A Review of the Ocular Phenotype and Correlation with Genotype in Poretti-Boltshauser Syndrome.

Background and Objectives: Poretti-Boltshauser syndrome (PBS) is a rare, autosomal recessive disorder caused by pathogenic variants in the LAMA1 gene, resulting in laminin dysfunction. This manifests as a cerebellar malformation with cysts, and patients present with developmental delay and ataxia; however, ocular features are not well-characterised. We aimed to summarise the ocular phenotypes of PBS based on cases reported in the literature. Materials and Methods: A literature search was conducted on Medline, Embase, and PubMed on PBS and its ocular associations. Genetically confirmed PBS case

Same-Day Approach for Combined Intravitreal and Intracerebroventricular Enzyme Replacement Therapy to Prevent Retinal Disease Progression in Children With Neuronal Ceroid Lipofuscinosis Type 2.

BACKGROUND: Classic late infantile neuronal ceroid lipofuscinosis (CLN2) is caused by a biallelic mutations of the TPP1 gene. Vision loss begins around age four years, resulting in blindness by age seven to ten years. Intracerebroventricular cerliponase alfa (Brineura; BioMarin) is indicated to slow the loss of ambulation in pediatric patients with CLN2. However, treated children continue to experience visual loss. Intravitreal cerliponase alfa allows the enzyme to target tissues in the eye, offering a treatment option. METHODS: We developed a pathway for same-day administration of both intra

Smartphone use-related acute accommodative esotropia : A possible new paradigm of visual disorders associated with the use of modern technologies.

We present a rare case of a 9-year-old child with acute accommodative comitant esotropia, a form of acquired strabismus caused by excessive smartphone use. Symptoms commonly include double vision, closing or covering one eye when working at close range and tilting the head or its unnatural position to correct visual abnormalities; however, there are also patients without any symptoms. As part of the differential diagnosis it is necessary to exclude any eye diseases or refractive errors and it is highly recommended to carefully exclude pathologies of the central nervous system and intracranial

Evaluation of timed dexamethasone eye drops to prevent proliferative retinopathy of prematurity: a study protocol for a randomized intervention, multi-centre, double-blinded trial (DROPROP).

BACKGROUND: As the survival rate of preterm infants continues to rise worldwide, more infants are at risk of developing sight-threatening retinopathy of prematurity (ROP). Destructive retinal laser treatment and intravitreal injections of anti-vascular endothelial growth factor (VEGF), factor, which have potential systemic side effects, are necessary to prevent blindness in severe cases of ROP. Off-label use in clinical settings suggests that dexamethasone eye drops, 1 mg/ml, may prevent the progression of ROP to severe disease (Type 1 ROP) requiring treatment. Our current study aims to assess

Strabismus and nystagmus in oculocutaneous albinism: clinical perspectives, diagnosis, and role of neurotransmitters.

The rare recessive autosomal non-communicable disorder oculocutaneous albinism causes discoloration of the eye, hair, and skin. Oculocutaneous albinism is a hereditary group of disorders with sub-differential characteristics like reduction of pilar, cutaneous, and ocular pigmentation. Clinical characteristics and symptoms include strabismus, iris hypopigmentation, nystagmus, reduced visual acuity, foveal hypoplasia, refractive errors, photophobia, and colour-visual impairment. The associated genetic mutation results in the reduced activity of tyrosine activity required for the metabolism of me

Current status of school vision screening-rationale, models, impact and challenges: a review.

Uncorrected refractive error is the leading cause of vision impairment in children globally, and studies have demonstrated that spectacle correction addresses the large majority of childhood vision impairment. Furthermore, trial evidence illustrates the beneficial impact of spectacles on learning, with effect sizes exceeding that of other school health interventions. While it is established that good vision is important for learning and optimising childhood development and quality of life, many countries lack healthcare systems that provide vision screening or universal access to eyecare for a

Surgical Management of a Prominent Adduction-Induced Upshoot in Duane Retraction Syndrome Type III: A Case Report.

Duane retraction syndrome (DRS) with good primary gaze alignment and no abnormal head posture is often managed conservatively to avoid worsening alignment. However, surgical intervention may be considered when significant cosmetic concerns are present. We present the case of a 29-year-old male patient with type III DRS of the right eye and a prominent upshoot. Despite having good primary gaze alignment, the patient desired cosmetic improvement. Forced duction testing revealed restrictions in multiple directions. Based on the spring-back balance test, we performed an 8.0 mm recession of the lat

Deviation control that deteriorates with fixation distance in basic-type intermittent exotropia.

BACKGROUND/AIMS: The clinical assessment of deviation control at near (30 cm or 1/3 m) and far (5-6 m) does not reflect if and how patients' control changes between these two predetermined test distances. We examine how patients' control varies at 6 fixation distances between the two clinical test distances of 30 cm and 5 m. METHODS: The medical records of 48 patients with basic intermittent exotropia (IXT) were retrospectively reviewed. Patients who had visited the Kindai University Hospital between October 2021 and March 2022 without a prior history of treatments except spectacle correction

Many Faces of Melanoma: A Comparison Between Cutaneous, Mucosal, Acral, Nail, and Ocular Malignancy.

A melanoma is a malignant neoplasm that originates from melanocytes. Melanocytes are found in various body sites-they are most commonly found in the skin (including acral sites and nail units), ocular structures, and mucosal membranes. This narrative review aims to comprehensively summarize the current information available regarding several types of melanomas and to analyze the existing evidence. Despite the same origin-melanocytes-melanomas appear to be a heterogeneous neoplasm. A thorough analysis of several types of melanomas reveals differing genetics, risk factors, epidemiology, and prog

Report of a Rare Case and Literature Review of Combined Marcus Gunn Jaw Winking Synkinesis and Monocular Elevation Deficiency.

Marcus Gunn Jaw Winking Synkinesis (MGJWS) is a rare congenital condition characterized by unilateral ptosis that improves with ipsilateral jaw movement. It frequently presents alongside other ocular abnormalities, including strabismus and anisometropia, which significantly increase the risk of amblyopia. Monocular elevation deficiency (MED), a type of strabismus marked by impaired elevation of the eye in all gaze positions, is most commonly associated with MGJWS. We report the case of a 14-year-old male with a history of refractive amblyopia who presented with both MGJWS and MED. This case su

Restrictive strabismus caused by infantile myositis masquerading as Duane retraction syndrome.

We report a rare case of restrictive strabismus in a pediatric patient who was initially misdiagnosed with Duane retraction syndrome (DRS) because she had large-angle exotropia, restricted adduction, globe retraction, narrowing of the palpebral fissure, and downshoot on attempted adduction. Notably, the parents first observed strabismus at 12 months of age, with photographs taken before 6 months of age revealing no ocular misalignment. Despite the absence of typical inflammatory signs, the temporal discrepancy, characterized by delayed onset inconsistent with congenital DRS, prompted further i
