Isolated clival Langerhans cell histiocytosis in a pediatric patient: systematic review and illustrative case report.

BACKGROUND: Langerhans cell histiocytosis is a rare clonal myeloid disorder with heterogeneous clinical manifestations, predominantly affecting children. Osseous involvement is common; however, isolated localization to the clivus is exceptionally rare and poses significant diagnostic and therapeutic challenges due to its proximity to critical neurovascular structures. We present an illustrative pediatric case alongside a systematic review of the literature to better characterize this uncommon entity. METHOD: This study presents a rare case of a 3.5-year-old boy diagnosed with isolated clival

Moyamoya disease masquerading as CNS vasculitis: Diagnostic challenges and imaging insights.

BACKGROUND: Moyamoya disease (MMD) is a rare, progressive cerebrovascular disorder that can mimic central nervous system (CNS) vasculitis on imaging, particularly when advanced imaging techniques, such as vessel wall imaging, are used; thereby creating significant diagnostic uncertainty. CASE PRESENTATION: A 38-year-old woman presented with a one-week history of headache and binocular diplopia. Neuroimaging demonstrated a subacute right occipitoparietal infarct. Magnetic resonance angiography showed multifocal stenoses with extensive collateral vessels. High-resolution vessel wall imaging dem

Acute acquired comitant esotropia in the smartphone era: a narrative review.

PURPOSE: Acute acquired comitant esotropia (AACE) has received increasing attention in the smartphone era, following numerous clinical reports suggesting an association with intensive and prolonged device use. However, it remains unclear whether smartphone use is a direct cause of AACE, a trigger in susceptible individuals, or a surrogate marker for broader modern lifestyle changes that may contribute to its pathogenesis. STUDY DESIGN: Focused narrative review. METHODS: This article presents a focused narrative review of the literature on AACE in relation to intensive smartphone or other han

Optimal Timing for Surgical Intervention in Orbital Fracture Management: A Systematic Review and Meta-Analysis.

BACKGROUND: The optimal timing for surgical repair of orbital floor and combined orbital wall fractures remains a subject of clinical debate. While early surgery may prevent fibrosis, muscle entrapment, and enophthalmos, delayed intervention after edema resolution can sometimes yield comparable outcomes. This systematic review and meta-analysis aimed to evaluate the influence of surgical timing on functional and esthetic outcomes following orbital fracture repair. METHODS: The present systematic review followed PRISMA 2020 guidelines and was prospectively registered on PROSPERO (CRD4202450279

R2Tg retrotransposon mediated targeted integration of exogenous genes into the 28S rDNA locus in mammalian cell lines for PKU gene therapy.

Phenylketonuria (PKU) remains a challenging genetic disorder due to the absence of therapeutic strategies capable of achieving sustained restoration of phenylalanine hydroxylase (PAH) function. Here, we employed an R2 retrotransposon in Taeniopygia guttata (R2Tg)-based system to enable targeted integration of exogenous PAH mRNA into the 28S ribosomal DNA (rDNA) locus of the host genome, with the goal of rescuing PAH deficiency caused by loss-of-function mutations. Experimental results demonstrated that lipid nanoparticle (LNP) delivery of R2Tg facilitated efficient site-specific integration of

Expression of miR-802 in glaucoma and its diagnostic significance.

INTRODUCTION AND OBJECTIVE: Glaucoma ranks as the second leading cause of blindness globally. The key factors leading to glaucoma are increased intraocular pressure and retinal ganglion cell apoptosis. Glaucoma is an irreversible disease that causes blindness in the eye, characterized by progressive optic nerve damage and visual field defects. The expression of miR-802 is abnormal in the plasma of glaucoma patients, but its function and mechanism remain unclear. The aim of the study is to investigate the expression of miR-802 in glaucoma and retinal ganglion cells, and the effect of miR-802 on

[The first case of Miller Fisher syndrome associated with anti-GM3 IgG antibodies: Classic triad and an atypical immune response].

Miller-Fisher syndrome (MFS) is a rare variant of Guillain-Barré Syndrome (GBS), an acute immune-mediated peripheral neuropathy most preceded by infections, particularly Campylobacter jejuni, through a mechanism of molecular mimicry that underlies its predominantly autoimmune pathogenesis. Anti-ganglioside antibodies play a significant role in the pathogenesis of MFS, with a particularly high prevalence of anti-GQ1b antibodies among affected individuals. However, the presence of alternative anti-ganglioside antibodies does not exclude the diagnosis. We report the case of a 72-year-old male who

Bone-Preserving Management of Maxillary Cemento-Ossifying Fibroma With Orbital Floor Thinning: A Case Report.

Cemento-ossifying fibroma (COF) is a benign fibro-osseous neoplasm that most often affects the mandible and occurs predominantly in women during the third and fourth decades of life. Maxillary involvement is less common and may remain minimally symptomatic while expanding into the maxillary sinus. We present a 26-year-old woman with a six-month history of progressive right midface enlargement without pain, nasal obstruction, diplopia, visual symptoms, or dental mobility. Computed tomography demonstrated a well-circumscribed expansile right maxillary lesion measuring 44.9 × 42.5 × 31.9 mm, occu

A Rare Dual Complication of First-Line Anti-tubercular Therapy: A Case Report of Drug Reaction With Eosinophilia and Systemic Symptoms (DRESS) Syndrome and Ethambutol-Induced Optic Neuropathy.

Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome is an extremely rare and serious systemic drug reaction, often posing significant diagnostic challenge to clinicians. Ethambutol-induced optic neuropathy (EON) is a well-recognised but often insidious complication of first-line anti-tubercular therapy (ATT), typically manifesting as reduced visual acuity, and visual field defects. We report the case of a 78-year-old woman with confirmed diagnosis of pulmonary tuberculosis, who simultaneously developed probable DRESS syndrome, secondary to rifampicin and EON shortly after in

Surgical outcomes of intermittent and constant exotropia in an adult population between 18 and 60 years old.

BACKGROUND: Comitant exotropia in adults poses distinct surgical challenges and has been less extensively studied compared with paediatric cases. Evaluating surgical outcomes in this population can inform treatment approaches and help establish realistic expectations for patients and clinicians. METHODS: Retrospective chart review of adults 18-60 years old who underwent surgery for comitant exotropia between 2014 and 2024 at a single institution. Patients were classified as intermittent (X(T)) or constant exotropia (XT) and divided into three groups: basic exotropia (BXT), divergence excess (

DOTATATE Scan in Meningiomatosis in a Patient With Ophthalmoplegia.

DOTATATE (1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid [DOTA]-octreotate) is a compound that binds somatostatin receptors seen in tumors such as meningiomas. Here, we present a case of a patient with progressive bilateral ophthalmoplegia due to meningiomatosis involving both cavernous sinuses, which was highlighted by DOTATATE imaging. Some studies have shown DOTATATE imaging is superior to contrast-enhanced magnetic resonance imaging (MRI) in identifying extent of intraosseous meningioma growth, which has been associated with worse prognosis. DOTATATE imaging should be considered

Clinical Validity of the 15-Degree Hess Area Ratio as a Postoperative Functional Outcome Measure in Orbital Blowout Fracture.

Orbital blowout fractures (BOFs) often result in diplopia and impaired ocular motility due to the entrapment of orbital contents. The Hess screen test is a widely used method for evaluating ocular motility. However, performing a full 30 degrees Hess chart is not always feasible in clinical practice. This retrospective preliminary study aimed to evaluate the clinical validity of the 15-degree Hess Area Ratio (HAR%) as an objective indicator of functional recovery after surgical repair of BOF. Eight patients with pure BOFs who underwent surgical reconstruction and both preoperative and at least
