The decompensated monofixation syndrome (an American Ophthalmological Society thesis).

PURPOSE: To describe the clinical features and response to treatment of patients with decompensated monofixation syndrome (MFS) and to propose a hypothesis for a decompensation mechanism in such patients. METHODS: Fourteen adults with MFS who had been symptomatically stable for a mean duration of 25 years developed diplopia in the absence of neurologic or orbital disease. After retrospective chart review, they underwent detailed orthoptic testing. Results from this cross-sectional analysis were compared with similar data from 16 control subjects with stable MFS. RESULTS: Compared to stable M

[Accessory extraocular muscle-a case report and review].

OBJECTIVE: Explored the causation of a case of incomitant vertical strabismus accompanied with elevation deficit and globe retraction by surgery. METHODS: Case report. Orbital imaging study of MRI was used to discover the anatomic feature of extraocular muscle. By released the restrictive structure to treat strabismus. Histopathologic inspection was used to confirm the origin of the abnormal structure. RESULTS: Abnormal extraocular muscle that located within the cone formed by the four recti muscles was the causation of strabismus. It arose at the annulus of Zinn, passing forwards between th

Understanding skew deviation and a new clinical test to differentiate it from trochlear nerve palsy.

Skew deviation is a vertical strabismus caused by a supranuclear lesion in the posterior fossa. Because skew deviation may clinically mimic trochlear nerve palsy, it is sometimes difficult to differentiate the 2 conditions. In this review we compare the clinical presentations of skew deviation and trochlear nerve palsy and examine the pathophysiology that underlies skew deviation. We then describe a novel clinical test-the upright-supine test-to differentiate skew deviation from trochlear nerve palsy: a vertical deviation that decreases by > or =50% from the upright to supine position suggests

Prevalence of strabismus in patients with pathologic myopia.

BACKGROUND: To determine the prevalence and clinical features of strabismus in patients with pathologic myopia. METHODS: Medical records of a total of 636 highly myopic patients were retrospectively reviewed. Pathologic myopia was defined as spherical equivalent (SE) of at least -8D, or axial length >26.5 mm in patients older than 9 years, <-4D in those younger than 5 years, <-6D in those aged from 6 to 8 years. Myopic refractive degree, axial length measurements, best-corrected visual acuity and eye position were then analyzed. RESULTS: Among 636 patients with pathologic myopia, 520 (81.8%)

Inferior rectus aplasia simulating contralateral monocular elevation deficiency.

Inferior rectus agenesis is usually not considered in the differential diagnosis of vertical strabismus because of its rare incidence. The authors examined an infant who presented with monocular elevation deficiency and was finally found to have contralateral inferior rectus agenesis by magnetic resonance imaging. Inferior rectus agenesis could manifest as contralateral elevation deficit. Careful extraocular motility examination and imaging study are essential for the diagnosis of inferior rectus aplasia.

Horizontal misalignment in patients with unilateral superior oblique palsy.

PURPOSE: To determine the frequency and distribution of horizontal misalignment in patients with unilateral superior oblique palsy (SOP) and to determine the indications for combining horizontal with vertical strabismus surgery. METHODS: Patients included in the study had a vertical heterophoria or tropia that fit Parks three-step test for SOP and had no previous strabismus surgery or other ocular motility disturbance. Ocular motility and alignment were recorded. Outcomes between patients who had vertical surgery alone and those who had combined vertical and horizontal surgery were compared u

Pattern recognition of vertical strabismus using an artificial neural network (StrabNet).

BACKGROUND: Our goal is to develop an accessible expert system (StrabNet) that will assist in the clinical diagnosis of vertical strabismus, form the basis of a teaching/learning tool, and contribute to the audit process. Potentially, this model can be extended to other strabismus deviations. METHODS: Vertical deviations were separated into eight classifications (diagnoses). An expert system based on architecture of artificial neural networks learned the patterns for each class of deviation based on 10 prism cover-test measurements (9 cardinal positions and near fixation). The expert system w

Short tag noose technique for optional and late suture adjustment in strabismus surgery.

OBJECTIVE: To present and evaluate a new technique that allows the second-stage suture adjustment in strabismus surgery to be skipped or delayed if the immediate postoperative alignment is satisfactory. METHODS: The "short tag noose" technique replaces long sutures with short tags that can be left under the conjunctiva after adjustment. Retrospectively, the medical records of all patients treated by a single surgeon with this approach between January 1, 2005, and December 31, 2008, were evaluated for success rate (< or =10 prism diopters [PD] horizontal and < or =6 PD vertical), reoperation r

Sphenoethmoidal mucocele masquerading as trochlear palsy.

Acquired vertical strabismus is commonly caused by superior oblique muscle palsy, often resulting from blunt head trauma or vascular problems, and less often from brain tumors, meningitis, and aneurysms. To date, mucoceles in the ethmoid sinus have rarely been reported as a cause for superior oblique muscle palsy. We report a case of trochlear nerve palsy and subsequent optic neuropathy caused by a mucocele in the ethmoid and sphenoid sinuses.

Mosaicism for trisomy 21 in a patient with young-onset dementia: a case report and brief literature review.

BACKGROUND: Skew deviation is typically caused by brainstem damage, and has not been identified with focal cerebellar lesions. This vertical strabismus has been attributed to asymmetric disruption of vestibuloocular reflex (VOR) projections from otolithic receptors of the utricle to ocular motoneurons, but asymmetry of the utriculo-ocular counter-roll reflex has not been detected. METHODS: Lesions localized to the cerebellum were identified by MRI in five patients with vertical strabismus. Their skew deviation was measured by prism cover tests in all patients and by search coils in three pati

"Was it something I said?": finding retinal pathology without actually examining the retina.

INTRODUCTION: Diplopia secondary to maculopathy is a rare disturbance of binocular vision that may present to the orthoptist. The purpose of this study was to identify the distinguishing symptoms and clinical signs of this condition. METHODS: A retrospective medical record review of adults presenting with diplopia that could not be relieved with prism or strabismus surgery. RESULTS: Six cases were identified. All complained of vertical diplopia, though only one patient had a vertical strabismus. The diplopia improved in dim illumination in all cases. All patients noted monocular visual distu

Adaptive neural mechanism for Listing's law revealed in patients with skew deviation caused by brainstem or cerebellar lesion.

PURPOSE: Skew deviation is a vertical strabismus caused by damage to the otolithic-ocular reflex pathway and is associated with abnormal ocular torsion. This study was conducted to determine whether patients with skew deviation show the normal pattern of three-dimensional eye control called Listing's law, which specifies the eye's torsional angle as a function of its horizontal and vertical position. METHODS: Ten patients with skew deviation caused by brain stem or cerebellar lesions and nine normal control subjects were studied. Patients with diplopia and neurologic symptoms less than 1 mont

The linear vestibulo-ocular reflex in patients with skew deviation.

PURPOSE: The linear vestibulo-ocular reflex (LVOR) is mediated primarily by the otolith organs in the inner ear. Skew deviation is a vertical strabismus believed to be caused by imbalance of otolithic projections to ocular motor neurons (disynaptically through the medial longitudinal fasciculus in the brain stem or polysynaptically through the cerebellum). The authors postulated that if skew deviation is indeed caused by damage to these projections, patients with skew deviation would show abnormal LVOR responses. METHODS: Six patients with skew deviation caused by brain stem or cerebellar les

Binocular vertical rectus muscle recession for comitant vertical strabismus.

BACKGROUND: Binocular vertical rectus muscle recession has not been formally evaluated in the correction of comitant vertical strabismus. METHODS: Eight patients with stable comitant vertical strabismus for at least 6 months were included. All underwent recession of the superior rectus muscle of the hypertropic eye combined with an equal or nearly equal recession of the inferior rectus muscle in the hypotropic eye. On the day before surgery, on one of the first three postoperative days, and at one year postoperatively, ocular alignment in vertical and horizontal gaze directions were measured

Relative afferent pupillary defect with normal vision and vertical strabismus--implications for pupillary pathway anatomy.

BACKGROUND: A relative afferent pupillary defect (RAPD) is usually a sign of visual dysfunction. Here, an unusual case of an RAPD combined with vertical strabismus but normal vision is described. Implications for pupillary pathway anatomy are discussed. METHODS: A 12-year-old girl with chronic headache was shown to have a midbrain tumour. She presented to us with intermittent diplopia. Examination included visual acuity, visual fields, pupillary function, anterior and posterior segments, and strabismus evaluation with the tangent screen. RESULTS: There was a right-sided pathologic RAPD but n

[Binocular vision and vertical strabismus].

BACKGROUND/AIM: Elevation in adduction is the most common pattern of vertical strabismus, and it is mostly treated with surgery. The results of weaking of inferior oblique muscle are very changeable. The aim of this study was to evaluate binocular vision using sensory tests before and one and six months after the surgery. METHODS: A total of 79 children were divided in two groups: the first, with inferior oblique muscle of overaction (n = 52), and the second with dissociated vertical deviation (DVD), and primary inferior oblique muscle overaction (n = 27). We tested them by polaroid mirror te
