International consensuses and guidelines on clinical practices on abducens nerve palsy by the Asia-Pacific Strabismus and Pediatric Ophthalmology Society (APSPOS) and the Academy of Asia-Pacific Professors of Ophthalmology (AAPPO).

Abducens nerve palsy is the most common ocular motor neuropathy, with a population-based annual incidence of approximately 4.7-11.3 per 100,000. It classically manifests as esotropia, horizontal diplopia, abduction deficit and compensatory head turn. Management follows a stepwise approach: immediate evaluation and treatment of underlying causes, followed by nonsurgical interventions, and surgical correction if misalignment persists beyond six months. However, various etiologies make it challenging to accurately diagnose and manage abducens nerve palsy in a cost-effective way. Controversies per

Microsurgery of a rare case of abducens schwannoma: surgical techniques.

Abducens schwannoma is relatively very rare, and accounts for only about 1% of all intracranial schwannomas. Here, we report microsurgical resection of a rare case of cisternal-segment abducens schwannoma in a female patient. Upon physical examination, the patient showed no eye movement symptoms, and facial & auditory nerves function were normal, but showed slight unsteady walking and positive Romberg sign. Magnetic resonance imaging (MRI) scan of the brain revealed a mass located at the right cerebellopontine angle (CPA), and the tumor was partially enhanced after contrast MRI scan. Thus, sch

Is elevated serum homocysteine in isolated ischemic cranial nerve palsies a predictor of stroke?

BACKGROUND: Isolated third, fourth, and sixth cranial nerve palsies (CNP) in elderly people occur commonly due to microvascular ischemia. Ischemic isolated CNP share several atherosclerotic risk factors that are responsible for stroke which include hypertension, diabetes mellitus and dyslipidemia. Hyperhomocysteinemia is atherogenic and hence is also considered as an independent risk factor for stroke. So indirectly, elevated homocysteine in CNP may act as a risk factor for stroke. AIM: To determine the incidence of isolated ischemic CNP secondary to elevated serum homocysteine (predisposing

An Atypical Pediatric Headache: Outpatient Presentation of Petrous Apicitis With Cerebral Venous Sinus Thrombosis.

BACKGROUND: Severe complications of acute otitis media (AOM) are rare in the United States due to widespread antibiotic use. One severe complication of AOM is the contiguous spread of infection to the petrous apex of the temporal bone, causing petrous apicitis. Due to its proximity to the dural venous sinuses, petrous apicitis can lead to a septic cerebral venous sinus thrombosis (CVST). The presentation of these complications is discussed here in the case of an 8-year-old male. CASE REPORT: An 8-year-old male with no significant past medical history presented to an outpatient clinic with a h

Plication-Augmented Modified Nishida's Procedure for Large-Angle Esotropia: A Case Series.

To evaluate the surgical outcomes of a combined modified Nishida's procedure with medial rectus (MR) recession augmented with lateral rectus (LR) plication in patients with long-standing complete sixth nerve palsy presenting with large-angle esotropia (ET). This retrospective interventional case series included three patients with chronic traumatic complete sixth nerve palsy and large-angle ET (70 to 75 prism diopters [PD]). Postoperatively, two patients achieved orthophoria at distance and near and one patient had a small residual ET of 5 PD at 6 months. The combination of modified Nishida's

Abducens nerve palsy as a false localising sign in spontaneous intracranial hypotension.

Spontaneous intracranial hypotension (SIH) with the formation of a spinal longitudinal epidural collection (SLEC) is an important treatable cause of headache. Left untreated, neurological sequelae can include cranial neuropathy, superficial siderosis and brain-sagging dementia. We describe a male in his late 40s presenting with orthostatic low-pressure headache and binocular horizontal diplopia due to an isolated left abducens nerve palsy, a recognised false localising sign. Imaging revealed intracranial hypotension associated with a SLEC, attributed to a cervical osteophyte associated dural t

Ophthalmoplegia Caused by Non-Aneurysm Neurovascular Conflict: Clinical Cases of Eight Patients.

BACKGROUND: Neurovascular conflict (NVC) is a rare but often overlooked cause of oculomotor cranial nerve (OCN) palsy. In this study, we aimed to enhance understanding of NVC as a potential cause of cranial nerve palsies by summarizing the characteristics of eight OCN palsy cases associated with NVC and reviewing previously reported cases. METHODS: A retrospective case series of eight patients with OCN palsy due to NVC was analyzed. Diagnosis was made using 3.0 Tesla magnetic resonance imaging (MRI) with 3D-fast imaging with steady-state acquisition (3D-FIESTA) sequences. Differential diagnos

Clinical Profile of Patients With Isolated Lateral Rectus Palsy in Adults.

Background Isolated lateral rectus (LR) palsy is a common cranial nerve palsy with a variety of causes. Horizontal diplopia is frequently caused by isolated LR palsy, a common cranial nerve palsy in adults. The abducens nerve is vulnerable to a variety of vascular, inflammatory, traumatic, viral, and compressive diseases because of its lengthy intracranial journey. This study aims to describe the clinical profile, etiological distribution, and outcomes in adults with isolated LR palsy. Methodology This retrospective observational study included 29 consecutive adults diagnosed with isolated LR

Protective effects of C5 level saline injection around the phrenic nerve in interscalene brachial plexus block: a randomized controlled trial.

Interscalene brachial plexus block (ISB) frequently causes phrenic nerve palsy. This study investigated whether ultrasound-guided saline injection around the C5 phrenic nerve could lessen diaphragmatic paralysis during shoulder surgery. A randomized controlled trialrandomized 120 patients into either a saline or control group. The saline group received 10 mL of normal saline around the C5 phrenic nerve before the routine ISB; the control group received the standard ISB alone. The primary endpoint was the incidence of diaphragmatic paralysis, while secondary endpoints included analgesic effecti

[Endoscopic Surgery for Trigeminal Schwannoma].

Minimally invasive surgical techniques, including the endoscopic endonasal transmaxillary-pterygoid approach (EETMPA), the endoscopic transorbital approach (ETOA), and the endoscopic keyhole approach (EKA), have recently been applied to the surgical treatment of trigeminal schwannomas. However, criteria for patient selection and technical nuances remain sparsely reported; therefore, we describe these aspects based on our previous work. For patient selection, a sufficient surgical corridor to the tumor from the paranasal sinus must be identified on magnetic resonance imaging to perform EETMPA.

Polyarteritis nodosa presenting as sixth nerve palsy: a case report.

INTRODUCTION: Polyarteritis nodosa (PAN) is a rare systemic necrotizing vasculitis affecting medium-sized vessels. While peripheral neuropathy occurs in 50-75% of patients, cranial nerve involvement is exceptionally rare, affecting fewer than 2% of patients. We report a case of PAN presenting with isolated sixth nerve palsy and orbital apex syndrome. CASE PRESENTATION: A 70-year-old man presented with the acute onset of painful left abducens nerve palsy. His medical history included 3 months of constitutional symptoms, polyarthralgia, unintentional weight loss, and progressive hearing loss. O

A systematic review of extraocular movement-related schwannomas (CN III, IV, VI). Part II: Surgical outcomes and prognostic factors for postoperative nerve function.

OBJECTIVE: Extraocular movement-related schwannomas (EOMS)-arising from the oculomotor (CN III), trochlear (CN IV), or abducens (CN VI) nerves-are rare, and comparative data on nerve-specific surgical outcomes and prognostic factors are limited. This paper represents Part II of a two-part study on EOMSs. While Part I addressed tumor localization, clinical features, and surgical approaches, the present paper focuses on surgical outcomes and prognostic factors for postoperative neurological function. METHODS: Systematic review identified surgically treated EOMS. Of 156 patients found, 117 had c

Real-world data analysis of cranial nerve III, IV, and VI palsies: demographics, association factors, and comparative all-cause mortality.

INTRODUCTION: Binocular diplopia is a distressing clinical condition that prompts clinicians to investigate underlying etiologies, particularly cranial nerve (CN) III, IV, or VI palsies. These may arise from microvascular ischemia, inflammation, trauma, compression, or neuromuscular junction disorders. This study aims to explore the association factors of these three ocular motor cranial nerve palsies and to compare their respective all-cause mortality rates using the real-world TriNetX Clinical Research Database (TriNetX CRD). MATERIALS AND METHODS: TriNetX is a global federated administrati

Anterior segment OCT, Schlemm's canal, and carotid-cavernous fistula: Reflections on a clinical case.

We rerport a 69-year-old man who presented with diplopia. He had a conjunctival injection with corkscrew-vessels, proptosis, and sixth cranial nerve palsy in his right eye (RE), and was diagnosed with a right carotid-cavernous fistula (CCF). Intraocular pressure was within normal limits. Gonioscopic examination revealed a pinkish appearance of the trabecular meshwork, but the Schlemm's canal could not be identified on anterior segment optical coherence tomography (OCT). We discuss the potential usefulness and limitations of OCT in the assessment of the Schlemm's canal in CCF.

Incidence of idiopathic intracranial hypertension (IIH) in adults and children: a 14-year population-based study.

BACKGROUND: Idiopathic intracranial hypertension (IIH) is characterised by raised intracranial pressure in the absence of structural pathology and with normal cerebrospinal fluid composition. IIH is strongly associated with obesity and rising incidence has been reported in northern hemisphere countries. However, paediatric and Southern Hemisphere populations, including Australia, are under-represented in the existing literature. METHODS: We conducted a multicentre retrospective study of adults (≥ 18 years) and children (< 18 years) diagnosed with definite IIH (Revised Friedman criteria 2013)

Chronic oculomotor nerve palsy secondary to a sphenoid-cavernous sinus lesion in an adolescent: a case report and literature review.

BACKGROUND: Oculomotor nerve (cranial nerve III) palsy in children and adolescents is uncommon and typically suggests an underlying structural lesion rather than microvascular ischemia. Sphenoid sinus disease represents a rare cause of ocular cranial neuropathies and most frequently affects the abducens nerve. Consequently, isolated chronic oculomotor nerve palsy secondary to sphenoid sinus pathology is an unusual clinical presentation. CASE PRESENTATION: We report a 16-year-old male who presented with long-standing right ocular deviation and progressive limitation of eye movements beginning
