Double insertion of superior oblique muscle of the eye: a rare variation.

Superior oblique muscle of the eye is involved in depression, abduction and intorsion of the eyeball. Knowledge of its variations is useful for understanding and repairing strabismus cases and other visual problems. We report a rare unilateral variation of right superior oblique muscle. The superior oblique muscle had a normal origin. However, it had two insertions. Its tendon passed through the fibrous trochlea and was inserted to the sclera in a usual manner. In addition, it had an additional insertion to the roof of the orbit through an aponeurosis. We also observed the encroachment of the

Strabismus surgery under local anaesthesia at a tertiary eye hospital: a 5-year case review.

PURPOSE: Strabismus surgery is traditionally performed under general anesthesia (GA), however this is not safe or suitable for many patients. There has been increasing interest in strabismus surgery under local anesthesia (LA), however large datasets comprehensively exploring what is feasible under LA are lacking. METHODS: We identified 440 operations performed under LA at a tertiary eye center over a 5-year period from 2020 to 2024 and performed a retrospective review of surgical notes for all cases. Further in-depth analysis of anaesthetic notes was performed for a subset of 153 LA cases an

Diagnostic Accuracy of a Retinal Birefringence Scanning Device Compared With a Traditional Autorefraction Device.

IMPORTANCE: Photoscreening devices are commonly used to identify children at risk for eye disease, although they often demonstrate low specificity as a trade-off for high sensitivity. A newly introduced photoscreening device that uses a newer mechanism, retinal birefringence scanning, purports to detect amblyopia and strabismus directly, and may theoretically increase specificity. OBJECTIVE: To compare the diagnostic performance of 2 photoscreening devices, the Spot Vision Screener (Welch Allyn), hereafter referred to as a traditional autorefraction device, and the Blinq vision scanner (Rebio

Orbital dermoid cyst presenting with hypertropia and defective depression: a rare presentation.

INTRODUCTION: Intraconal dermoid cysts are a rare clinical entity. They are typically associated with symptoms such as proptosis, orbital pain, and diplopia. However, atypical presentations without these common features can occur, making diagnosis more challenging. METHODS: We describe a clinical case of a one-year-old child who presented with left hypertropia and restricted depression in the absence of proptosis or other clinical manifestations. High-resolution orbital and cranial magnetic resonance imaging (MRI) was performed. RESULTS: Orbital MRI revealed an intraconal lesion consistent w

Characteristics of refractive errors in children with intermittent exotropia compared with the general population.

This study compared refractive errors in children with intermittent exotropia (IXT) against the general population using two large nationwide datasets: the Korean Intermittent Exotropia Multicenter Study (KIEMS), involving 2,744 patients aged 5-18 years, and the Korean National Health Examination Survey (KNHANES), involving 1,224 participants. Age-specific spherical equivalent (SE) distributions in IXT patients were generally similar to the general population. While the non-dominant eye in IXT patients tended to be more myopic than the dominant eye, it did not exceed the myopia levels observed

Large Language Model-Based Interactive Code Generation for Developing a 3D Eye Movement Schematic.

Introduction Understanding the three-dimensional (3D) geometric relationship between extraocular muscles and the globe is essential for strabismus management. Conventional educational tools are static, and existing 3D biomechanical software requires highly specialized skills, making routine clinical use difficult. Furthermore, image-generating artificial intelligence (AI) frequently produces anatomically incorrect outputs (hallucinations). This study aimed to develop a structurally coherent, interactive 3D eye movement schematic as a proof-of-concept, using the coding capabilities of a large l

Holiday Souvenir Gone Wrong: Laser Pointer-Induced Maculopathy.

Self‑induced maculopathy caused by handheld laser pointers has emerged as an increasingly recognized source of retinal injury in children and adolescents. We report the case of a 13‑year‑old girl with pre‑existing unilateral amblyopia and small‑angle strabismus who presented with a paracentral scotoma and bilateral visual reduction following exposure to a non‑certified 100 mW green laser pointer purchased during a holiday in Spain. On presentation, best‑corrected visual acuity measured 8/10 in the right eye and 7/10 in the left eye. Fundus examination revealed a central macular retinal pigment

Neuro-Optometric Rehabilitation to Prevent Post-concussion Visual Complications: A Case Report.

A concussion is a mild traumatic brain injury (mTBI) caused by an insult to the brain from an outside force. It is a concerning health issue affecting all ages, sexes, and races. Diagnosing mTBI involves physical tests, as mTBI can't be seen through imaging, such as CT or MRI scans. Management of an mTBI is typically rest and over-the-counter medications; however, mTBI can cause complications such as photosensitivity and ocular dysfunction. In this context, this article presents a case report of a 15-year-old hockey player who was hit in the face by an elbow at high speed. The patient was init

Goltz-Gorlin Syndrome: A Case Report and Literature Review.

Goltz-Gorlin syndrome, also known as focal dermal hypoplasia (FDH), is an uncommon X-linked dominant mesoectodermal dysplasia that predominantly affects females, with most nonmosaic males presumed to be nonviable. The disorder arises from abnormal development of mesodermal and ectodermal tissues and manifests as a broad spectrum of multisystem abnormalities. It is characterized by distinctive cutaneous findings and extracutaneous involvement involving the ocular, dental, skeletal, GI, genitourinary, cardiovascular, and central nervous systems. We report the case of a 49-year-old woman who pres

Spinal Leptomeningeal Dissemination of Pineoblastoma in a 16-Year-Old Girl: A Case Report and Review of the Literature.

Pineoblastoma is a rare and highly aggressive embryonal tumor of the pineal region, predominantly affecting children and adolescents. It is classified as a CNS WHO grade 4 tumor and is characterized by a high risk of cerebrospinal fluid dissemination. Although leptomeningeal spread is a recognized feature of this tumor, spinal metastatic involvement at diagnosis remains uncommon and indicates high-risk disease. We report the case of a 16-year-old girl with no relevant past medical history who presented with a two-week history of sleep disturbances, right-sided strabismus, and projectile vomiti
