BACKGROUND AND AIM: Intracranial vertebrobasilar artery (VBA) stenosis portends a stroke and death rate of 8.5-22.8% annually despite medical therapy. Stenting has emerged as a treatment option but also carries substantial risk. Awake stenting under local anesthesia to minimize major procedural complication was investigated.
METHODS: Between January 2007 and December 2008, 43 of 46 consecutive patients with severe symptomatic intracranial VBA stenosis underwent elective angioplasty assisted with self-expanding Wingspan stent under local anesthesia at our institute. All data were collected pro

Neurologic events following bee stings are very rare. We report a 59-year-old man who became drowsy with slurred speech following multiple bee stings. In the hospital, he was found to have left-sided hemplegia, seventh cranial nerve palsy, and left conjugate gaze palsy. Further investigation revealed dyslipidemia, impaired glucose tolerance, and a middle cerebral artery territory infarct. His limb weakness and speech improved before his discharge from the hospital.

INTRODUCTION: Oculomotor deficiencies in multiple sclerosis (MS) are frequently characterized by internuclear ophthalmpoplegia or isolated abduction or adduction palsies. Complete unilateral conjugate gaze paralysis and the "one and a half" syndrome are rare. Complete bilateral horizontal gaze paralysis has been exceptionally reported.
OBSERVATION: Here, we describe an unusual oculomotor paralysis as a suspected first event of MS. A 24-year-old woman with an uneventful medical history presented for sudden onset of binocular diplopia. On examination, abduction and adduction saccades were impos

BACKGROUND AND AIM: Ocular gaze deviation (OGD) is a well known clinical observation (Prevost's sign) in patients with acute cerebral ischemic stroke. Although OGD has been observed on CT in acute stroke, no investigation has quantified the degree of OGD in acute stroke.
MATERIAL AND METHODS: A blinded prospective comparison was performed of two groups of adult patients who underwent CT of the brain. Group 1 comprised patients with acute hemiplegia or hemiparesis due to middle cerebral artery ischemic stroke. Group 2 included ambulatory outpatients with a history of headache but no clinical n

An 88-year-old female with atrial fibrillation and hypertension, was admitted to our hospital with sudden onset diplopia and somnolence. She had right hemiparesis with bilateral positive Babinski's sign. Additionally, there was bilateral blepharoptosis with right esotropia. With regard to extraocular movement, the patient demonstrated conjugate upgaze palsy and left monocular down gaze palsy (vertical one-and-a-half syndrome: VOHS). Horizontal gaze in the left eye was completely impaired and there was limited abduction of the right eye. Magnetic resonance imaging of the brain showed left thala

One-and-a-half syndrome is a clinical disorder featuring extraocular movements characterized by horizontal conjugate gaze palsy with internuclear ophthalmoplegia. It usually results from a unilateral lesion of the midbrain, and the most common cause of this syndrome in young women is multiple sclerosis. We report the case of a 38-year-old woman diagnosed as having acute myeloblastic leukemia presenting with characteristic neurologic and imaging features of one-and-a-half syndrome. Hyperleukocytosis, cancer procoagulants, tissue factor expression, and the increased proteolysis of coagulation fa

BACKGROUND: The human nucleus prepositus hypoglossi (NPH), which is known to be a neural integrator of horizontal eye movement, may also serve vestibular function. The present study investigated the clinical spectrum and etiological mechanism of isolated, small infarctions involving the NPH area demonstrated on MRI.
METHODS: The subjects of this study were 18 consecutive patients with a small infarction involving the NPH that was demonstrated by diffusion-weighted MRI. We assessed their clinical features according to the level involved (pons or medulla oblongata) and determined etiological me

Trochlear nerve palsy leads to kinematic aberrations of both the paretic and the unaffected eye. During dynamic head roll, the rotation axis of the covered paretic or unaffected eye deviates inward, while the rotation axis of the viewing paretic or unaffected eye aligns with the line of sight; this convergence of rotation axes increases with gaze moving in the direction of the unaffected eye. During downward saccades, the trajectories of both eyes curve towards the unaffected side; these curvatures increase when the head is rolled to the affected side and gaze directed to the unaffected side.

Horizontal gaze palsy with progressive scoliosis (HGPPS) is a rare, inherited disorder characterized by a congenital absence of conjugate horizontal eye movement with progressive scoliosis developing in childhood in patients who are otherwise neurologically intact. Detailed structural neuroimaging studies demonstrated abducens nerves and the absence of fibrosis in the extraocular muscles, and a remarkably dysmorphic hindbrain, with hypoplasia and flattened, butterfly-like medulla with deep midline cleft. Diffusion tensor imaging further demonstrated a widespread lack of crossing fibres in the

Horizontal gaze palsy with progressive scoliosis (HGPPS) is a rare congenital disorder characterized by absence of conjugate horizontal eye movements and progressive scoliosis developing in childhood and adolescence. We present a child with clinical and neuroimaging findings typical of HGPPS. CT and MRI of the brain demonstrated pons hypoplasia, absence of the facial colliculi, butterfly configuration of the medulla and a deep midline pontine cleft. We briefly discuss the imaging aspects of this rare entity in light of the current literature.

Horizontal eye movements are conducted by the medial rectus and the lateral rectus muscles, which are innervated by the oculomotor nerve (cranial nerve III) and the abducens nerve (cranial nerve VI), respectively. The oculomotor and the abducens nuclei are interconnected by a tract in the brainstem named the medial longitudinal fasciculus (MLF). Through the MLF, the actions of the oculomotor and the abducens nuclei are coordinated, generating conjugate horizontal eye movements. The disorders of horizontal eye movement that are caused by brainstem lesions are classified into three groups: (a) l

We reported a case of pontine infarction presenting with conjugate deviation and unilateral conjugate gaze palsy. A 75-year-old man was admitted to our hospital because of vomiting. On the day of admission, his neurological findings showed only conjugate deviation to the right without consciousness disturbance, hemispatial neglect, and hemiparesis. On day 5 after admission, the conjugate deviation to the right disappeared, but he could not gaze left with either of his eyes. The pathological lesions, therefore, were considered to be a unilateral paramedian pontine reticular formation (PPRF) and

A small localized infarction in the dorsal pontine area can cause various eye-movement disturbances, such as abducens palsy, horizontal conjugate gaze palsy, internuclear ophthalmoplegia, and one-and-a-half syndrome. However, complete loss of vertical saccades and pursuit with horizontal gaze palsy has not been reported previously in a patient with a small pontine lesion. We report a 67-year-old man with a small dorsal caudal pontine infarct who exhibited total horizontal gaze palsy as well as loss of vertical saccades and pursuit.

BACKGROUND AND PURPOSE: The authors studied clinical features and lesion patterns associated with conjugate eye deviation (CED) in acute stroke.
METHODS: Consecutive patients with supratentorial stroke (<6 hours after symptom onset) were prospectively recruited. CED was classified according to the National Institute of Health Stroke Scale (NIHSS) item "gaze palsy" as being forced, partial, or absent. Lesion patterns on diffusion-weighted images (DWI) were analyzed in 46 patients using a normalization procedure for direct comparisons between patients. Perfusion data were analyzed for a subset

In this report, we describe a patient with left conjugate gaze palsy and internuclear ophthalmoplegia on the opposite gaze one-and-a-half-syndrome [11/2 syndrome]. Magnetic resonance imaging of the brain showed nodular enhancing brain stem lesions. After excluding other potential diseases, tuberculoma was thought to be the underlying etiology. Recovery was achieved 8 weeks after initiation of anti-tuberculous anti-TB therapy. The isolated nature of the tuberculoma and the association with this rare syndrome is highlighted.

We present a patient with progressive supranuclear palsy (PSP) who had a bilateral internuclear ophthalmoplegia (INO) that could be fully overcome by the oculocephalic maneuver. In addition to being an unusual finding in the clinical setting of PSP, this phenomenon has interesting implications for the functional control of conjugate horizontal gaze.
