COQ8B gene deficiency as a potential cause of retinal abnormalities in Pediatric kidney transplant recipients.

Coenzyme Q10 synthesis disorder caused by COQ8B gene deficiency is among the most prevalent causes of end-stage renal disease (ESRD) in children, which usually presents as isolated kidney disease, with sporadic cases associated with extrarenal symptoms such as retinitis pigmentosa (RP). Through long-term follow-up of 26 renal transplant children with COQ8B variants at our center, it is observed that, despite favorable renal transplant outcomes, 23.1% of children experienced night blindness or other ocular symptoms. Nine children were recruited for systematic ophthalmic examination and found th

A Case of Melanoma-Associated Retinopathy Presenting with Ocular Symptoms as the Initial Manifestation: A Case Report.

INTRODUCTION: Melanoma-associated retinopathy (MAR) is a distinct subset within the spectrum of retinal-dysfunction disorders. It is triggered by the cross-reaction between circulating antibodies originating from malignant melanoma (MM) and retinal antigens. The timely identification of paraneoplastic retinal diseases is crucial, as it can significantly contribute to the early diagnosis and treatment of underlying malignancies. Currently, increasing awareness of the early clinical manifestations of MAR is essential for clinicians to detect primary or metastatic MMs at an earlier stage. CASE P

Clinical and Biochemical Characterization of Specific GUCY2D Alleles Associated With a Rare Form of Night Blindness.

PURPOSE: To clinically and biochemically characterize a rare autosomal recessive rod-cone dysfunction, with electroretinographic similarities to some forms of stationary night blindness (SNB), associated with biallelic variants in GUCY2D. METHODS: Six patients from five families with a history of longstanding night blindness, no fundus features suggestive of retinitis pigmentosa, and an unusual electroretinographic phenotype were ascertained. Clinical examination and genotyping were performed. Selected GUCY2D variants were tested for binding to and activation by guanylate cyclase-activating p

Review of Four Refined Clinical Entities in Hereditary Retinal Disorders from Japan.

In the past, only Oguchi disease was reported as a hereditary retinal disease from Japan. Dr. Chuuta Oguch was a Professor of Nagoya University in Japan. During the past 40 years, four new clinical entities in hereditary retinal disorders have been detected by the Miyake group from Nagoya, Japan. All disorders show essentially normal fundi, and the diagnosis was made mainly by the analysis of an electroretinogram (ERG). Gene mutations are detected in three of them. Bipolar cell (BP) dysfunction syndrome: Congenital stationary night blindness (CSNB) with negative ERG (a-wave is larger than b-wa

Genetic analysis of congenital stationary night blindness and Oguchi disease in an Indian cohort.

BACKGROUND: Congenital stationary night blindness (CSNB) is a group of genetically and clinically heterogeneous non-progressive retinal disorders and can be classified based on fundus abnormalities as found in Oguchi disease or fundus albipunctatus (FA) or based on the absence of severe fundus abnormalities but altered electroretinography (ERG) findings. Here, we report the clinical and genetic findings of 46 CSNB families, with 18 families showing fundus abnormalities and 28 families without fundus abnormalities but having an altered ERG, showing complete CSNB (cCSNB) and Riggs type CSNB. Germline Disruption of Retinal Pigment Epithelium-Expressed Zebrafish rlbp1b-/- Results in Selective Dim Light Visual Behavior Deficits and Provides a Screening Platform for Evaluating the Pathogenicity of Human RLBP1 Variants.

Cellular retinaldehyde binding protein (CRALBP) plays a crucial role in the visual cycle by chaperoning 11-cis-retinoids. Mutations in its encoding gene RLBP1 lead to inherited retinal diseases with the common feature of poor night vision. Zebrafish possess two RLBP1 paralogs, rlbp1a and rlbp1b, with distinct retinal expression profiles, providing a bespoke opportunity to dissect cell-specific functions of CRALBP. Here, we first resolved conflicting reports on paralog expression by interrogating zebrafish single-cell RNA-sequencing datasets, which revealed predominant rlbp1a expression in Müll

Divergent mechanisms of neural adaptation and instability in the mammalian retina.

Sensory circuits can exhibit remarkable resilience to disruption, often maintaining function through recruitment of compensatory mechanisms. In the mammalian retina, the balance between ON and OFF pathways that encode distinct luminance profiles is essential for processing visual information. How selective disruption of one input stream can trigger adaptive and/or compensatory measures in retinal output neurons is not fully understood. To determine how retinal output circuits can adapt to different degrees of input suppression, we genetically suppressed the ON pathway input in two models with

Comprehensive genotypic and phenotypic analysis of Bietti crystalline dystrophy: insights from a large cohort study.

In this cross-sectional observational study, we conducted comprehensive ophthalmic evaluations, including fundus imaging, fundus autofluorescence (FAF), swept-source optical coherence tomography (SS-OCT), microperimetry, and electroretinography, in 164 patients with Bietti crystalline dystrophy (BCD) from 159 families. The mean age of onset was (29.9±8.9) years, with night blindness being the most common onset symptom (54.3%). FAF revealed diffuse hypoautofluorescence in 86.7% of eyes, and SS-OCT identified outer retinal tabulations (ORTs) in 69.9% of eyes, indicative of severe retinal pigment

Electronegative electroretinograms in two siblings with bradyopsia, an underdiagnosed pediatric inherited retinal disorder.

In the scotopic flash response of the full-field electroretinogram (ffERG), the downward a-wave reflects photoreceptor response and the upward b-wave is indicative of bipolar cell function. If the b-wave is equal to or smaller than the a-wave, the waveform (and the ffERG) is termed electronegative. For pediatric inherited retinal disorders this finding narrows the differential diagnosis to certain conditions, most commonly congenital stationary night blindness if multimodal imaging is grossly normal. We describe 2 siblings with reduced vision in the setting of normal multimodal imaging. Inheri

Progressive Cone Dystrophy and Cone-Rod Dystrophy.

A heterogenous group of diseases, progressive cone dystrophy usually begins in the mid-teenage years or later in life. The estimated prevalence is 1 in 30,000-40,000 individuals. Patients usually present with decreased central vision and a color vision deficit; the visual loss is progressive and often accompanied by day blindness (hemeralopia) and light intolerance (photophobia). Over time, affected individuals develop night blindness and loss of peripheral field. Visual acuity deteriorates to 20/200 or even counting fingers. There is some association between X-linked cone-rod dystrophy (CORD)
