From famine to NAFLD: long-term association between fetal famine exposure and adult non-alcoholic fatty liver disease.

BACKGROUND: Malnutrition in early life may change the structure and function of the body, which lead to a number of diseases in adulthood. There are very few studies in health examination population about the prevalence of NAFLD (non-alcoholic fatty liver disease) in adulthood after early experiences of malnutrition. METHODS: A total of 4,188 participants were selected as research objects from a large tertiary Class A general hospital in western China. We divided the subjects into four groups based on their ages at the time of exposure to the famine during the era of the Great Chinese Famine.

A Congenital Melanocytic Nevus With Halo Phenomenon and Complete Regression Following Surgical Excision.

We present a case of a 4-year-old girl who underwent two planned staged surgical resections of a benign-appearing medium-sized congenital melanocytic nevus (CMN) on the right buttock. The nevus was 4 cm in diameter, medium brown in color, and exhibited central hypertrichosis with mild cobblestoning. Ten days after the first excision, a striking depigmented halo was noted around the residual nevus, which over time progressed to complete clinical regression of the residual CMN. While spontaneous development of the halo phenomenon in CMNs has been previously reported, no cases of this immune resp

Flow in screen-based gamified virtual reality sports: development and validation of a behaviorally anchored proxy rating scale for adolescents with autism spectrum disorder.

BACKGROUND: Adolescents with autism spectrum disorder (ASD) face substantial barriers to physical activity participation because of motor coordination difficulties, sensory hypersensitivity, and motivational challenges. Screen-based gamified virtual reality (VR) sports have emerged as a promising alternative, with flow identified as a key psychological mechanism underlying their effects. However, alexithymia and atypical interoceptive processing in adolescents with ASD render conventional self-report flow scales inappropriate for this population. This study aimed to develop and validate a beha

Differences in Left-Right Vestibulo-Ocular Reflex Gain in the Video Head Impulse Test.

Objectives: The objective of this study was to evaluate left-right symmetry of vestibulo-ocular reflex (VOR) gain on the video head impulse test (vHIT) and its diagnostic implications relative to caloric test results. Methods: Patients presenting with dizziness who underwent a bithermal caloric test and vHIT were retrospectively analyzed. Of 764 patients identified, nine with bilateral vestibular hypofunction were excluded, leaving 755 patients for analysis (451 showing symmetric caloric responses; 304 with unilateral vestibular hypofunction (UVH)). Horizontal VOR gain was compared between dir

Case report: A Newly Identified G-protein Subunit α-11 Loss-of-Function Variant Causing Familial Hypocalciuric Hypercalcemia Type 2 in an Italian Kindred.

BACKGROUND: FHH2 is the rarest subtype of familial hypocalciuric hypercalcemia disorders affecting the CaSR signaling pathway. It is caused by heterozygous loss-of-function mutations in the GNA11 gene, encoding Gα11 protein. Only seven GNA11 pathogenic variants have been reported so far. CASE DESCRIPTION: We report the case of a 39-year-old man referred to our outpatient clinic for a non-toxic multinodular goiter. During biochemical examination, mild hypercalcemia with normal PTH levels and low urinary calcium excretion (calcium-creatinine clearance ratio< 0.01) was detected. He had no eviden

Reactivation of Thyroid Eye Disease: Incidence, Timing and Clinical Phenotype.

BACKGROUND: Thyroid eye disease (TED) is an immune-mediated orbitopathy with an active inflammatory phase followed by quiescence. Reactivation after apparent quiescence is recognised but poorly characterised. We aimed to describe the incidence, timing, and phenotype of TED reactivation and explore potential risk factors and triggers. METHODS: Retrospective cohort study of 727 consecutive patients with TED (2015-2025). Reactivation was defined as new or worsening TED after ≥ 1 year of documented inactivity. Data included demographics, smoking status, interval from quiescence, phenotype, severi

Cardiometabolic multimorbidity and cognitive-functional transitions across international ageing cohorts.

BACKGROUND: Cardiometabolic multimorbidity (CMM) may accompany changes in cognition and everyday function in later life. We reassessed these associations across international ageing cohorts while addressing outcome structure, unequal threshold severity, measurement precision, repeated observations, mortality and non-death attrition. METHODS: We harmonised repeated observations from HRS, ELSA, CHARLS, KLoSA, MHAS and SHARE. Within-interval change in a complete four-condition CMM count (hypertension, diabetes, heart disease and stroke) was used in continuous analyses, whereas transition analyse

Active surveillance and risk prediction of piperacillin/tazobactam‑associated hypokalaemia: development and external validation of a clinical nomogram.

INTRODUCTION: Piperacillin/tazobactam (TZP) is widely used in hospitalised adults. Real‑world data suggest a higher risk of hypokalaemia than previously recognised. AIM: To investigate incidence, severity and risk factors of TZP‑induced hypokalaemia, and to develop and externally validate a nomogram for individualised risk prediction. METHOD: This retrospective study included hospitalised adults receiving TZP (2021-2025). TZP‑induced hypokalaemia was defined as serum potassium < 3.5 mmol/L ≥ 48 h after TZP initiation. Logistic regression identified predictors. A nomogram was constructed. Mod

Passive sensing of gait and medication-related fluctuations in Parkinson's disease.

BACKGROUND: Gait impairment is a hallmark symptom of Parkinson's Disease (PD). Traditional clinical assessments cannot capture real-world motor fluctuations, as they are sparsely performed. We validated the use of nearables, passive sensing technologies, including Kinect RGB-D cameras and ultra-wideband (UWB) radar, for continuous, objective assessment of gait fluctuations in PD within a home-like setting. METHODS: Fifteen PD patients with mild symptoms and fourteen age- and sex-matched healthy controls (HC) performed 4-metre walking tasks in a living lab facility. Patients repeated the task

Severe atomoxetine overdose presenting with status epilepticus and wide-complex ventricular tachycardia.

Atomoxetine is a selective norepinephrine reuptake inhibitor commonly prescribed for attention-deficit/hyperactivity disorder. Most overdoses are mild to moderate; however, severe neurologic and cardiovascular toxicity can occur. We report the case of a 21-year-old woman who ingested approximately 9720 mg of atomoxetine, corresponding to 174 mg/kg. She presented with generalized convulsive status epilepticus, hyperthermia, and severe lactic metabolic acidosis. Arterial blood gas analysis immediately after intubation showed pH 6.94, PaCO₂ 37 mmHg, HCO₃- 7.9 mmol/L, base excess -24.3 mmol/L, and

When Treatment Becomes the Trigger: Fludrocortisone-Associated Hypokalaemia in an Older Adult.

Hypokalaemia is a common electrolyte abnormality in older adults and is frequently multifactorial in origin. Although often mild and asymptomatic, severe hypokalaemia can result in significant neuromuscular and cardiac complications. Medication-induced hypokalaemia remains an important and potentially reversible cause, particularly in frail elderly populations. We present the case of an 85-year-old woman who presented with generalised weakness, muscle cramps, reduced mobility, and a fall, and was found to have significant hypokalaemia in the context of long-term fludrocortisone therapy prescri

Hepatoerythropoietic porphyria in a 2-year-old child: Clinical features and effects of treatment.

Hepatoerythropoietic porphyria (HEP) is a rare autosomal recessive disorder of heme biosynthesis caused by severe deficiency of uroporphyrinogen decarboxylase (UROD). It typically presents in infancy or early childhood with marked photosensitivity, skin fragility, blistering, dyspigmentation, hypertrichosis, and dark urine. We report a girl from Brazil who developed blistering and ulcerative lesions of the face and upper extremities beginning at 10 months of age, along with dark urine and gray discoloration of the teeth. She was 2.5 years old when first evaluated at the Massachusetts General H

Pembrolizumab-Associated Polyserositis with Eosinophilic Pleural Effusion During Adjuvant Therapy for Clear Cell Renal Cell Carcinoma: A Case Report and Targeted Review.

Pembrolizumab is standard adjuvant therapy for high-risk clear cell renal cell carcinoma, but serositis is an uncommon immune-related adverse event that may mimic recurrence or infection. We report a 55-year-old man who achieved no evidence of disease after nephrectomy and metastasectomy and developed anasarca, large bilateral pleural effusions, mild ascites, peripheral eosinophilia, and a small pericardial effusion after six cycles of adjuvant pembrolizumab. Pleural fluid was exudative and contained 20% eosinophils. Cytology showed inflammatory cells without evidence of malignancy; bacterial,

Early Pubertal Progression and Increased 11-Oxygenated Androgens in a Girl with Wiedemann-Steiner Syndrome and a KMT2A Variant: A Case Report.

INTRODUCTION: The factors that determine the tempo of pubertal progression are not fully understood. Although a few patients with KMT2A variants were noted to have precocious puberty, in addition to various clinical features of Wiedemann-Steiner syndrome (WSS), the hormonal data of these individuals were unreported. Recently, high blood levels of 11-ketotestosterone, a potent 11-oxygenated androgen, were observed in a girl with WSS. CASE PRESENTATION: We encountered a Japanese girl who experienced menarche at 9 years and 4 months of age. She showed pubertal levels of LH and estradiol and adva
