Case Report: transcavernous sinus resection of parotid carcinoma with perineural invasion to Meckel's cave and cavernous sinus.

BACKGROUND: Adenoid cystic carcinoma (ACC) of the salivary glands is a rare but aggressive malignancy known for perineural invasion (PNI), allowing tumor spread along cranial nerves to the skull base, Meckel's cave, and cavernous sinus. Cavernous sinus involvement has traditionally been considered inoperable due to the density of neurovascular structures. Recent advancements in skull base microsurgery and anatomical landmark-guided navigation have enabled radical tumor resection in carefully selected patients. OBJECTIVE: To describe a tailored pretemporal extradural transcavernous skull base

Modified suture adjustments improve the precision of surgical outcomes in acute acquired comitant esotropia.

PURPOSE: To investigate whether modified postoperative suture adjustments can reduce postoperative deviation in patients with acute acquired comitant esotropia (AACE) who have significant postoperative phoria or tropia within 1 week of surgery. METHODS: The medical records of patients with AACE whose deviation was ≥ 10Δ assessed with the single Maddox rod test (SMRT) in the early postoperative period were analyzed retrospectively. A modified adjustable suture was placed intraoperatively in all patients. Patients who underwent subsequent suture adjustments within 1 week of surgery comprised th

Strabismus Surgery in Thyroid Eye Disease.

Thyroid eye disease (TED) produces chronic orbital inflammation that may result in restrictive extraocular myopathy and associated diplopia. TED-related strabismus can be challenging to address due to complex and often large-angle deviations, variable surgical results, high rates of reoperation, and profound impact on patients' function and quality of life. We address the evaluation and management of strabismus in TED, and discuss changes that the recent introduction of teprotumumab may have on timing and treatment options previously chosen.

Rapidly progressive visual impairment and internal carotid artery stenosis caused by sphenoid sinus mucocele: illustrative case.

BACKGROUND: Sphenoid sinus mucocele is a rare type of paranasal sinus lesions that can occasionally cause neurological deficits such as optic and oculomotor nerve deficits. However, internal carotid artery (ICA) stenosis associated with sphenoid sinus mucocele is extremely uncommon. OBSERVATIONS: A 36-year-old man with a history of craniotomy for Rathke's cleft cyst in childhood presented with rapidly progressive right-sided retro-orbital pain, ptosis, and diplopia. CT and MRI revealed a cystic lesion occupying the sphenoid sinus with adjacent ICA stenosis that markedly worsened within 4 days

Evaluation of acute visual changes in the pediatric emergency department: Etiologies and red flags.

INTRODUCTION: Acute visual changes-including blurred vision, diplopia, photopsia, visual hallucinations, and vision loss-pose a diagnostic challenge in children due to limited symptom description and limited cooperation during examination. We aimed to (1) characterize the etiologies of children presenting to a pediatric emergency department (PED) with acute visual changes and (2) identify clinical predictors of pathologic neuroimaging findings. METHODS: We conducted a retrospective, single-center study at a tertiary PED between 2022 and 2024. Patients aged 1 month to 18 years were identified

Case Report: Bilateral perineuroma of the orbit presenting as vertical binocular diplopia.

In this case report, we describe a 65-year-old woman who presented to the ophthalmology clinic with vertical binocular diplopia for approximately 2 years. Visual exam was notable for bilateral proptosis and ophthalmoplegia. A computed tomography (CT) of the head revealed bilateral enhancing orbital masses. A biopsy was performed, and the result was positive for epithelial membrane antigen (EMA) and CD34, confirming a diagnosis of perineuroma. To our knowledge, this is the first reported case of bilateral perineuroma of the orbit, a rare location for an already uncommon benign tumor.

Microsurgical Resection of a Cavernous Malformation of the Trochlear Nerve: A Case Report.

BACKGROUND: Cavernous malformations (CMs) involving cranial nerves are exceedingly rare. To date, only six cases of trochlear nerve involvement have been published in the literature. We present the case of a microsurgical resection of a CM affecting the trochlear nerve. METHODS: We report the clinical case of a 32-year-old patient who developed diplopia due to a CM affecting the trochlear nerve. Magnetic resonance imaging (MRI) showed a contrast lesion measuring 9 × 5 mm affecting the trochlear nerve. Microsurgical resection of the lesion via a subtemporal approach under continuous neuromonit

A modified orbital index for CT-based risk stratification in pediatric and adolescent orbital floor fractures.

Pediatric orbital floor fractures differ biomechanically from adult injuries owing to greater bone elasticity and incomplete sinus pneumatization. Surgical indications remain debated, as subtle radiologic findings may underestimate clinically significant muscle entrapment. The orbital index (OI), a quantitative CT-based metric developed in adult populations, has not been systematically evaluated in pediatric and adolescent patients. We evaluated the applicability of OI in pediatric orbital floor fractures and proposed a modified OI (mOI) incorporating fracture type to improve risk stratificati

Brainstem Encephalitis Following COVID-19 Vaccination: A Case Report.

Encephalitis is an extremely rare neurological adverse event following vaccination against coronavirus disease 2019 (COVID-19). We report a case of brainstem encephalitis following mRNA COVID-19 vaccination. A 30-year-old Japanese female presented with acute diplopia characterized by recurrent, paroxysmal episodes following the second dose of the vaccine, which resolved spontaneously. Brain magnetic resonance imaging revealed inflammatory changes in the brainstem. The patient responded rapidly to steroid therapy and subsequently improved. This case shares key clinical features with another cas

Superior oblique myokymia: diagnostic criteria and long-term outcome.

BACKGROUND: Superior oblique myokymia (SOM) is a rare disorder characterized by brief episodes of high-frequency, low-amplitude monocular nystagmus frequently caused by vascular contact with the trochlear nerve. Comprehensive analyses of SOM symptomatology and long-term progression are limited. No diagnostic criteria are available. METHODS: Adult patients who were diagnosed with SOM between 2008 and 2024 were asked to complete a detailed questionnaire on disease symptomatology, time course, and medication. The impact of the disease on patient-reported functioning was assessed using a Likert s

Diplopia and bilateral optic disc swelling as the initial presentation of B-lymphoblastic lymphoma: a case report.

B-lymphoblastic lymphoma (B-LBL) is an uncommon and aggressive hematologic malignancy that rarely presents with primary ocular manifestations. Initial presentation with ocular or neuro-ophthalmic symptoms is particularly unusual and may mimic more common intracranial tumors. We report a 21-year-old man with a one-year history of intermittent headaches, initially managed as migraine. He developed diplopia over one month and was referred to ophthalmology, where examination revealed left-sided proptosis and bilateral optic disc swelling with hemorrhage. Neuroimaging demonstrated a large invasive

The SF3b complex in cancer: structural basis, molecular mechanisms, and therapeutic opportunities.

BACKGROUND: Acetylcholine receptor antibodies (AChR-Ab) are established biomarkers in myasthenia gravis (MG). In ocular MG (OMG), their presence has been linked to disease severity, treatment response, and progression to generalized MG (GMG); however, the relevance of AChR-Ab titers or seropositivity to ocular manifestations remains uncertain. OBJECTIVE AND METHOD: We retrospectively reviewed OMG patients treated at Rajavithi Hospital (2009-2024). AChR-Ab titers were measured using enzyme-linked immunosorbent assay (ELISA). Clinical and demographic factors associated with AChR-Ab seropositivi

Endoscopic ipsilateral mononostril transethmoidal removal of an orbital apex cavernous hemangioma: illustrative case.

BACKGROUND: Orbital apex cavernous hemangiomas are rare, benign vascular malformations that pose significant surgical challenges due to their proximity to critical neurovascular structures. While traditional approaches such as orbitotomy or craniotomy remain standard, they are associated with higher morbidity. Endoscopic endonasal techniques have emerged as minimally invasive alternatives, particularly for medially located lesions. OBSERVATIONS: A 60-year-old woman presented with progressive retro-orbital pressure, binocular diplopia, and a mild left abduction deficit. MRI revealed a 10 × 8-m

[Invasive aspergillosis of the sphenoid sinus as a differential diagnosis in lesions of the sellar region].

We present a case of a 60-year-old woman with a history of poorly controlled type 2 diabetes who came to the emergency room with severe headache, nausea, and vomiting that had been ongoing for one week. Her vision was progressing with decreased visual acuity and diplopia. A MRI of the brain and sellar region was ordered, which revealed a lesion occupying the sphenoid sinus. Biochemical parameters included leukocytosis, hyperglycemia, central hypothyroidism and hypogonadism. Considering these findings, an interdisciplinary approach was undertaken to initiate a diagnostic and therapeutic algorit

The Endoscopic ExtraForaminal TransChoroid (EFTC) Approach in the Management of Pineal Tumors: Gross Total Resection through a Single Burr Hole.

Pineal region tumors pose a significant surgical challenge due to their deep location and proximity to critical neurovascular structures. Endoscopic techniques, particularly the transchoroid approach, have expanded the possibilities for minimally invasive access to the posterior third ventricle. In select cases, this route may not only allow simultaneous biopsy and third ventriculostomy but also meaningful tumour resection while limiting morbidity. We report the case of an 11-year-old male presenting with headache, drowsiness, vomiting and diplopia. Imaging demonstrated a heterogeneous pineal
