Internuclear ophthalmoplegia associated with ipsilateral downbeat nystagmus and contralateral incyclorotatory nystagmus.

A case of left internuclear ophthalmoplegia combined with downbeat nystagmus and right incyclorotatory nystagmus on left downward gaze, left exotropia and skew deviation is presented. All signs subsided completely within about 3 months after the onset of the disease. The etiology was possibly of vascular origin. The potential site of the lesion causing these oculomotor signs is discussed.

Internuclear ophthalmoplegia following arch aortography. Occurrence in association with an occult vertebral artery aneurysm.

The onset of internuclear ophthalmoplegia immediately following arch aortography in a previously healthy 49-year-old female is presented. This is the second description of internuclear ophthalmoplegia developing after cerebral angiography and represents the first report of its occurrence in association with an occult traumatic vertebral artery aneurysm.

Analysis of characteristic eye movement abnormalities in internuclear ophthalmoplegia.

Quantitative electro-oculographic recording techniques were used to analyze four characteristic eye movement abnormalities in 21 patients with internuclear ophthalmoplegia (INO). The frequency of each of the abnormalities was determined to suggest a pattern that is the most sensitive in detecting the syndrome of INO. Slowing of the adducting saccade was the most frequently found abnormality, being present in all patients. The other characteristic eye movement disorders were found less frequently: dissociated nystagmus at 30 degrees of eccentric gaze, dysmetria of the abducting eye, and limitat

Eye movement disorders in multiple sclerosis and optic neuritis.

Horizontal saccadic and smooth pursuit eye movements were studied in 84 patients with multiple sclerosis (MS) and 21 patients with optic neuritis (ON). The MS patients were clinically classified as 'definite', 'probable', or 'possible'; subclinical eye movement disorder was found in 80 per cent of the definite, 74 per cent of the probable and 60 per cent of the possible category. Five of the ON patients (25 per cent) showed a subclinical eye movement deficit; these 5 were young patients with a recent history of ON. In a group of 27 MS patients with symptoms of spinal cord involvement only, 14

Bilateral internuclear ophthalmoplegia reversed by naloxone.

We encountered an apparent bilateral internuclear ophthalmoplegia (INO) in a stuporous patient who used narcotics and benzodiazepines and had taken phenytoin sodium for drug-withdrawal seizures. The INO was promptly reversed by administration of the narcotic antagonist naloxone, which binds opiate receptors. This suggests the INO resulted from a specific toxic effect of narcotics, but opiate receptors have not been anatomically demonstrated within the medial longitudinal fasciculus or associated structures. Stimulation of inhibitory GABA-ergic (alpha-aminobutyric acid) vestibulo-ocular fibers

Bilateral internuclear ophthalmoplegia in carcinomatous meningitis.

A 53-year-old man with oat cell carcinoma of the lung developed a bilateral internuclear ophthalmoplegia in association with carcinomatous meningitis. While bilateral internuclear ophthalmoplegia has been described in one case of cryptococcal meningitis, this is the first reported case occurring in association with carcinomatous meningitis.

Internuclear ophthalmoplegia in patients with toxic coma frequency, prognostic value, diagnostic significance.

Internuclear Ophthalmoplegia (INO), a dysfunction of the medial longitudinal fasciculus, is frequently seen in toxic coma (7 out of 70 cases). INO is most often bilateral and can be associated with different stages of coma. Such an association is a strong argument for the toxic etiology of a coma. But INO has no value in determining the source of intoxication and is no prognostic indicator for the outcome.

Saccadic intrusions in internuclear ophthalmoplegia.

Saccadic intrusions disrupted forward fixation in four patients with bilateral internuclear ophthalmoplegia (INO). Three had multiple sclerosis and one had brainstem infarction. Sporadic bursts of monocular abducting saccades were recorded in either eye. Each saccade was followed by a slower return to midposition. The presence of the INO prevented conjugate saccades of the fellow eye. The adducting phases were faster than convergence eye movements or the glissadic smooth eye movements that sometimes follow saccades. Dynamic overshoot of the abducting saccadic phase may contribute to the relati

CSF myelin basic protein levels in acute optic neuritis and multiple sclerosis.

Normal CSF-MBP levels as determined by a RIA were less than 6.2 ng/ml CSF (mean 3.9). Eighty percent of patients with acute optic neuritis have CSF-MBP levels greater than 6.2 ng/ml (mean 7.6 ng/ml CSF). Five of 7 patients with acute internuclear ophthalmoplegia due to an initial exacerbation of demyelination have CSF-MBP levels above 6.2 ng/ml (mean 6.8 ng/ml). Fifty percent of MS patients with chronic progressive disease have CSF-MBP levels above 6.2 ng/ml (mean 6.7 ng/ml). MS patients experiencing monosymptomatic exacerbations show elevated CSF-MBP levels in 75% in 75% of cases (mean 8.2 ng

Saccadic velocity measurements in strabismus.

Traditional evaluation of strabismus has included cover test measurements, evaluation of the range of ocular rotations, and an array of subjective sensory tests. These studies could not always differentiate paresis of an extraocular muscle from restrictions and from various neuro-ophthalmic motility disorders. The measurement of horizontal and vertical saccadic movements can provide an objective test of rectus muscle function. Using EOG, saccades can be recorded easily, inexpensively, and repeatably at any age. In ocular muscle paresis or paralysis, saccadic speed is reduced mildly to markedly

The one-and-a-half syndrome--a unilateral disorder of the pontine tegmentum: a study of 20 cases and review of the literature.

The one-and-a-half syndrome is a clinical disorder of extraocular movements characterized by a conjugate horizontal gaze palsy in one direction plus an internuclear ophthalmoplegia in the other. The syndrome is usually due to a single unilateral lesion of the paramedian pontine reticular formation or the abducens nucleus on one side (causing the conjugate gaze palsy), with interruption of internuclear fibers of the ipsilateral medial longitudinal fasciculus after it has crossed the midline from its site of origin in the contralateral abducens nucleus (causing failure of adduction of the ipsila

[Fischer's "one and a half" syndrome: associated paralysis of horizontal eye movements and anterior internuclear ophthalmoplegia: report of 5 cases (author's transl)].

Five cases of Fischer's "one and a half" syndrome are reported. A complex central ophthalmoplegia provokes paralysis of horizontal eye movements on the affected side in both eyes and an internuclear ophthalmoplegia on the same side. No horizontal movement, except in convergence, is possible in the eye on the affected side, the other eye exhibiting movement between the median line and abduction only. Three patients with this syndrome were examined by oculography. The outcome was fatal in one case, pathological examination demonstrating widespread damage to half the pontine protuberance. In anot

[Internuclear ophthalmoplegia in multiple sclerosis].

In the years 1963-1981 internuclear ophthalmoplegia syndromes were observed 106 times in 94 patients treated in hospital for multiple sclerosis. Besides the typical syndromes of anterior and posterior internuclear ophthalmoplegia, Webino syndrome and the one-and-a-half syndrome in 4 cases syndromes were observed with superimposition of bilateral syndromes of anterior and posterior internuclear ophthalmoplegia. For these cases the author suggests the term--complete internuclear ophthalmoplegia.

Acquired pendular nystagmus: its characteristics, localising value and pathophysiology.

Investigations were made of 16 patients with acquired pendular nystagmus and a further 32 cases reported in the literature were reviewed. Amongst our own patients two thirds had multiple sclerosis, almost one third a cerebrovascular accident or angioma and two had optic atrophy with squint. The nystagmus took forms which could be monocular or binocular, conjugate or disconjugate and could involve movements about single or multiple axes. Spectral analysis was used to characterise the amplitude and frequency of the movements and to estimate the degree of relationship (coherence) between movement
