OBJECTIVE: Preserving a perforating artery incorporated into a basilar tip aneurysm is challenging, particularly in partially thrombosed intracranial aneurysms (PTIAs), and treatment is frequently limited to partial embolization alone.
CASE PRESENTATION: A 53-year-old woman presented with diplopia caused by an 18-mm partially thrombosed basilar tip aneurysm. An artery of Percheron appeared on angiography to arise from the aneurysm wall, and maximum-intensity projection images of DSA confirmed that this single trunk supplied both paramedian thalami. Partial coil embolization was performed to p

Silent somatotroph adenomas are uncommon pituitary neuroendocrine tumours (PitNETs) of the PIT1 lineage that express growth hormone (GH) immunohistochemically but without biochemical evidence of acromegaly. We report the case of a 32-year-old male patient referred with recurrent epistaxis, headache, right-sided diplopia, and third cranial nerve palsy. Clinical examination revealed acromegaloid features including frontal bossing, spade-like hands, and coarse facies, which were strongly suggestive of longstanding GH excess. However, preoperative insulin-like growth factor-1 (IGF-1) was paradoxic

BACKGROUND: Trochlear nerve schwannomas (TNSs) are exceptionally rare benign tumors, accounting for a very small fraction of intracranial schwannomas. Due to their deep locations and slow growth, they often reach a considerable size before diagnosis. Giant or multiloculated cystic variants are exceptionally uncommon and present significant diagnostic and surgical challenges.
METHODS: A 32-year-old female presented with headache and dizziness but no diplopia. Magnetic resonance imaging revealed a giant multiloculated cystic lesion extending from the posterior fossa to the right temporal lobe,

Giant cell arteritis (GCA) is a systemic granulomatous vasculitis of medium and large-sized arteries, which represents an ophthalmic emergency because of the risk of sudden, severe and often irreversible loss of vision. Its most important ocular manifestation is arteritic anterior ischemic optic neuropathy; less frequent manifestations include central retinal artery occlusion, cilioretinal artery occlusion, posterior ischemic optic neuropathy, choroidal ischemia and ocular motor nerve palsies. Giant cell arteritis should be actively considered in every patient over 50 years of age presenting

Mycobacterium avium complex (MAC) lung disease requires prolonged multidrug therapy. We report the case of an 80-year-old woman with MAC lung disease who developed an isolated right abducens nerve palsy after approximately nine months of treatment with moxifloxacin, azithromycin, and ethambutol. The patient presented with horizontal diplopia, and ophthalmological evaluation confirmed sixth cranial nerve palsy. Although ethambutol-induced optic neuropathy was initially considered, clinical findings were not consistent with this diagnosis. Given the temporal relationship with moxifloxacin exposu

Quadrigeminal cistern arachnoid cysts are rare intracranial lesions. While most intracranial arachnoid cysts in adults are incidentally detected and asymptomatic, quadrigeminal cistern arachnoid cysts, located adjacent to the dorsal midbrain and cerebral aqueduct, may become symptomatic, most often with obstructive hydrocephalus. Trochlear nerve palsy secondary to quadrigeminal cistern arachnoid cysts has been reported only rarely, and the pathophysiology and optimal management remain unclear. We report a 60-yr-old man who presented with progressive diplopia due to left trochlear nerve palsy.

WEBINO syndrome (wall-eyed bilateral internuclear ophthalmoplegia) is a rare neuro-ophthalmological disorder characterized by bilateral adduction deficit, abducting nystagmus, large-angle exotropia in the primary gaze position, and impaired convergence, secondary to bilateral lesion of the medial longitudinal fasciculus. We report the case of a 35-year-old woman with multiple sclerosis who consulted for persistent horizontal binocular diplopia. Examination revealed alternating exotropia of 50 prism diopters with bilateral adduction limitation, abducting nystagmus, and absence of convergence, c

PURPOSE: To describe a case of rheumatoid arthritis (RA)-associated orbital and intracranial inflammatory disorder (OIID).
BACKGROUND: RA is a systemic autoimmune disease associated with a multitude of extra-articular manifestations. Cigarette smoking increases the risk of RA vasculitis and adversely influences the expression of the proto-oncogene p53 in RA. Although ocular manifestations of RA are well known, orbital and intracranial inflammatory presentations are much less common.
CASE PRESENTATION: A 63-year-old woman with seropositive RA and a significant tobacco smoking history presente

Myasthenia gravis is an autoimmune neuromuscular disorder characterized by fluctuating skeletal muscle weakness and fatigability. Perioperative management is challenging because patients may have bulbar or respiratory muscle involvement and altered sensitivity to neuromuscular blocking agents, increasing the risk of postoperative respiratory failure and myasthenic crisis. Thymectomy is an established therapeutic option in selected patients with myasthenia gravis and may be performed using minimally invasive thoracoscopic techniques. We report the anesthetic management of a 48-year-old man with

BACKGROUND: Orbital decompression is an effective surgery for reducing orbital congestion and proptosis for patients with thyroid eye disease (TED). However, it is commonly stated that elective orbital decompression, i.e., for reasons other than compressive optic neuropathy, should await the quiescent phase of the disease. The reason is to avoid inducing a flare-up of the orbitopathy secondary to surgical inflammation. Still, this means that patients with moderate to severe TED who are suffering from symptoms associated with TED must wait months to years before initiating surgical treatment. T

INTRODUCTION: Isolated orbital floor fractures in juveniles are a challenge, as children and adolescents may experience diplopia and limited ocular motility, even with little to no periorbital edema or visible signs of injury to the eye. The aim of the present study is to assess the demographic and clinical variables, and the outcomes of pediatric patients affected by isolated orbital wall fractures at several European oral and maxillofacial surgery centers.
METHODS: Pediatric patients (less than 18 years) with hospitalized and surgically treated for unilateral isolated orbital blow out fract

BACKGROUND: Secondary bacterial infections complicating allergic fungal rhinosinusitis (AFRS) are uncommon but can be serious given the expansile nature of the disease which can thin and erode the bone separating the sinuses from the brain and orbits. Zoonotic pathogens are rarely implicated as pathogens.
CASE REPORT: A 21-year-old African American male with several years of progressive nasal obstruction, facial pressure, and hyposmia, presented emergently with acute left eye pain and swelling, diplopia, afferent pupillary defect and inferior visual field deficit. Computed tomography and magn

BACKGROUND: The management of locally advanced periorbital skin tumors with orbital invasion is challenging. Orbital exenteration historically provides reliable local control but causes major functional and cosmetic morbidity. We evaluated stereotactic body radiation therapy (SBRT) as an eye-sparing alternative, focusing on local control, vision preservation, and toxicity.
METHODS: This retrospective series includes 26 consecutive patients with locally advanced periorbital tumors treated prospectively with SBRT. Tumors involved the canthi (zones III-IV) in 20 patients, with multi-zone involve

BACKGROUND AND PURPOSE: Middle meningeal artery embolization (MMAE) with liquid embolic agents (LEAs) is increasingly used to manage chronic subdural hematoma (CSDH). Conventional microcatheters require distal wedge positioning of the microcatheter tip or proximal plug formation to prevent reflux and achieve effective distal penetration. Dual-lumen balloon microcatheters limit reflux and enable controlled LEA injection, which may improve procedural efficiency. The evidence base for dual-lumen balloons in CSDH is limited.
MATERIALS AND METHODS: This single-center retrospective study included c

Background Skull base chondrosarcomas are rare and locally aggressive tumors with heterogeneous biology accounting for 0.1-0.2% of all intracranial tumors. Dedifferentiated chondrosarcoma is an exceptionally uncommon and aggressive variant, seldom seen at the skull base. We report a recurrent clival chondrosarcoma with a small, dedifferentiated component, and an unusually long latency after initial treatment. Case Report A 66-year-old woman was referred with a recurrent progressive clival lesion, initially suspected to be a chordoma. She had undergone subtotal resection followed by single-frac

BackgroundA petrous apex mucocele is a rare benign cystic lesion that can present with symptoms due to pressure on surrounding structures, including the internal carotid artery, Meckel's cave and the fifth and sixth cranial nerves, as well as the cavernous sinus region and inner ear.MethodsA 36-year-old male presented with 8 months of diplopia when looking to the right, a right-sided temporoparietal headache, and paresthesia in the right half of his face. An MRI revealed a 26 × 32 × 37 mm lesion in the right petrous apex. The lesion's signal characteristics were most consistent with a mucocele

A 51-year-old man presented with progressive, subacute diplopia and ophthalmoplegia. Examination by the hospitalist demonstrated right ophthalmoplegia on left lateral gaze. No neurologist or neuro-ophthalmologist was available. Imaging and laboratory work were largely unremarkable. Although a microvascular etiology was suspected, the unremarkable workup expanded the differential diagnosis, highlighting the diagnostic and management challenges of cranial nerve III palsy when imaging is unremarkable in a community hospital with limited resources.

We report the case of a 12-year-old competitive athlete who presented with persistent ocular motility restriction and diplopia following surgical repair of an orbital floor trapdoor fracture. Although the initial surgery was performed at another institution on the day following the injury and postoperative CT confirmed the successful release of tissue entrapment, her clinical symptoms persisted (elevation restriction grade -3). Revision orbital surgery was performed 53 days after the initial injury. Pre-incision forced duction testing (FDT) confirmed mechanical tethering. Under microscopic gui

Sudden-onset diplopia warrants an emergency management in ophthalmology. A binocular, vertical and persistent diplopia, should raise suspicion of a central lesion, particularly within the brainstem. We reporte the case of a 53-year-old man, who presented with acute vertical binocular diplopia. Ophthalmological examination revealed a bilateral elevation limitation, characteristic of Parinaud's syndrome. Brain magnetic resonance imaging identified a right-sided ischemic midbrain infarction, and further cardiovascular work-up demonstrated a patent foramen ovale causing a right-to-left shunt.
