and to provide timely diagnosis and management. A 53-year-old woman with advanced breast cancer presented with progressive headaches, vomiting, and vision changes. On neurological examination, bilateral lateral rectus palsy and papilledema were observed. Mag

Strabismus fixus convergens secondary to protracted hydrocephalus.

We present a rare case of a middle-aged male with his eyes fixed in hypotropia-esotropia for 12 years. He had been diagnosed with and operated on for posterior fossa pilocytic astrocytoma prior to symptom onset. Contrast-enhanced MRI of the brain and orbit revealed dilated ventricles, a shunt in situ and a distorted midbrain architecture. The recti did not show any displacement, and axial length was not elongated. It was hypothesised that damage to vertical gaze centres, in conjunction with lateral rectus palsy due to protracted hydrocephalus, caused the clinical presentation. Surgical treatme

Bilateral subdural hygromas following dural puncture.

A parturient reported a headache 20 hours following unintentional dural puncture during epidural placement for labour analgesia. It was positional, mild to moderate in severity with no focal neurological signs. Six days later, the headache became more severe. She developed a left sixth cranial nerve palsy. A CT brain revealed bilateral fronto-parietal subdural hygromas with no evidence of raised intracranial pressure (ICP). Following neurology and neurosurgical opinions, it was felt that there were no absolute contraindications to an epidural blood patch (EBP). Eight days following dural punct

Transfer of the Intercostal Nerves to the Radial Nerve Branch Innervating the Long Head of the Triceps Muscle for Elbow Extension.

BACKGROUND: Transfer of intercostal nerves to the radial nerve branch innervating the long head of the triceps muscle for elbow extension is indicated in patients with traumatic brachial plexus palsy that is either the pan-plexus type or C5-C7 palsy with no triceps muscle function. The procedure aims to restore triceps muscle function through the use of the intercostal nerves, which are expendable nerves, as donors. DESCRIPTION: The procedure is performed by first identifying the third to fifth intercostal nerves and coaptating them to the radial nerve branch innervating the long head of the

Spontaneous Pseudoaneurysm of an External Carotid Artery Branch Presenting as a Parotid Mass: A Case Report and Literature Review.

This case report describes a rare spontaneous pseudoaneurysm of an external carotid artery (ECA) branch presenting as a parotid mass in a 65-year-old female. Initial imaging for right sixth cranial nerve palsy incidentally identified a 3 cm parotid lesion. Doppler ultrasound revealed a pseudoaneurysm, confirmed by computed tomography (CT) angiography, demonstrating thrombosed areas and ruling out a true aneurysm. Conservative management was pursued with no progression on follow-up imaging. This case highlights the diagnostic value of Doppler ultrasound in differentiating vascular anomalies and

Invasive fungal sinusitis with abducens nerve palsy.

BACKGROUND: Though rare, sphenoid sinusitis can cause abducens nerve palsy because of the anatomical proximity of the sphenoid sinus and the abducens nerve. CASE PRESENTATION: A male patient in his late seventies presented with double vision and left abducens nerve palsy. Imaging revealed sinus opacifications later identified as due to Scedosporium apiospermum, a rare fungal pathogen. Despite being immunocompetent, the patient developed invasive fungal sinusitis involving the sphenoid sinus. Treatment included systemic antifungal therapy with voriconazole and sinus surgery. Follow-up imaging

Pituitary adenoma extended to the basilar sinus: Lessons from anatomical and radiological studies (Experimental Research).

Pituitary adenoma (PA) with basilar sinus extension has often been overlooked in previous studies. In this study, 47 cases of such a subset of PAs were introduced, and the surgery-related anatomy and strategies were discussed. The medical records of patients with basilar sinus extension were analyzed retrospectively. Four human head specimens were used for epoxy sheet plastination, and five were used for endoscopic endonasal dissection. The connection between the cavernous sinus (CS) and the basilar sinus is in the superomedial space of the gulfar segment abducens nerve and beneath the petrosphe

Outcomes of Strabismus Surgery in Patients with Cranial Nerve Palsy.

Strabismus, or squint or deviating eyes, is defined as misalignment of the eyes when fixating on an object and is a common problem in ophthalmology. Palsy of the third, fourth or sixth cranial nerve is one of the leading underlying causes for paralytic strabismus, often requiring surgery. However, uncertainty regarding factors influencing surgical success remains. Background/Objectives: The purpose of this study is to review the outcome and influencing factors of strabismus surgery in patients with cranial nerve palsy. Methods: A retrospective study of 57 patients with third cranial nerve (CN3

Chiari malformation I and abducens nerve palsy: an overview of the documented cases.

INTRODUCTION: Chiari malformation (CM) type I is commonly distinguished by the caudal displacement of the cerebellar tonsils via the foramen magnum, leading to the compression of the brainstem and spinal cord. Lower cranial nerve palsies are frequently reported in individuals diagnosed with CM, occurring in approximately 15-20% of cases. However, the occurrence of abducens nerve palsy (ANP) is an exceptionally rare finding that necessitates additional examination. METHODS: To review the current literature on ANP in CM, we performed a thorough search of scientific databases such as PubMed, Goo

Breast Cancer Metastasis to the Pituitary Presenting With Apoplexy.

A 66-year-old female was referred to the skull base surgeons with diplopia, worsening bilateral visual disturbances, headaches, polydipsia, and polyuria. For the past 17 years, she had been treated for recurrent breast cancers and sigmoid colon carcinoma surgically and radiologically with aromatase inhibitors. MRI scans indicated significant metastases in her liver, bones, breast, pituitary, and left orbit with infiltration into the left optic nerve. During the investigations, the proximity of the large lesion to the carotid arteries and optic nerves had to be taken into account, while balanci
