PURPOSE: To report the outcomes of a management protocol for Marcus Gunn jaw winking synkinesis.
METHODS: The records of 31 patients (16 female, 15 male) presenting to a tertiary referral center with Marcus Gunn jaw winking synkinesis between 1993 and 2003 were retrospectively analyzed. Generally, patients with mild wink and a small degree of ptosis underwent unilateral upper eyelid retractor surgery. Patients with a moderate or marked wink and ptosis underwent bilateral levator weakening procedures and brow suspension. Patients were assessed and treated for amblyopia and vertical strabismus

PURPOSE: To describe the possible abnormal upper eyelid positions in congenital Brown syndrome (BS) and discuss physiopathology.
CONTEXT: A typical symptom of BS, the incapacity to raise the eye in adduction, is usually accompanied by a retraction of the upper eyelid. However, as the abnormal position of the eye often refers to a patient looking straight ahead into the distance, abnormal eyelid positions are possible but are on the whole unknown.
PATIENTS AND METHODS: The study investigated 82 patients with unilateral congenital BS, who consulted between 1989 and 2001. The primary position o

The purpose of this study was to report on 12 patients with acquired strabismus following cosmetic blepharoplasty and to identify patterns of strabismus related to the surgical procedure. Clinical ophthalmologic examinations were performed to specifically clarify the type of strabismus. Operative reports of the blepharoplasty procedures were reviewed. Patients were followed for a minimum of 3 months after the blepharoplasty procedure before surgical intervention was considered. Operative findings at corrective strabismus surgery were noted and an attempt was made to correlate these findings wi

We have operated 79 children with vertical strabismus. We have treated 52 children with primary overaction of the inferior oblique muscles and 27 of them with DVD and primary overaction of the inferior oblique muscles. In all of them, we have done recession of the inferior oblique muscles (for 4, 6, 8 or 10 mm). In only seven of them, from the second group, we did recession of the superior recti (for 7, 8 or 9 mm) simultaneously. This surgery very significantly and adequately ameliorate vertical strabismus (p = 0.004). Mentioned surgery on vertical extraocular muscles theoretically may product

PURPOSE: To identify the threshold at which horizontal or vertical strabismus becomes reliably detectable by observers and to determine the effects of interpupillary distance, age, gender, and observer experience.
METHODS: Six models of different gender, age, and interpupillary distance were digitally photographed in several predetermined gaze positions off-axis in the horizontal and vertical planes. Standardized distance, zoom factor, and lighting were used. The images were digitally altered to exactly superimpose one eye deviated and one eye aligned with the axis of the camera. This simulat

BACKGROUND: The aim of this paper was to determine whether the surgery on vertical muscles of the eye influenced the change in refraction and visual acuity.
METHODS: This prospective study included testing of 79 children with infantile esotropia who suffered from the vertical strabismus after the surgery on horizontal muscles. Patients were divided into two groups: the first group of 52 children (the test group) had only vertical component (overaction of m. obliqui inferior) and the second group (the control group) had two vertical components (dissociated vertical deviation-DVD and overaction

PURPOSE: To evaluate the adaptive vertical vergence aftereffect and determine whether it contributes to a difference of vertical deviation with respect to gaze distance in patients with vertical strabismus.
DESIGN: Prospective noncomparative studies.
METHODS: Eighty-four patients with unilateral superior oblique palsies were enrolled and classified into three types-A, B, and C-based on the difference in vertical deviation between distant and near viewing. The prism adaptation test was performed for 2 to 3 hours to correct vertical deviation and the response of vertical deviation to the prism

BACKGROUND: Patients with skull defects sometimes develop neurological deficits, which have been grouped under "the syndrome of the trephined". The deficits are usually nonspecific or nonlocalizing, such as apathy or diffuse headaches. We report, to our knowledge, a first case of severe midbrain syndrome associated with a skull defect. Cranioplasty dramatically resolved the patient's symptoms. A midbrain syndrome represents the main manifestation of the syndrome of the trephined and can be corrected by cranioplasty.

Anomalous orbital structures are a rare cause of strabismus. These structures attach to the globe and produce a mechanical restriction, resulting in incommitant motility disorders. Three types of anomalous structures have been described. The first arise from the extraocular muscles themselves and insert in abnormal locations. The second are fibrous bands located beneath the rectus muscles. The third are discrete anomalous muscles that originate in the posterior orbit and insert in abnormal locations on the globe. These structures have been associated with unusual patterns of strabismus. Clinic

PURPOSE: The manifest angle of the horizontal deviation in monofixation syndrome (MFS) has been reported to reach a maximum of 8 to 10 PD. Review of the literature, however, revealed no studies about the vertical deviation associated with MFS. The purpose of this study is to evaluate the range of the angle of the vertical deviation in MFS.
METHODS: Forty patients who had MFS with vertical strabismus in the distance primary position were included in this study. All fulfilled the criteria for monofixation, fusing Worth 4 dots at near but suppressing at distance and/or having stereoacuity betwee

BACKGROUND: Surgery of the inferior oblique muscle (IO) has undergone significant changes in the past 160 years. Many investigators have contributed to our understanding of the action of this muscle and to the surgical options that have developed. This article reviews the history of IO surgery with particular emphasis on the anterior transposition procedure.
METHODS: Anatomic and physiologic studies on the neurofibrovascular bundle of the IO are presented.
RESULTS: The ligamentous structure of the neurofibrovascular bundle of the IO provides the ancillary origin for the posterior temporal fi

Traumatic superior oblique dysfunction from cataract surgery appears to be rare, with only 3 reported cases of postoperative Brown syndrome and 1 reported case of postoperative superior oblique weakness. We are not aware of any prior reports of superior oblique overaction occurring after cataract surgery. We describe a patient with acquired superior oblique overaction as a cause of vertical strabismus after cataract surgery. Ocular torsion analysis was essential in localizing the malfunction to the superior oblique muscle. The most likely etiology is myotoxicity from inadvertent intramuscular

Introduction: In recent years a new perforated PDS (poly-p-dioxanon) foil (0.15 mm) has become available and has not yet been proven to be successful in reconstruction of the orbital floor after blow-out-fractures in randomized studies. The main aim of this clinical trial is to compare this new PDS foil with titanium dynamic mesh (0.3 mm) (TD), which is well established in reconstruction of the orbital floor. Patients and Methods: In a prospective multicentre randomized trial, conducted between 1997 and 1998, out of 42 patients with fractures of the orbital floor, 28 patients needing material

The diagnosis of an acquired vertical strabismus is not always straightforward. There is no one specific test that will diagnose a vertical deviation. The clinical presentation, signs, and symptoms are the driving forces that will help lead to the correct diagnosis. Patients with binocular vertical diplopia may have symptoms of recent onset or that have been long-standing. Others may not even be completely aware that their ocular symptoms are attributable to a doubled vertical image. The differential diagnosis for vertical diplopia includes oculomotor nerve palsy, superior oblique palsy, restr
