[Surgical correction of strabismus in Graves-Basedow ophthalmopathy. Personal experiences].

UNLABELLED: The aim of the study was to evaluate long-term results of surgical treatment of strabismus and diplopia connected with Graves' ophthalmopathy. Surgery of extraocular muscles was performed in 64 patients, previously with different methods for their Graves' ophthalmopathy. All patients showed stabilization of the strabismus angle and diplopia as well as euthyroidism. Before the operation ultrasonography, orbital computed tomography and forced duction test were performed. Before the operation horizontal strabismus with vertical deviation and vertical strabismus were most frequent (36.

Anterior segment ischemia and retinal detachment after vertical rectus muscle surgery.

PURPOSE: The authors describe the clinical course of a woman who developed two complications following vertical strabismus repair: anterior segment ischemia (ASI) and retinal detachment. METHODS: A 62 year-old woman is described. She presented with new onset proptosis and left hypertropia with significant diplopia in all fields of gaze. This presentation, her 15 year history of thyroid disease, and preoperative computed tomography (CT) of the orbits were consistent with Graves' ophthalmopathy. Vertical strabismus repair was carried out by recessing the left superior rectus muscle and resectin

Facial asymmetry in superior oblique muscle palsy and pulley heterotopy.

INTRODUCTION: Some observers have considered facial asymmetry as characteristic of congenital superior oblique muscle (SO) palsy. However, recent orbital imaging studies have determined that incomitant vertical strabismus resembling SO palsy can be caused by heterotopic rectus muscle pulleys. This finding suggests that facial asymmetry may predict the presence of abnormal orbital anatomy rather than be secondary to ocular torticollis. METHODS: Subjects who underwent orbital computed tomography or magnetic resonance imaging were divided into 5 groups based on clinical evaluation and previously

The Lancaster red-green test before and after occlusion in the evaluation of incomitant strabismus.

BACKGROUND: Patients with incomitant strabismus can often fuse in a limited area of gaze. Prolongation of neurologically learned fusional vergence tone ("vergence adaptation") in and near this area can result in misleading measurements with standard clinical measures of strabismus. Monocular occlusion for at least 30 minutes eliminates most of the effect of vergence adaptation. The Lancaster red-green test provides an elegant and convenient map of incomitant strabismus. We investigated the efficacy of the Lancaster red-green test before and after monocular occlusion for the investigation of in

Treatment of strabismus and nystagmus with botulinum toxin type A. An evaluation of effects and complications.

PURPOSE: Injection of botulinum toxin type A into eye muscles leads to a temporary paralysis and the effects have been evaluated in strabismus or nystagmus. METHOD: A total of 112 patients with different types of concomitant and paralytic strabismus and acquired nystagmus were treated with botulinum toxin, according to well-established indications. RESULTS: The lasting effects of the injections on strabismic angle were largest in esotropia, consecutive exotropia and abducens palsy, and amounted to, on an average, 12 prism diopters or 6 degrees. The larger the strabismus the better was the ef

Vertical location of the corneal light reflex in strabismus photography.

OBJECTIVE: To improve the clinical documentation of strabismus by mathematically predicting and clinically verifying the location of a fixation target that produces a vertically centered corneal light reflex (first Purkinje image) in clinical photographs of the eye using a standard photographic flash unit. MATERIALS AND METHODS: Mathematical modeling of the corneal light reflex during clinical photography was based on the schematic eye. Clinical photographs were taken using a range of fixation targets located between the center of the camera lens and the center of the flash. Image quality was

Vertical strabismus resulting from an anomalous extraocular muscle.

Unusual ocular motility abnormalities have been rarely reported to result from anomalous extraocular structures. These structures, which may be either muscular or fibrotic, attach to the globe and produce a mechanical restriction resulting in incomitant strabismus. To our knowledge, we report the first patient with an anomalous extraocular muscle in whom the clinical, radiologic, surgical, and histopathologic findings are described.

Results of a combined adjustable recession and posterior fixation suture of the same vertical rectus muscle for incomitant vertical strabismus.

PURPOSE: The posterior fixation suture (fadenoperation) is an effective treatment for complicated incomitant vertical strabismus. Traditional operative methods do not permit the simultaneous use of an adjustable recession of the same muscle. METHODS: Seven patients with incomitant vertical strabismus and diplopia were treated with a combined adjustable recession and posterior fixation suture of the same vertical rectus muscle. Preoperative vertical misalignments in the primary position ranged from 4 to 10 prism diopters. Vertical incomitance from the primary position into the field of action

Synostotic plagiocephaly causing pseudoparalysis of the superior oblique and ocular torticollis: report of a case with unique sensory findings.

PURPOSE: To report a case in which strabismus surgery did not eliminate the abnormal head posture although it did improve the vertical strabismus; and to consider the causes of this situation. CASE REPORT: The patient had a right eso-hypertropia and ocular torticollis to the left shoulder as a result of a right superior oblique mechanical deficiency, secondary to her craniosynostosis. A myectomy of the right inferior oblique muscle and a tucking procedure of the right superior oblique muscle were performed. Postoperatively, the abnormal head posture persisted, although strabismus surgery impr

Vertical strabismus after cataract surgery.

PURPOSE: To compare anesthesia methods with resultant strabismus patterns in patients with vertical diplopia after cataract surgery. METHODS: The authors analyzed 28 consecutive patients with acquired vertical diplopia after cataract surgery to identify the strabismus pattern. The method of anesthesia administration was available in 21 patients. Three orbital dissections with simulated retrobulbar blocks were performed on cadavers to ascertain the possibility of injuring the vertical rectus muscles at the time of injection. RESULTS: Fifty percent of the involved muscles were overactive, 39 p

[Clinical aspects of strabismus with insufficiency of the superior oblique muscle].

Clinical features of vertical strabismus with superior oblique muscle insufficiency were studied in 123 patients. A complex of clinical methods was used and clinical evaluation of these methods made. Various forms of strabismus with superior oblique muscle insufficiency were detected. Four degrees of superior oblique muscle weakness were distinguished corresponding to four degrees of inferior oblique muscle hyperfunction.

Dissociated vertical deviation (DVD): The saccadic and slow eye movements.

Dissociated vertical deviation (DVD), a vertical strabismus, is often associated with infantile esotropia, latent nystagmus and excyclo-torsion. DVD usually becomes apparent at about three years of age. The authors wished to determine whether DVD is a manifestation of an abnormal motor vergence system or is part of a visual system disorder. Vertical eye movements of six subjects with DVD, on cover-uncover and alternate cover tests, were studied using the magnetic search eye coil system. Asymmetry between the eyes' saccades were quantified during the DVD upward movement and during a trial of pu
