A Novel Heterozygous ARL3 Variant in Non-Syndromic Retinitis Pigmentosa: Clinical and Functional Characterization.

Retinitis pigmentosa (RP) comprises a heterogeneous group of inherited retinal dystrophies characterized by the progressive degeneration of photoreceptors, leading to night blindness and gradual loss of peripheral vision. RP is characterized by a substantial genetic heterogeneity, with more than 85 genes implicated across autosomal dominant, autosomal recessive, and X-linked inheritance patterns. Recent studies have identified mutations in the ARL3 gene as a causative factor in both syndromic and non-syndromic forms of RP, including autosomal dominant and recessive cases. ARL3 encodes a small

Bardet-Biedl syndrome in two sibling pairs: a case series.

BACKGROUND: Bardet-Biedl syndrome is a rare multisystem ciliopathy defined by retinal dystrophy, obesity, polydactyly, hypogonadism, and renal anomalies, often accompanied by neurodevelopmental and behavioral issues. Diagnosis relies heavily on clinical features, particularly in resource-limited settings where genetic testing is unavailable. CASE PRESENTATION: We describe two sibling pairs, all of South Asian, Pakistani ethnicity, clinically diagnosed with Bardet-Biedl syndrome using contemporary diagnostic frameworks. The first pair, two male brothers aged 17 and 21 years, came from a consan

Cryo-EM structure of TRPM1 reveals a non-canonical architecture with an inverted transmembrane domain.

Transient receptor potential melastatin 1 (TRPM1) is a membrane protein essential for vision in dim light, and mutations in TRPM1 cause complete congenital stationary night blindness. Although TRPM1 shares sequence similarity to other TRPM ion channels such as TRPM3, whether it independently functions as an ion channel remains controversial. This controversy is largely caused by TRPM1's challenging biochemical behaviors that prevent detailed molecular characterization. In this work, we isolate TRPM1 and determine its structures using cryogenic electron microscopy (cryo-EM). The structures reve

Functional Validation of a Novel Deep Intronic IMPG2 Variant Causing Pseudoexon Activation in Retinitis Pigmentosa with Macular Involvement.

OBJECTIVE: Retinitis pigmentosa (RP) is a genetically heterogeneous group of inherited retinal dystrophies often accompanied by macular involvement. Variants in IMPG2 are known to cause RP type 56 and vitelliform macular dystrophy type 5, but the pathogenic role of deep intronic variants has rarely been characterized. This study aimed to identify and functionally validate a novel deep intronic IMPG2 variant in a patient with RP. METHODS: A comprehensive clinical, genetic, and functional assessment was performed. Ophthalmic evaluations included fundus photography, optical coherence tomography

Generation of a human-induced pluripotent stem cell line from a retinitis pigmentosa patient carrying biallelic mutations in EYS gene.

Retinitis pigmentosa (RP) is a hereditary retinal disorder characterized by progressive degeneration of photoreceptor cells in the retina, leading to gradual vision loss. Symptoms of RP typically begin with night blindness and progressive loss of peripheral vision, eventually leading to severe tunnel vision and, in advanced stages, loss of central vision or complete blindness. To date, no effective therapeutic strategies have been developed to halt disease progression, mitigate vision loss, or achieve a complete cure, and currently available disease models have limitations in fully recapitulat

Domain-specific functions of LRIT3 in synaptic assembly and retinal signal transmission.

LRIT3 is a leucine-rich repeat (LRR) protein that is expressed in the retina, and its absence causes complete congenital stationary night blindness (cCSNB), a genetically diverse disorder characterized by impaired low-light vision, myopia, and nystagmus. LRIT3 is expressed in rod and cone photoreceptors, and it transsynaptically organizes the assembly of the glutamate signaling complex, the signalplex, on depolarizing bipolar cells (DBCs). LRIT3 is a single-pass membrane protein with extracellular LRR, IG, and FN3 domains. We express domain deletion constructs using rAAV and examine the impact

Electronegative electroretinography in inherited retinal disease: a systematic review of genotype-phenotype correlations.

BACKGROUND: The electronegative electroretinogram (ERG)-in which the dark-adapted bright-flash b-wave amplitude falls below the a-wave (b: a ratio < 1.0)-localizes dysfunction to the inner retina or photoreceptor-bipolar synapse. In inherited retinal disease (IRD), this waveform has been associated with a restricted set of genetic etiologies, but the evidence has not been systematically synthesized. OBJECTIVE: To systematically identify and appraise the evidence on genetically confirmed IRD associated with electronegative ERG, characterizing reported gene-specific electrophysiologic patterns,

Expanding the Genotypic Landscape of Congenital Stationary Night Blindness in an Ethnically Diverse Canadian Population.

Congenital stationary night blindness (CSNB) is a rare and typically nonprogressive group of genetically heterogeneous disorders resulting in impaired night vision and high myopia with varying levels of visual impairment. Despite being a rare disease with a prevalence of 1:294,000, variants in 22 genes have been associated with specific CSNB phenotypes. Approximately, 13% of cases remain without a genetic diagnosis, highlighting the importance of ongoing genetic studies. Clinical and diagnostic information was collected retrospectively from patients who were diagnosed with CSNB. Patients under

Light-Activated RPE65 Inhibitors Enable On-Demand Visual Cycle Control.

Light initiates visual perception, but it also exacerbates a subset of blinding diseases in which visual (retinoid) cycle metabolic intermediates contribute to pathophysiology. Small molecule visual cycle modulators (VCMs) have demonstrated efficacy in preclinical models, but have been limited clinically by chronic, indiscriminate visual cycle suppression leading to side effects including night blindness in human subjects. Here, we demonstrate VCMs that are activated via a Z→E photoisomerization of an azobenzene-containing VCM by visible light within the eye. One such VCM photoswitch, (Z)-9, i

Multimodal imaging and intravitreal faricimab for polypoidal choroidal vasculopathy associated with a choroidal nevus in genetically confirmed Usher syndrome type 2: a case report.

BACKGROUND: Usher syndrome is an autosomal recessive disorder characterized by retinitis pigmentosa (RP), sensorineural hearing loss, and vestibular dysfunction. Polypoidal choroidal vasculopathy (PCV) is characterized by branching neovascular networks and polypoidal lesions. While typically classified within the pachychoroid spectrum, PCV can also manifest in eyes without choroidal thickening. Conversely, RP is usually associated with choroidal thinning, which may lead to underdiagnosis of PCV. To the best of our knowledge, PCV has not previously been reported in patients with Usher syndrome.
