Horizontal gaze palsy with progressive scoliosis in siblings with opposing spinal curves and heart positions: is there a relationships and/or connection to etiology?

PURPOSE: Horizontal gaze palsy with progressive scoliosis (HGPPS) is a rare neurodevelopmental disorder marked by the absence of horizontal eye movement and gradual progressive scoliosis during childhood. Here, we report two biological siblings that presented with a shared diagnosis; however, the curves are in opposite directions and one presents with dextrocardia. METHODS: Two siblings presented with acute scoliosis and conjugate absence of horizontal gaze. The diagnosis was confirmed based on characteristic clinical presentation, congenital absence of horizontal gaze, and supportive radiogr

Artery of percheron infarction and a very rare finding of ping pong gaze: A case report.

The Artery of Percheron (AP) is a rare anatomical variation where both paramedian thalami and the rostral midbrain receive a supply from a single perforating artery that branches off the posterior cerebral artery. When this artery becomes infarcted, this results in a unique phenomenon of bilateral thalamic as well as midbrain ischemia. A 70-year-old male showed up in the emergency department following a fall. He was oriented and awake with abnormal, slow, conjugate horizontal gaze abnormalities typical of ping pong gaze. The rest of the neurological exam was normal. Cranial computed tomography

Ocular motor and vestibular examination in the unconscious patient-standard of care.

BACKGROUND: Eye movements play an essential role in the assessment of the unconscious patient and offer a window to the function of the brain. We review the range of ocular motor and vestibular findings in patients with impaired consciousness and present a practical approach to these patients. METHODS: Based on a structured review of the literature (Pubmed, Embase) 54 suitable citations were identified amongst 4,241 total citations. A manual search of the reference list of selected papers added another 57 papers. Based on these publications the spectrum of eye movement abnormalities in the un

Eight-and-a-Half Syndrome: A Case Report on a Rare Pontine Neuro-Ophthalmologic Presentation.

Eight-and-a-half syndrome is a distinctive neuro-ophthalmologic presentation resulting from a focal pontine lesion that simultaneously disrupts horizontal gaze pathways, internuclear connections, and facial nerve fibers. Clinically, it manifests as horizontal gaze palsy, internuclear ophthalmoplegia, and ipsilateral lower motor neuron facial weakness. We report a case of an elderly man with vascular risk factors who presented with the inability to perform conjugate horizontal gaze, adduction deficit of one eye with abducting nystagmus of the contralateral eye, and ipsilateral facial palsy, whi

Effects of Eye Movement Training and Changes of Gaze During Walking in a Patient With Oculomotor Disorder After Brainstem Hemorrhage.

We report a case of a patient who developed pontine hemorrhage and presented with eye movement disorders but was able to regain conjugate eye movement through eye movement training, resulting in improved walking ability.  The patient was a 39-year-old man who presented with cerebral hemorrhage. He was admitted to the hospital due to a pontine hemorrhage extending from the midbrain to the medulla oblongata and perforation of the fourth ventricle. Symptoms included right hemiplegia, right upper and lower limb paresthesias, ataxia of the trunk, left abducens nerve palsy, left facial nerve palsy,

What Time Is It? It Is 8 and 1/2 Time: A Rare Case of Ischemic Stroke Consistent With Eight-and-a-Half Syndrome.

Eight-and-a-half syndrome is a rare neuro-ophthalmic syndrome characterized by conjugate horizontal gaze palsy, ipsilateral internuclear ophthalmoplegia, and ipsilateral facial nerve palsy. It results from a lesion affecting the paramedian pontine reticular formation, the medial longitudinal fasciculus, and the abducens nucleus of the same side, at the level of the lower pons. We present a case of a 49-year-old man with 2-week diplopia and 3-day right-sided facial droop, drooling, and dysarthria. CT angiography of the head and neck revealed atherosclerotic disease, with decreased caliber of th

Lost in Cognition: Paraneoplastic Rhombencephalitis With Anti-Ri/Antineuronal Nuclear Antibody Type 2 (ANNA-2) Antibodies Presenting as Rapid-Onset Cognitive Decline and Ophthalmoplegia-Ataxia With Reversible Brainstem Lesion.

Anti-Ri/antineuronal nuclear antibody type 2 (ANNA-2) autoantibodies have emerged as significant markers within the realm of paraneoplastic neurological syndromes (PNS), which are recognized for their connection with onconeural antibodies and their potential to cause neurological manifestations in the presence of an underlying malignancy. If left untreated, PNS involving anti-Ri antibodies can result in substantial morbidity and mortality, and the heterogeneous range of associated syndromes and clinical presentations often complicates the diagnosis. We report the case of a 67-year-old Asian fe

Surgery for longer duration supranuclear ophthalmoplegia secondary to brain stem cavernoma: A case report and literature review.

BACKGROUND: Previous reports revealed that patients with acquired paralytic strabismus caused by central nervous system diseases are primarily affected by the etiology and treatment of the condition. Strabismus correction for these acquired paralytic strabismus should be performed as soon as the primary disease has been stabilized for 6 months in order to archive a favorable surgical outcome. CASE: We followed an infrequent case of longer-lasting supranuclear ophthalmoplegia secondary to brain stem cavernoma. OBSERVATION: A 25-year-old Chinese Han female developed aberrant head posture and i

Progressive supranuclear palsy, a case report.

Progressive Supranuclear Palsy (PSP), also known as Steele-Richardson-Olszewski syndrome, is a rare neurodegenerative disease characterized by a variety of motor and neuro-ophthalmological symptoms. We present the case of a 73-year-old male patient with a history of type 2 diabetes and high blood pressure, who consulted for gait disorders, tremors in the extremities, and difficulty controlling conjugate gaze. During physical examination, findings consistent with PSP were noted, including hypomimia, muscle rigidity, and abnormal movements. The initial misdiagnosis of Parkinson's disease and sub

A Rare Multinuclear Lesion Secondary to Multifactorial Ischemic Stroke: A Case Report on Eight-and-a-Half Syndrome.

Internuclear ophthalmoparesis (INO) is a horizontal eye movement disorder that is associated with a lesion at the medial longitudinal fasciculus (MLF). One-and-a-half syndrome occurs when the lesion involves the MLF and the ipsilateral abducens nuclei or the paramedian pontine reticular formation (PPRF) in the dorsomedial tegmentum of the pons. When the lesion is large enough, the fascicles of the facial nerve (CNVII) can also be involved, resulting in an ipsilateral facial nerve palsy. In combination with one-and-a-half syndrome, this condition becomes eightand- a- half syndrome (EHS). Here,

A Rare Case of Isolated One and Half Syndrome Associated with Choroidal Plexus Papilloma.

One-and-a-half syndrome (OAHS) is characterized by ipsilateral conjugate horizontal gaze palsy and internuclear ophthalmoplegia. We report a case of a 24-year-old gentleman who presented with a complaint of binocular diplopia, with limitation of extraocular movements. There was gaze-evoked nystagmus on attempted abduction in the right eye and down-beating nystagmus on downgaze. MRI brain showed well-defined homogeneously enhancing mass lesion in the fourth ventricle, which was surgically managed, and histopathology showed choroid plexus papilloma. This is a rare case report of Choroidal plexus

Eight-and-a-half syndrome as the first presentation of multiple sclerosis in an Asian male: a case report.

BACKGROUND: Multiple sclerosis is a diffuse chronic demyelinating disease of the central nervous system. It is relatively uncommon in the Asian population and even more so in males. Despite the usual involvement of the brainstem, eight-and-a-half syndrome remains a rare first presentation in multiple sclerosis. Only a few cases have been reported previously, but none involving the Asian population. Eight-and-a-half syndrome, a neuro-ophthalmological condition, is characterized by one-and-a-half syndrome with ipsilateral lower facial nerve palsy, which localizes lesions to the pontine tegmentum

Case report: Saccadic ping-pong gaze in progressive supranuclear palsy with predominant postural instability.

We report a 63-year-old female patient with progressive supranuclear palsy (PSP). She presented predominant postural instability and "saccadic ping-pong gaze" (SPPG). She had unprovoked falls recurrently within a year from the onset of gait disturbance. She tended to fall backward with eye closure but had no freezing of gait on examination. She showed no signs of nuchal dystonia, limb tremor, rigidity, spasticity, or ataxia. The dopaminergic response was negative. On the initial examination, her vertical eye movements were normal, but frequent macro square wave jerks and SPPG were observed. SP

Recurrent pineal parenchymal tumor of intermediate differentiation with intratumoral hemorrhage: A case report and review of the literature.

Pineal apoplexy is a rare clinical condition. Its common symptoms include headaches, nausea, vomiting, ataxia, and gaze paralysis. These symptoms are mainly caused by obstructive hydrocephalus or direct compression of the cerebellum or midbrain. There have been no previous reports on the development of a recurrent pineal parenchymal tumor of intermediate differentiation (PPTID) with intratumoral hemorrhage. We report a case of PPTID with intratumoral hemorrhage. A 44-year-old woman developed recurrent PPTID following tumor removal and ventriculoperitoneal shunting in 2010. She visited the emer

A Rare Case Report of Eight Syndrome Secondary to Syringomyelia Associated with Type I Chiari Malformation.

Eight syndrome is defined as the combination of a unilateral conjugate gaze palsy and ipsilateral seventh cranial nerve palsy. It may occur as a result of demyelinating, vascular, infectious, or compressive lesions of the brainstem localized to the caudal pontine tegmentum. A 43-year-old woman was admitted to our clinic with complaints of headache, inability to look to the left, and weakness on the left side of her face. The complaints had begun abruptly about a month before her admission. Suboccipital decompression surgery for type I Chiari malformation had been performed 10 years earlier. Ne

A Case Report on Pseudo-Internuclear Ophthalmoplegia: A Rare Manifestation of Myasthenia Gravis.

A 41-year-old male with recently diagnosed diabetes mellitus type 2 presented with drooping of the left eyelid with double vision and was found to have an adduction deficit in the left eye and nystagmus in the abducting right eye during conjugated gaze: a left-sided internuclear ophthalmoplegia (INO). A medial longitudinal fasciculus (MLF) lesion was excluded exhaustively with brain imaging. The possibility of a pseudo-INO was considered. The autoantibody profile demonstrated positivity to acetylcholine receptor (AChR) antibody. Repetitive nerve stimulation (RNS) and electromyography (EMG) wer

Horizontal Gaze Palsy and Progressive Scoliosis in Dizygotic Twins.

Horizontal gaze palsy and progressive scoliosis (HGPPS) is a rare autosomal recessive disorder caused by mutations in the ROBO3 gene. Clinical presentation consists of impairment of conjugate horizontal eye movements together with a progressive scoliosis beginning in childhood. We report dizygotic twins with HGPPS that had absence of conjugate horizontal eye movements combined with divergent strabismus and synergistic divergence. One of them also had a congenital palpebral ptosis and vertical strabismus of the right eye. Onset of scoliosis occurred in childhood with rapid progression in the se

Horizontal Gaze Palsy with Progressive Scoliosis with Overlapping Epilepsy and Learning Difficulties: A Case Report.

Horizontal gaze palsy with progressive scoliosis (HGPPS) is a rare congenital disease characterized by the absence of horizontal gaze movements, progressive scoliosis, and typical brain, cerebellum, and medullary malformations. Here we describe a pediatric HGPPS case with overlapping epilepsy and learning difficulties. A 6-year-old girl was admitted to the University Hospital of Bari for the onset of a tonic-clonic seizure. Electroencephalogram showed slow and sharp waves on the right side with the tendency to diffuse. Brain magnetic resonance imaging demonstrated malformations compatible with

Acute vertical pendular nystagmus: eye-movement analysis and review of the literature.

Vertical pendular nystagmus (PN) rarely occurs with acute pontine lesions. To hypothesize a pathophysiology for acute vertical PN, we analyzed the clinical characteristics and quantitative eye-movement recordings of one new case with acute vertical PN and an additional 11 patients from the literature. Most patients had extensive pontine lesions causing either the locked-in syndrome or unresponsiveness, but two conscious patients had focal lesions restricted to the paramedian caudal pontine tegmentum. All patients presented a complete or partial horizontal gaze palsy, and about half showed ocul
