Supranuclear disorders of eye movement.

Eye movements are the result of activity in discrete systems. Saccades and smooth pursuit movements respond to target position and target velocity respectively. Compensatory responses to movements that alter the position of the eyes in space depend upon the vestibular and optokinetic systems. Vergence movements allow fusion and binocular vision. Eye movement abnormalities of supranuclear origin may be divided into nystagmus, other ocular oscillations, saccadic dysfunction, smooth pursuit dysfunction, gaze palsy, tonic deviation, and vergence disability. An algorithm for the analysis of supranu

Criteria for decompensation in binocular vision.

Two groups of patients, one under 40 years of age and the other 40 years and over, were divided into subgroups according to whether they had symptoms associated with near vision. A method developed in 'signal detection theory' was used to determine whether a value could be found for heterophoria, or associated heterophoria, which would predict which patients had symptoms due to decompensated heterophoria in normal routine investigation. Receiver operator characteristic curves (ROC curves) were constructed to look for these cut-off values between patients with, and without, symptoms. No value f

Visual evoked potentials in Prader-Willi syndrome.

Oculocutaneous, electrophysiological, and cytogenetic factors were evaluated in 14 patients with Prader-Willi syndrome and in three controls, two albinos and a normal observer. In a substantial number of PW patients chromosomal anomalies, particularly deletions of the long arm of chromosome 15 as well as hypopigmentation of hair, skin, and eye have been identified. In the genetic condition of albinism, hypopigmentation related to neural ectoderm derivatives is associated with reduced visual acuity, foveal hypoplasia, and aberrant retinogeniculocortical projections. The latter can be observed b

Impaired vision and hip fracture. The Framingham Study.

Falls affect a large proportion of the elderly and can result in a variety of injuries, including hip fractures. Several studies have suggested that visual impairment contributes to falls, but studies have not used standardized definitions of visual impairment and have not examined injurious falls or fractures. We looked at the risk of hip fracture associated with visual impairment in those members of the Framingham Study Cohort who took part in the Framingham Eye Study in 1973-75. Of 2,633 subjects followed for 10 years after the eye exam, 110 sustained hip fractures. The fracture rates in th

[Activation-induced changes in visual functions].

Visual functions were tested in 18 healthy male medical students on a control day and on the day of an oral examination. Stress induced impairment was observed in binocular tests measuring accommodation, convergence or their mutual relationship (near points of accommodation and convergence, Maddox-wing). Results of tests in which fusion operates as stabilizing factor (slightly dissociating test: Polatest) or of tests designed for far distance vision (synoptophore, Maddox-cross, Pola-cross) did not show significant impairment under examination stress. In slightly dissociating tests an activity-

Characteristics of convergence insufficiency.

Convergence insufficiency (CI) is a well-known syndrome of binocular visual dysfunction. In a review of 58 papers, considerable variation was noted in the criteria used to define the condition. Symptoms and decreased positive fusional vergences (both at the nearpoint) were the only criteria named in more than one-half of the studies. An extended nearpoint of convergence (NPC) and increased exophoria at near were criteria in about one-third of the papers. Examination of data in the reviewed papers shows that although considerable variability was noted, the distance exodeviation, distance negati

Optic neuritis in the elderly: prognosis for visual recovery and long-term follow-up.

We evaluated 14 patients with idiopathic optic neuritis, 50 years of age or older, within 1 month of symptomatic onset from 1980 through 1985. We obtained follow-up information from case records an average of 13 months after initial examination and from telephone interviews an average of 57 months after initial evaluation. Four patients were men and ten were women. Visual loss was monocular in ten cases and binocular in four. Only one of 18 affected eyes initially had an acuity of 20/30 or better. Eleven eyes recovered acuities of 20/30 or better. There was no apparent benefit to those patient

Fusion ability lost and regained in visual adults.

The case reports are given of two adult patients who lost all fusion ability and had the signs and symptoms of central fusion disruption and intractable diplopia. Both patients suffered binocular sensory deprivation for several years caused by a combination of a traumatic cataract and uncorrected unilateral aphakia. One patient had a posterior chamber intraocular lens inserted to correct the aphakia and the other patient had a contact lens. Aniseikonia was not appreciated by either patient and did not appear to contribute to the fusion problem. Adjustable strabismus surgery, in both patients,

The effect on fixation disparity and associated heterophoria of reading at an abnormally close distance.

It is well established that prism stress on binocular vision produces fixation disparity. It is therefore suggested that fixation disparity is a sign of stress on binocular vision. The authors have previously looked at the stress caused by reading in reduced levels of illumination, and found that the average fixation disparity and associated heterophoria become more exophoric. In this paper the effect on fixation disparity and associated heterophoria of reading at an abnormally close distance is reported. It was found that in 50 young adults fixation disparity and associated heterophoria incre

Isolated environmental tilt associated with lateral medullary compression by dolichoectasia of the vertebral artery. Is there a cause and effect relationship?

An elderly woman with the persistent isolated symptom of monocular and binocular tilting of environmental objects, was found on magnetic resonance imaging to have compression of the lateral medulla by an ectatic vertebral artery. The double Maddox rod test showed no relative torsion of either globe although 5 degrees of right excyclotorsion and left incyclotorsion (rightward ocular counter-rolling) was detected on testing with monocular viewing. There was no accompanying hyperdeviation, nystagmus or other neurologic signs. Symmetrical tilting of each globe associated with head tilt and skew de

Squint surgery in patients with Graves' ophthalmopathy.

The results of extraocular muscle surgery in 22 patients with Graves' ophthalmopathy were evaluated. All patients were classified in NOSPECS class 4. The mean age at the time of squint surgery was 55(10)/12 years; there were 6 males and 16 females. Ten patients had primary extraocular muscle surgery. Twelve patients were first treated with prednisone alone or prednisone and retrobulbar irradiation therapy (1000 RAD) and orbital decompression because of other ocular symptoms. All patients were euthyreotic at the time of operation, and their motility was unchanged for at least 6 months. Altogeth

Visual field defects for vergence eye movements and for stereomotion perception.

An objective visual field can be mapped in terms of stimulus-induced eye movement. The authors used the scleral coil technique to record vergence and conjugate eye movements while stimulating different visual field locations with a 3 X 3 deg target whose image vergence was oscillated. For each of three subjects tested there was a visual field location where vergence eye movements were much weaker than in a control location of equal retinal eccentricity. On the other hand, conjugate eye movements driven from these two locations by lateral motion were similar. Field defects for ocular vergence c

[Does proprioception of the extrinsic eye muscles participate in equilibrium, vision and oculomotor action?].

Extrinsic eye muscles respond to both proprioceptive and visual impulses. Histology of ocular proprioceptive receptors shows them to be qualitatively and original, neuromuscular bundles are found in primates, and so called "palisade sensory nerve endings" in felines. Golgi's osteotendinous receptors are apparently absent. The number of muscle receptors is high and their proportion in relation to muscle weight even more so. The afferent pathway for this sensory system initially accompanies the motor fibers of oculomotor III, IV, ans VI nerves. Some fibers continue with these nerves to the brain

Inter-relationships between British drivers' visual abilities, age and road accident histories.

Statistical analysis is described of data on 1000 British drivers' accident histories and their performance on a vision screening instrument. Significant positive associations are evident between various accident variables and tests of hyperphoria, right eye visual acuity, left eye acuity and binocular acuity. The clustering of associations is strongest for older drivers. The significance of hyperphoria has received little attention previously. The age-related associations with accidents parallel the decline with age of other visual abilities. Statutory re-testing of drivers' vision at about t

Comparative monocular contributions to brightness in relation to abnormal stereothreshold.

This study was designed to compare brightness contributions of the two monocular inputs to the binocular visual system at three different levels of luminance adaptation in two groups of 15 subjects each. The only known difference between these groups was stereothreshold, 50 to 70 sec arc in one group vs. 40 sec arc or less in the other group. The group with elevated threshold was found to have a significantly larger average difference (13.5%, p less than 0.002) in monocular brightness contributions, between the right and left eyes, when compared to the group with lower stereothresholds (4% mis

[Value of the stereoscopic tests in the detection of sensorimotor anomalies in children].

Limitations and advantages of the stereotests are briefly discussed. In this trial, two stereotests (TNO and Frisby) have been tested in 3 groups of patients: group 1 was composed of 180 cases with normal binocular vision; group 2 of 312 patients with known ocular motility disorders: group 3 of 186 patients with unknown sensorimotor status. The utility of the two stereotests in detecting ocular motility disorders and/or amblyopia is uncertain when used alone. Because of a high number of false positives and false negatives the tests are best used in conjunction with other clinical tests of ocul

Traumatic third nerve palsy.

Twenty patients with a traumatic third nerve palsy had sustained a closed head injury with prolonged loss of consciousness in a high-speed deceleration accident. Sixteen were male, and the average age was 25 years. Seven had skull or facial fractures, 15 damage to the anterior visual pathways, and 16 other permanent neurological damage. Nineteen developed the misdirection/regeneration syndrome. Thirteen had strabismus surgery, and an area of binocular single vision was enlarged or achieved in three.

[Visual scanning in patients with unilateral visual neglect due to right-sided cerebro-vascular lesion].

The characteristics of visual scanning in patients with unilateral visual neglect (UVN) were studied. Forty-one patients who showed UVN in the figure finding test and 21 patients who showed to UVN in the test were selected from 176 patients with the right-sided cerebro-vascular lesion. They ranged in age from 46 to 78 years. The severity of UVN in each subject was determined by the number of the figures neglected unilaterally in the figure finding test in which a subject was asked to circle the designated figures scattered among the other figures on the test paper. The following two tests were

Analysis of tonic and accommodative vergence disorders of binocular vision.

Tonic and accommodative vergence disorders of binocular vision are analyzed with separate methods. The center of symmetry and the associated phoria of forced duction fixation disparity curves are two criteria used for prismatic correction of tonic vergence disorders. The associated phoria is corrected physiologically when patients who have tonic disorders are able to develop prism adaptation with orthoptic exercises. Graphical methods are used to analyze accommodative disorders resulting from mutual interactions between accommodation and convergence. Graphic analysis illustrates that patients

Clinical applications of preferential looking measures of visual acuity.

We have undertaken 3 lines of research aimed at the eventual transformation of the laboratory-based preferential looking (PL) acuity-testing procedures into clinically-useful techniques. (1) The first line of research involves studies of the parameters of PL testing, and the establishing of norms for various groups. The results show that acuity development is closely tied to gestational age, that infants' acuity is reduced at low luminances but does not vary significantly at levels above 1 log cd/m2, and that monocular acuity is poorer than previously-published binocular acuity norms.
