Fifty one patients with abnormalities of horizontal gaze were studied with magnetic imaging of the brain (MRI) and eye movement recordings to identify the loci of lesions responsible for isolated abducens palsy, conjugate gaze palsy and different types of internuclear ophthalmoplegias. The lesions responsible for a particular disorder were identified by overlapping enlarged drawings of the individual scans at comparable brain-stem levels and identifying the areas where the abnormal MRI signals intersected. A statistical procedure was devised to exclude the possibility that the areas of overlap

We reviewed the neuro-ophthalmic findings in 177 subjects with the acquired immunodeficiency syndrome (AIDS) or AIDS-related complex who underwent an eye examination in one center from January 1984 to May 1989. The findings included ocular motor nerve palsies (five cases), papilledema (two cases), cytomegalovirus optic neuritis (two cases), cortical blindness (one case), conjugate gaze palsy (one case), and altitudinal visual field defect (one case). These findings were attributed to central nervous system toxoplasmosis (four cases) or lymphoma (one case), cryptococcal meningitis (two cases),

Midbrain lesions may give rise to the most complex eye movement disorders observed in clinical neurology. Three main types of dysfunction may be delineated, which may be combined: 1) intra-axial fascicular syndromes of the third and fourth cranial nerves; 2) nuclear syndromes of the third and fourth cranial nerves; 3) prenuclear and supranuclear syndromes, in which vertical gaze dysfunction is the main abnormality, but frequently with associated horizontal gaze dysfunction. The fascicular syndromes of the third nerve may be diagnosed when the oculomotor palsy is associated with some specific n

We report a patient with a one-and-a-half syndrome due to an isolated small infarction in the left rostral part of the paramedian pontine reticular formation and the ipsilateral medial longitudinal fasciculus. Oculocephalic movements toward the left were preserved (dissociated ipsilateral horizontal conjugate gaze palsy).

Ocular motility manifestations of focal brainstem dysfunction were the initial clinical features in three patients with human immunodeficiency virus (HIV) infection. These included conjugate gaze palsy with ipsilateral facial paresis, bilateral abducens palsy and a gaze paresis, and homolateral internuclear ophthalmoplegia and abducens nerve paresis. Two patients had focal brainstem lesions as evidenced on neuroimaging. The third showed concurrent infection with Treponema pallidum.

Recent experimental studies allow more precise functional-anatomical correlations in patients with ocular motor disorders. The abducens nucleus contains two groups of cells that together subserve ipsilateral horizontal conjugate gaze: Motoneurons with axons that innervate the lateral rectus and internuclear neurons with axons that cross the midline, ascend in the medial longitudinal fasciculus (MLF) and impinge upon medial rectus motoneurons. The pontine paramedian reticular formation (PPRF) contains burst neurons for horizontal saccades. The rostral interstitial nucleus of the MLF (riMLF) in

The case is reported of a Swiss woman with neuro-Behçet. She displayed all major findings of this disease, e.g. oral and genital ulcers, chorioretinitis, dermatologic lesions and arthritis, as well as a severe neurological syndrome consisting of conjugated paralysis of the vertical gaze (Parinaud's syndrome) and disorientation. Lumbar puncture confirmed meningoencephalitis. The success of cortisone therapy in the treatment of neuro-Behçet, and the potential of colchicine for prevention of new attacks of the disease, are discussed.

A 58-year-old woman, with a past history of classic migraine since youth, suddenly experienced blurred vision and flexor spasms of her left hand, followed by a right hemicrania and photophobia, similar to previous attacks of migraine. Within a few hours a progressive left hemiplegia and paralysis of left conjugate gaze developed. Severe right hemicrania continued. CT brain scans showed a progressing large right parietotemporal infarct. Her level of consciousness declined and she died ten days after admission to hospital. The autopsy showed a large infarct in the area of supply of the right mid

A patient with an infarct limited to the paramedian thalamus and upper mesencephalon on the right side suffered a conjugate upgaze palsy associated with a monocular paresis of downward gaze in the ipsilateral eye (vertical "one-and-a-half" syndrome). This paresis involved tonic and phasic components. Vertical oculocephalic movements and conjugate horizontal gaze were normal. It is suggested that the unilateral lesion destroyed the fibres of the posterior commissure and the descending fibres to the ipsilateral subnucleus of the inferior rectus and contralateral subnucleus of the superior obliqu

Five patients with "locked-in" syndrome and dysconjugate palsy of horizontal gaze were studied. In all cases internuclear ophthalmoplegia due to dysfunction or destruction of the median longitudinal fasciculus was combined with an ipsilateral gaze palsy, producing the "one-and-a-half" syndrome. Clinical and electro-oculographic examination suggested involvement of the paramedian pontine reticular formation when all ipsilateral saccades were abolished, when exotropia of the contralateral eye was present, and when vestibular stimulation showed full conjugate deviation to the damaged side. Unilateral paramedian involvement of the midbrain tegmentum causes monocular paralysis of adduction in the ipsilateral eye, paresis of contralateral saccades in the opposite eye, and conjugate paresis of ipsilateral smooth pursuit. The adduction paralysis can be nuclear, or internuclear from a lesion in the medial longitudinal fasciculus. This distinctive midbrain syndrome of horizontal gaze paresis is exemplified by means of quantitative infrared oculographic, radiological, and neuropathological correlation in two patients with predominantly paramedian midbrain tumors involving the mesencepha

A case of complete rightward conjugate gaze paralysis, with electro-oculographic recordings, is reported. The only additional feature on clinical examination was a right infranuclear facial paresis. Radiographs of the chest showed multiple infiltrates in both lung fields; lung biopsy with ultrastructural study led to the diagnosis of histiocytosis X. A CT scan showed a single small tumour-like lesion, hyperdense and clearly defined, lying centrally in the fourth ventricle with a small ventral extension located just to the right of the midline. With appropriate treatment all the clinical signs

Three patients, who subsequently developed the locked-in state characterised by quadriplegia and mutism with an alert sensorium, initially had mild dysarthria and uncrossed hemisensory or hemimotor deficits involving the face and ipsilateral extremities. Case one initially mimicked a left cerebral lesion with right hemisensory deficits, a mild right facial paresis and a right homonymous field deficit. Case two initially developed both left hemimotor and hemisensory deficits and later developed a paresis of right conjugate gaze. Case three presented with left hemimotor deficit, and mild paresis

Eye movements in a patient with Fisher's syndrome were examined clinically and documented photographically when palsies were most severe and on repeated occasions during the recovery period. Two recordings of horizontal eye movements were made using the infrared reflection method. Particular attention was paid to signs indicating supranuclear eye movement disorders. We also reviewed cases in which signs suggesting brain-stem involvement were reported. Mild ptosis in the presence of severe ophthalmoplegia, preservation of Bell's phenomenon despite paralysis of voluntary upward gaze, conjugate p

The one-and-a-half syndrome is a clinical disorder of extraocular movements characterized by a conjugate horizontal gaze palsy in one direction plus an internuclear ophthalmoplegia in the other. The syndrome is usually due to a single unilateral lesion of the paramedian pontine reticular formation or the abducens nucleus on one side (causing the conjugate gaze palsy), with interruption of internuclear fibers of the ipsilateral medial longitudinal fasciculus after it has crossed the midline from its site of origin in the contralateral abducens nucleus (causing failure of adduction of the ipsila

Hypertension intracerebral hemorrhages are caused by leakage from small penetrating branches of larger cerebral vessels. In the brainstem, these vessels penetrate in a median, paramedian, and more lateral orientation. As illustrated by three patients reported here, hemorrhage from the lateral vessels may be limited to the lateral tegmentum, or spread to the dorsal basis pontis. The syndrome is relatively consistent and includes (1) ipsilateral conjugate gaze palsy, (2) ipsilateral internuclear ophthalmoplegia, (3) small reactive pupils with a smaller pupil ipsilateral to the lesion, (4) limb a

Eye movements were studied in a sister and brother with familial, congenital paralysis of horizontal gaze. Horizontal and vertical eye movements were recorded with DC electro-oculography and analyzed with a laboratory computer. All horizontal, conjugate eye movements were absent (saccades, pursuit, optokinetic nystagmus, vestibulo-ocular response, and visual-vestibular responses). Voluntary vergence eye movements were preserved and were used to track visual targets. An involuntary, horizontal, pendular nystagmus was found to represent disconjugate, smooth, vergence eye movements. Vertical sacc

Permanent upward conjugated ocular deviations are exceptional in conscious subjects : 8 cases only have been reported. They are always associated with a more or less selective paralysis of downward gaze. A further case is reported, with results of oculographic and computed tomography examinations. The latter showed a median or bilateral lesion at the diencephalo-mesencephalic junction. The mechanism provoking the deviation was clarified by studying vestibular movements which appeared abnormally ample and rapid. The authors suggest that the deviation of the eyes could result from this release o
