The amblyopic acuity deficit: Identification of letters distorted by spatial scrambling algorithms.

The amblyopic deficit in letter acuity is often more severe than that predicted using periodic grating stimuli. One proposed explanation for this is spatial scrambling of the input received from the amblyopic eye. In this study, we generated letter stimuli simulating either: i) "cortical scrambling" at the output of oriented model "simple cells", or ii) "subcortical scrambling" of isotropic subunits forming the input to those simple cells. We used these stimuli to investigate whether amblyopic participants might be particularly vulnerable, or particularly resilient, to spatial scrambling. In t

AI-Guided Home-Based Therapy for Convergence Insufficiency: A Comparative Feasibility Study with Pencil Push-Ups.

BACKGROUND: Convergence insufficiency (CI) is a common binocular vision disorder that causes eye strain, headaches, and blurriness. Although pencil push-ups (PPU) are widely used to treat CI, their real-world effectiveness may be limited by inconsistent performance and poor adherence. MobileS (the App) is an AI-based smartphone application that tracks eye movements through the front camera, enabling guided "digital push-ups" with instant visual feedback and automatic performance monitoring. This follow-up study evaluated the feasibility, recorded adherence, and preliminary clinical signals of

Neuro-Optometric Rehabilitation to Prevent Post-concussion Visual Complications: A Case Report.

A concussion is a mild traumatic brain injury (mTBI) caused by an insult to the brain from an outside force. It is a concerning health issue affecting all ages, sexes, and races. Diagnosing mTBI involves physical tests, as mTBI can't be seen through imaging, such as CT or MRI scans. Management of an mTBI is typically rest and over-the-counter medications; however, mTBI can cause complications such as photosensitivity and ocular dysfunction. In this context, this article presents a case report of a 15-year-old hockey player who was hit in the face by an elbow at high speed. The patient was init

Abducens nerve palsy as a false localising sign in spontaneous intracranial hypotension.

Spontaneous intracranial hypotension (SIH) with the formation of a spinal longitudinal epidural collection (SLEC) is an important treatable cause of headache. Left untreated, neurological sequelae can include cranial neuropathy, superficial siderosis and brain-sagging dementia. We describe a male in his late 40s presenting with orthostatic low-pressure headache and binocular horizontal diplopia due to an isolated left abducens nerve palsy, a recognised false localising sign. Imaging revealed intracranial hypotension associated with a SLEC, attributed to a cervical osteophyte associated dural t

Stereopsis Assessment in a Pediatric Population and Its Association With Refractive Error: A Cross-Sectional Study.

INTRODUCTION: Stereopsis is the perception of depth arising from binocular vision and is essential for fine motor skills and overall visual function in children. It has been observed that refractive errors diminish binocular function by causing visual blur and impairing fusion, which results in poor stereopsis. Poor stereoacuity can lead to inferior quality of vision and substandard work performance, especially where good eye-hand coordination and visual motor skills are required. PURPOSE: The purpose of this study is to assess the level of stereoacuity among children with refractive errors a

Wernicke encephalopathy presenting with severe optic neuropathy and oculomotor dysfunction.

A female in her 40s presented with binocular diplopia and blurry vision for three days following two months of daily vomiting, diarrhoea and poor oral intake. Initial examination was notable for bilateral abduction palsies and bilateral dysmetria on finger-to-nose testing. One day later, oculomotor dysfunction progressed with new restriction in upwards and downwards gaze. Dilated fundoscopic examination revealed severe bilateral optic disc oedema with surrounding haemorrhage. MRI Brain and Orbits were notable for T2Fluid-Attenuated Inversion Recovery (FLAIR) hyperintensities in the periaqueduc

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Novel progressive rod-cone degeneration mutation causes retinitis pigmentosa.

Retinitis pigmentosa (RP) is a genetic disorder often caused by single or multiple mutations. The progressive rod-cone degeneration (PRCD) gene is linked to retinal degeneration. This study identified a family affected by RP carrying a frameshift mutation (c.67del) in the PRCD gene. Blood samples from a patient with RP and three family members were analyzed by whole-exome sequencing. A point-mutant mouse model was created using gene-targeted modification to validate the suspected causative gene phenotypically. Binocular ophthalmological exams, including electroretinography and optical coherenc

Gene therapy in children with AIPL1-associated severe retinal dystrophy: an open-label, first-in-human interventional study.

BACKGROUND: Retinal dystrophy caused by genetic deficiency of AIPL1 causes severe and rapidly progressive impairment of sight from birth. We sought to evaluate whether early intervention by gene supplementation therapy was safe and could improve outcomes in children with this condition. METHODS: This non-randomised, single-arm, clinical study conducted in the UK involved four children aged 1·0-2·8 years with severe retinal dystrophy associated with biallelic disease-causing sequence variants in AIPL1. We designed a recombinant adeno-associated viral vector comprising the human AIPL1 coding se

Driving status, avoidance, and visual impairment among older adults in the United States.

BACKGROUND: Late-life visual impairment (VI) increases driving difficulty and affects driving outcomes, such as driving cessation and avoidance. Existing studies are limited by small sample sizes and a lack of objective VI measures in older drivers on a national scale. METHODS: Using nationally representative data from the 2022 National Health and Aging Trends Study, we examined associations between objectively measured VI, driving cessation, and avoidance among U.S. adults aged ≥65 (n=4,980). VI was defined using measures of binocular distance visual acuity and contrast sensitivity (CS). Dri

Binocular dysfunction in Parkinson's Disease: Decoding near triad dynamics and divergence deficits.

Parkinson's Disease (PD) is a progressive neurodegenerative disorder that often affects the oculomotor system, causing strabismus and vergence impairments, particularly convergence insufficiency when focusing on nearby objects. This study evaluated eye deviation, vergence, accommodation, and pupil responses in PD during converging and diverging gaze shifts, correlating these findings with neurologic severity using the Unified Parkinson's Disease Rating Scale (UPDRS). We recruited 19 participants with varying severity of PD and 10 age-matched controls. 26 % of PD participants (PD Group 1) exhib

Magnocellular-biased interocular integration during onset rivalry in normal vision and binocular imbalance.

Previous studies have indicated that the magnocellular (M) pathway may resist binocular rivalry, but the roles of the M and parvocellular (P) pathways in binocular rivalry and integration have been unclear, as was whether these pathways show different deficits in visual disorders. This study used a self-designed dichoptic paradigm to investigate the roles of the M and P pathways in interocular integration and onset rivalry in normal and unbalanced vision. Results showed that interocular motion integration increased with higher temporal and lower spatial frequencies, aligning with M pathway pro
