Intrasellar meningioma versus pituitary apoplexy in the setting of postpartum hemorrhage: a case report.

BACKGROUND: Headache in the postpartum period can indicate serious underlying pathology, such as a meningioma or a pituitary apoplexy event. These conditions can present similarly but are managed differently, indicating the need for careful diagnostic consideration. CASE PRESENTATION: A 39-year-old G7P2042 female delivered a healthy male neonate by urgent repeat cesarean delivery secondary to premature rupture of membranes at 35 weeks 1 day gestation. She sustained blood loss of approximately 1000 mL during delivery. One week postpartum, she developed new-onset persistent non-positional headac

Clinical efficacy, safety, and success factors of botulinum toxin type A in the treatment of acute acquired comitant esotropia.

PURPOSE: Acute acquired comitant esotropia (AACE) is an increasingly reported form of esotropia, possibly linked to the rising use of digital devices. Botulinum toxin type A (BTXA) has emerged as a minimally invasive treatment alternative to surgery, particularly for patients with diplopia. We aimed to investigate the efficacy and safety of BTXA injections in treating AACE while identifying factors contributing to successful treatment outcomes. STUDY DESIGN: A retrospective, single-center study. METHODS: This study retrospectively evaluated 228 patients with AACE who received BTXA injections

When a Withdrawal Aid Mimics a Brainstem Lesion: Gabapentin-associated Transient Ophthalmoplegia.

OBJECTIVES: Gabapentin is frequently used as a benzodiazepine-sparing agent for the management of mild to moderate alcohol withdrawal and is generally considered well-tolerated. Although neurological adverse effects have been reported, objective ocular motor abnormalities associated with gabapentin use during alcohol withdrawal are rarely described. Recognition of such effects is important to guide evaluation and management. METHODS: We report a 60-year-old man admitted to a residential treatment program for alcohol use disorder who developed acute diplopia and transient ophthalmoplegia short

Analysis of Saccade Characteristics During Fusional Vergence Tests in Normal Binocular Vision Participants.

The purpose of the study was to analyze, characterize, and compare the measurements of saccades that occurred during the positive and negative fusional vergence test (PFV and NFV, respectively) as a function of the disparity vergence demand. Thirty-four participants' PFV and NFV amplitudes were measured in a haploscopic setup, recording eye movements with an Eyelink 1000 Plus (SR Research). The visual stimulus was a column of letters. Break and recovery points were determined objectively offline, and saccades were detected with a velocity-threshold-based method. A total of 13,103 and 14,381 sa

Exploratory whole-exome sequencing identifies candidate DNA variants in Ocular Behcet disease: a pilot study from a Pakistani cohort.

PURPOSE: Ocular Behcet disease (OBD) is a severe sight-threatening complication of Behcet disease (BD) with a poorly understood etiology, particularly in underrepresented populations such as Pakistan. It often arises from a combination of genetic predisposition, environmental triggers, and immune dysregulation. To address this gap, we explored genetic variants contributing to ocular involvement using whole-exome sequencing (WES). METHODS: Eleven clinically diagnosed BD patients were recruited, including ten with ocular symptoms such as uveitis, diplopia, and optic nerve involvement. WES was p

Extensive Cerebral Venous Sinus Thrombosis in a Young Male With Severe Hyperhomocysteinaemia: An Unusual Presentation With Early Diplopia.

Cerebral venous sinus thrombosis (CVST) is a rare cause of stroke, particularly in young adults, and often presents with nonspecific symptoms that may delay diagnosis. Hyperhomocysteinaemia is a recognized but frequently overlooked prothrombotic risk factor. We report a case of a 25-year-old previously healthy male who presented with sudden-onset severe unilateral headache and projectile vomiting. Neuroimaging with magnetic resonance imaging and venography revealed extensive thrombosis involving the superior sagittal, left transverse, and left sigmoid sinuses with cortical veins involvement. The eye as a diagnostic key: the ophthalmologist's role in an atypical case of Miller Fisher Syndrome.

Miller Fisher syndrome (MFS) is an uncommon variant of Guillain-Barré syndrome characterized by the triad of ophthalmoplegia, ataxia, and areflexia. Diagnosis can be particularly challenging when the clinical presentation is incomplete or atypical, especially in early stages. We report the case of a 54-year-old woman who presented to the emergency department with blurred vision and nonspecific dizziness, without diplopia or other relevant neurological findings. Hospital discharge was initially considered. However, ophthalmologic examination revealed severe bilateral ophthalmoplegia with supran
