[Lower eyelid retraction after inferior rectus recession in thyroid eye disease].

The involvement of extraocular muscles (EOMs) in the inflammatory process in thyroid eye disease (TED) often leads to fibrosis, persistent restrictive strabismus, and binocular diplopia. The inferior rectus muscle (IRM) is most commonly affected, resulting in vertical strabismus and hypotropia with significant limitation of upward gaze. Surgical management of restrictive strabismus in TED aims to increase the range of binocular eye movements, eliminate binocular diplopia, and correct the cosmetic defect associated with strabismus. The "gold standard" surgery involves weakening of the affected

Outcome of Horizontal Strabismus Surgery and Factors Influencing Surgical Success.

INTRODUCTION: Strabismus is a complex problem of eye alignment, binocularity, and cosmetics that affect vision and quality of life. Strabismus surgery helps to establish binocular fusion, expand the field of binocular vision, eliminate diplopia, correct a compensatory head posture, and improve cosmetic appearance. The outcomes of horizontal strabismus surgery range from 56.3-86.5%. It depends on different factors such as the type of strabismus, accurate measurement of pre-operative deviation, age at which strabismus surgery was performed for the first time, type of strabismus surgery, postoper

Inferior Rectus Muscle Y-Split for Thyroid-Related Vertical Strabismus.

Incomitant hypotropia in thyroid eye disease can be difficult to manage, especially in the presence of orthotropia with fusion in down gaze and reading position. Recessing the affected ipsilateral inferior rectus muscle may result in an undesirable downgaze diplopia secondary to a hypertropia in downgaze. Various surgical techniques have been described to manage this potential complication including asymmetric recession of both inferior rectus muscles, posterior myoscleropexy operation, and the Scott recess/resect procedure of the contralateral inferior rectus. In 2004, Hoerantner et al. intro

[Differential diagnosis of vertical strabismus in the elderly].

Due to the demographic changes, the number of older patients in ophthalmological practices and clinics, including those with diplopia, is increasing. Some of the patients report not only horizontally shifted double images but also or only vertically shifted double images. Vertical double vision often causes significant diagnostic problems for ophthalmologists. The underlying condition could urgently require further neurological, neuroradiological and/or internal medical diagnostics (e.g., skew deviation, 4th nerve palsy, myasthenia, Graves' orbitopathy, orbital floor fracture, orbital mass, 3r

The Outcome of Inferior Oblique Myectomy for Apparent Inferior Oblique Overaction Associated with Craniosynostosis.

PURPOSE: Strabismus in patients with craniosynostosis is common, but surgical correction of strabismus in these patients remains challenging. We report our findings in six patients (four of whom were Korean) with craniosynostosis who underwent strabismus surgery to specifically address V-pattern horizontal strabismus with moderate-to-severe inferior oblique (IO) overaction, using IO myectomy at a single tertiary hospital between 2005 and 2016. METHODS: We recorded preoperative characteristics including sex, age, type of strabismus, versions grading, refractive error, and visual acuity. The gr

Validity and Reliability of 2 Cirrus Optical Coherence Tomography Procedures for Measuring Objective Ocular Torsion.

PURPOSE: Torsional eye position by the disc-fovea angle (DFA) is a relevant objective parameter in vertical strabismus. DFA measurement with optical coherence tomography (OCT) has proven to be a good alternative to the gold standard measurement in fundus photography. Our study aims to analyze the validity and reliability of 2 undescribed Cirrus high-definition OCT (HD-OCT; Carl Zeiss Meditec) methods that offer clinical advantages for measuring objective cycloposition in normal patients compared with Spectralis spectral-domain OCT (Heidelberg Engineering) and the reference method. Evaluation of a Novel Virtual Reality Simulated Alternate Cover Test to Assess Strabismus: A Prospective, Masked Study.

PURPOSE: A sensorimotor examination is the gold standard for strabismus diagnosis and quantification but requires a highly skilled examiner and may be limited by a child's cooperation. Virtual reality (VR) employs eye-tracking technology to monitor eye position and may be able to measure strabismus. The aim of this study was to assess a prototype VR-simulated alternate cover test to detect and measure strabismus. DESIGN: Prospective, masked diagnostic test study. METHODS: Patients aged 5-18 years with visual acuity of 20/80 or better were prospectively enrolled to undergo strabismus measurem
