An expert system is described for the differential diagnosis of vertical deviation strabismus (squint) from measurements taken in the standard prism cover test. The deviations are represented as optical powers in prism dioptres using the graphic representation of strabismus (after Jampolsky). The expert is implemented in MatLab (Mathworks Ltd., Cambridge, UK) both as a stand-alone program on a PC and as a web application available over the Internet (see http://www.strabnet.com ). In trial and clinical datasets a diagnostic accuracy of 100% was achieved.

We evaluated the one-stage intraoperative adjustment strabismus surgery with adjustable suture under topical anesthesia. Medical records of the patients who had intraoperative adjustment surgery under topical anesthesia for horizontal or vertical strabismus in our hospital from March 1997 to March 2003 with follow-up of 6 months were analyzed retrospectively. This paper presents the case of a man with resulting diplopia due to multiple lesions of the extraocular muscles after an orbital trauma. The intact ocular globe has an horizonto-vertical strabismus. Purpose was the reduction of diplopia in primary gaze. The original contribution of the authors is the new method of prescription for the prismatic lenses, for the best results in glasses execution. A mathematical model of vectorial composition for light deviation through a system of prismatic lenses, is proposed.

METHODS: We reviewed the Toxin Clinic database over a 20-year period, identifying patients with strabismus secondary to permanent or temporary impairment of vision.
RESULTS: Five hundred-and-three patients who fulfilled our criteria were treated with Botulinum toxin. There were 301 males and 202 females, with a mean age at treatment of 35 years (range 6-86). METHODS: The following squint angles were measured by the alternate prism and cover test at a viewing distance of 5 m.
