Optokinetic nystagmus: a review of pathways, techniques and selected diagnostic applications.

There are a myriad of conditions producing abnormal OKN responses. In most of these conditions the diagnosis can be made without recourse to OKN. However, in the five conditions described in this paper (suspected poor visual acuity in infants and young children, parietal lesions, subclinical internuclear ophthalmoplegia, Parinaud's syndrome, and aberrant regeneration of the third nerve) OKN can greatly facilitate the diagnosis. A review of the neural pathways of the OKN response and an explanation of the various equipment and techniques used to assess OKN are included.

Lateral tegmental brainstem hemorrhages.

Hypertension intracerebral hemorrhages are caused by leakage from small penetrating branches of larger cerebral vessels. In the brainstem, these vessels penetrate in a median, paramedian, and more lateral orientation. As illustrated by three patients reported here, hemorrhage from the lateral vessels may be limited to the lateral tegmentum, or spread to the dorsal basis pontis. The syndrome is relatively consistent and includes (1) ipsilateral conjugate gaze palsy, (2) ipsilateral internuclear ophthalmoplegia, (3) small reactive pupils with a smaller pupil ipsilateral to the lesion, (4) limb a

Reversible bilateral internuclear ophthalmoplegia due to head trauma: a case report.

This case report concerns a patient who developed reversible bilateral internuclear ophthalmoplegia as an immediate consequence of head trauma. To our knowledge, only one case of internuclear ophthalmoplegia related to head trauma has been previously reported in the literature. The mechanism by which trauma can produce a relatively isolated brain stem lesion to the medial longitudinal fasciculus is unclear. Some experimental data on the mechanisms of cerebral concussion would indicate that the biomechanical forces due to impact acceleration of the head exert their predominant effect on the pos

Internuclear ophthalmoplegia after head trauma.

Internuclear ophthalmoplegia results from impairment of the medial longitudinal fasciculus. Multiple sclerosis is usually the cause in bilateral cases while a vascular lesion is commonly implicated in unilateral cases. Head trauma is a rare cause. We describe the case of a 52-year-old man who developed unilateral internuclear ophthalmoplegia following a head injury suffered in an automobile accident. Gradual improvement in the internuclear ophthalmoplegia occurred over a six-month follow-up period. Review of the literature reveals only ten previous reports of internuclear ophthalmoplegia secon

Internuclear ophthalmoplegia in Fabry's disease.

A 16-year-old boy with Fabry's disease had sudden onset of diplopia unassociated with any other neurologic symptoms. A right internuclear ophthalmoplegia characterized by optokinetic phenomena and ocular dysmetria was demonstrated. The diplopia resolved spontaneously in six weeks. This is the first reported case of internuclear ophthalmoplegia occurring with Fabry's disease and represents one of the youngest patients to have unilateral internuclear ophthalmoplegia presumably caused by vascular occlusive disease.

Unilateral internuclear ophthalmoplegia. The lack of inhibitory involvement in medial rectus muscle activity.

Inability to adduct the left eye beyond the midline was found in a patient after brain stem infarction. Pursuit movements, refixation saccades, and the vestibulo-ocular reflex were restricted to the temporal hemifield. Only near convergence led to an adduction of 15 degrees in the left eye. Nasally directed saccades were slow, and temporally directed saccades showed glissadic back-drift. Electromyography revealed a lack of burst activity in the left medial rectus; however, inhibition of the left (homolateral) and the right (contralateral) medial rectus muscles during off-saccades and in off-po

Bilateral internuclear ophthalmoplegia after intrathecal chemotherapy and cranial irradiation.

A 26-year-old man developed transient bilateral internuclear ophthalmoplegia with exotropia after cranial irradiation and intrathecal administration of methotrexate for lymphoma. Resolution of the ophthalmoplegia and the decrease in abnormally high levels of cerebrospinal fluid myelin basic protein after discontinuation of intrathecal medication suggested demyelination from chemotherapy and irradiation.

Ocular motor signs in some metabolic diseases.

Ocular motor disturbances are described with a miscellany of metabolic disturbances. Horizontal gaze abnormalities, often simulating congenital ocular motor apraxia, characterized Gaucher's disease. Vertical gaze abnormalities, especially downgaze paralysis, characterized what is generally considered a variant of Niemann-Pick disease, or sea-blue histiocytosis, but which we prefer to call the "DAF" syndrome. A form of internuclear ophthalmoplegia but with nystagmus of the adducting eye characterized abetalipoproteinemia. epileptiform eyelid and eye movements occurred in a case of methylmalonoh

[Internuclear ophthalmoplegia and associated oculomotor syndrome (author's transl)].

Fifteen patients with internuclear ophthalmoplegia (INO) were examined by simultaneous electro-oculography of each eye. In 11 patients the INO was bilateral and in 4 patients unilateral. In 6 of the bilateral cases, the INO was asymmetric. A classic presentation of the INO was shown by only 2 patients. The rest had additional disorders of eye mobility (INO plus). The high percentage of additional affections, which were not often clinically apparent, consisted of slowing of abduction (45%), impairment of vertical movements (80%) and convergence (30%), and dysmetric overshoot of the abducing eye

Posterior internuclear ophthalmoplegia of Lutz.

Discrete lesions interrupting the fiber tracts that connect the pontine center for conjugate horizontal gaze and the ipsilateral abducens nucleus create the syndrome of Lutz's posterior internuclear ophthalmoplegia. In this syndrome the lateral rectus on the side of the lesion fails to abduct the eye normally on attempted horizontal gaze to the same side. Adduction of the contralateral eye is normal. This syndrome may be differentiated from the more common abducens nerve palsy by noting the orthotropic position of the eyes in primary position, the absence of primary and secondary deviations wi

[Oculomotor disorders associated with Arnold-Chiari malformations (author's transl)].

Oculographic examinations were conducted in 24 cases of Arnold-Chiari malformation. Disorders in vertical eye movements with a spontaneous vertical nystagmus were frequently observed, together with disturbances in visual tracking. There was no case of paralysis of lateral eye movements, but an adduction internuclear ophthalmoplegia might be present. Though these anomalies are not specific, they differ for example from those usually observed in multiple sclerosis, which is often a differential diagnosis in such cases. Oculography therefore may be of particular value.

Brainstem auditory evoked responses in 200 patients with multiple sclerosis.

Brainstem auditory evoked responses (BAERs) were recorded from 202 patients with definite, probable, or possible multiple sclerosis (MS). Definitions of abnormality were based only on interwave separations and the wave I/wave V amplitude ratio. Thirty-two percent of the patients had abnormal BAERs, and the presence of clinically unsuspected lesions was revealed by BAER abnormalities in 7.4%. Thirty-five percent of the patients who had nystagmus and 53% of those who had internuclear ophthalmoplegia at the time of testing had BAER abnormalities. Forty-five percent of the abnormalities were elici

Disordered inhibition in internuclear ophthalmoplegia: analysis of eye movement recordings with computer simulations.

High resolution infra-red oculographic recordings were obtained in 19 patients with clinically evident internuclear ophthalmoplegia. The major findings were attenuated phasic and tonic components of adducting saccades, fractionated phasic components of abducting saccades, equally long durations for phasic components of adducting and abducting saccades with refixation, and nasal drift of the abducting eye which initiated abducting nystagmus. Ipsilateral gaze paresis and abduction lag were occasionally associated with primarily unilateral cases of internuclear ophthalmoplegia. These findings wer

A clinical comparative study of multiple sclerosis and neuro-Behçet's syndrome.

Clinical comparisons were made between Japanese patients with multiple sclerosis (66 cases) and neuro-Behçet's syndrome (23 cases). Those with neuro-Behçet showed marked male predominance, while those with multiple sclerosis showed slight female preponderance. Both showed encephalomyelopathy disseminated in time and space. Patients with multiple sclerosis, however, showed a more polyphasic course, whilst those with neuro-Behçet showed a more progressive one. In multiple sclerosis optic neuritis, acute transverse myelitis, painful tonic seizures, mental disturbance and internuclear ophthalmople
