Contrast detection, discrimination and adaptation in patients with Parkinson's disease and multiple system atrophy.

Visual spatial contrast thresholds and suprathreshold contrast matches were measured before and after adaptation to high-contrast sinewave gratings in patients with Parkinson's disease (n = 27), patients with multiple system atrophy (n = 6) and a group of age-matched control patients without CNS disease (n = 27). Contrast thresholds were higher in the Parkinson's disease patients than in either the multiple system atrophy patients or control patients. The effect of contrast adaptation on both contrast thresholds and matches was approximately equal in the three groups. This suggests that contra

Visual function and quality of life among patients with glaucoma.

This study determines the relation between visual field impairment, visual functioning, and global quality of life in patients with glaucoma. Binocular visual field impairment was calculated from simultaneous Esterman visual field testing using the Humphrey automated perimeter. Visual acuity impairment, defined with the American Medical Association's Guides to the Evaluation of Permanent Impairment; visual functioning, measured with the VF-14 and the field test version of the National Eye Institute-Visual Functioning Questionnaire; and global quality of life, assessed with the Medical Outcomes

Bitemporal hemianopia in photosensitive epilepsy: a case study.

This paper reports the occurrence of bilateral hemianopia in a 16 year old male who was having unusual seizures accompanied by severe migrainous headaches and loss of vision while watching a television programme and while playing with the computer. Electrophysiological tests not only confirmed his photo and pattern sensitivity, but also showed that he had bitemporal hemianopia. Hence, his basic EEG showed a great deal of abnormality including generalised spike and wave activity which was more marked in the temporal regions. The patient showed classical occipital spikes on exposure to 25 and 50

Stereopsis and disparity vergence in monkeys with subnormal binocular vision.

The surgical treatment for strabismus in infants generally results in microtropia or subnormal binocular vision. Although the clinical characteristics of these conditions are well established, there are important questions about the mechanisms of binocular vision in these patients that can best be investigated in an appropriate animal model. In the present psychophysical investigations, spatial frequency response functions for disparity-induced fusional vergence and for local stereopsis were studied in macaque monkeys, who demonstrated many of the major visual characteristics of patients whose

Timing of the critical period for plasticity of ocular dominance columns in macaque striate cortex.

Visual deprivation induced by monocular eyelid suture, a laboratory model for congenital cataract, results in shrinkage of ocular dominance columns serving the closed eye. We performed monocular suture in macaques at ages 1, 3, 5, 7, and 12 weeks to define the critical period for plasticity of ocular dominance columns. After a minimum survival of 8 months, complete montages of [3H]proline-labeled columns were reconstructed from flat-mounts of striate cortex in both hemispheres. In any given monkey, visual deprivation induced the columns throughout striate cortex (V1) to retract the same distan

Positron-emission tomographic study of human amblyopia with use of defined visual stimuli.

PURPOSE: The purpose of this study was to use positron emission tomography (PET) to evaluate effects of amblyopia on cerebral blood flow and glucose metabolism in humans viewing defined visual stimuli and to correlate these effects with specific behavioral and electrophysiologic measures of visual function. METHODS: One subject with normal vision and five patients with amblyopia were prospectively studied. During monocular viewing of a checkerboard reversal stimulus by each subject, we performed PET imaging of relative cerebral glucose metabolism with use of [18F]fluorodeoxyglucose, PET imagi

Full-time atropine, intermittent atropine, and optical penalization and binocular outcome in treatment of strabismic amblyopia.

OBJECTIVE: The purpose of the study is to evaluate the monocular and binocular outcome of three types of "penalization" (blurring of the sound eye) treatment of amblyopia: traditional full-time atropine or optical penalization and a new intermittent atropine regimen involving atropine instillation 1 to 3 days a week. DESIGN: The study design was a retrospective study. PARTICIPANTS: A total of 163 patients with strabismic amblyopia treated by full-time atropine (n = 38), intermittent atropine (n = 73), or optical (n = 52) penalization participated. MAIN OUTCOME MEASURES: Logarithm of the min

Binocular blindness following dynamic cardiomyoplasty.

A 40-year-old man with viral cardiomyopathy underwent uncomplicated dynamic cardiomyoplasty. On the third postoperative day, he was rescued from an 8-hour period of severe cardiogenic shock. Eleven days postoperatively, he was withdrawn and uncooperative, preferring to be left alone. He then complained of total blindness, which persisted to his sudden death on postoperative day 26. Clinical examination revealed no neurological abnormality except for signs consistent with a diagnosis of ischemic optic neuropathy. The optic nerve in the scleral canal is especially vulnerable to hypoperfusion. Bl

The neural basis of suppression and amblyopia in strabismus.

The neurophysiological consequences of artificial strabismus in cats and monkeys have been studied for 30 years. However, until very recently no clear picture has emerged of neural deficits that might account for the powerful interocular suppression that strabismic humans experience, nor for the severe amblyopia that is often associated with convergent strabismus. Here we review the effects of squint on the integrative capacities of the primary visual cortex and propose a hypothesis about the relationship between suppression and amblyopia. Most neurons in the visual cortex of normal cats and m

Ocular abnormalities in congenital cytomegalovirus infection.

BACKGROUND: Congenital cytomegalovirus (CMV) infection often results in damage to the brain, auditory system and visual system that leads to debility. This report describes the frequency, severity and effect on vision function of ocular abnormalities in a large cohort of children with congenital CMV infection. METHODS: Serial eye examinations were performed on 445 infants and children with congenital CMV infection from one month to 9 years of age, assessing anatomic integrity, motor sensory function, retina and posterior pole. RESULTS: Ocular disease was manifest principally by chorioretinit

Visual field defects in esotropic cats: a developmental consequence of the squint.

In nine adult cats with surgically induced esotropia (squint-angle ranging between 8 degrees and 32 degrees), the extent of binocular and monocular visual fields was behaviourally assessed by the food-perimetry method. Results indicate that: (1) The extention of both the nasal and the temporal hemifield of the esotropic eye is narrowed. (2) Field losses are largest in the cats with severe esotropia (> 15 degrees); yet the relationship between rate of impairment scored in each animal and individual angles of squint is not linear. (3) In the cats with severe esotropia, the visual field of the no

Visual acuity in unilateral cataract.

BACKGROUND: Patching the fellow eye in infancy is a well recognised therapy to encourage visual development in the lensectomised eye in cases of unilateral congenital cataract. The possibility of iatrogenic deficits of the fellow eye was investigated by comparing the vision of these patients with untreated unilateral patients and binocularly normal controls. METHODS: Sweep visual evoked potentials (VEPs) offer a rapid and objective method for estimating grating acuity. Sweep VEPs were used to estimate acuity in 12 children aged between 4 and 16 years who had had a congenital cataract removed

Review of experience with retinopathy of prematurity from the Pavia registry (1990-1993).

In this retrospective study we analysed the prevalence of retino-pathy of prematurity (ROP) and its sequelae in a sample of premature infants with birth weight less than 1500 grams. From 1 January 1990 to 31 October 1993, we studied 160 surviving premature infants with very low birth weight (< 1.500 g) referred to the Neonatal Intensive Care Unit of the I.R.C.C.S. Policlinico San Matteo, Pavia, Italy. Thirty percent of these infants suffered from ROP, and 13.7%, with severe ROP, underwent cryotherapy. These latter had very-very low birth weight (< 1.000 g) and low gestational age (< 28 weeks).

Blindness and low vision in southeast Turkey.

In preparation for the planning of a regional prevention of blindness programme, a population-based survey of blindness and eye disease was conducted in two provinces (Diyarbakir and Mardin) of southeast Turkey. A stratified cluster random sampling procedure was used to select 8,571 persons. The main objectives of the survey were to assess the population need for basic ophthalmic services, both in rural and in rapidly growing urban communities, and to secure baseline data for subsequent evaluation of the programme. The prevalence of visual impairment (best binocular vision poorer than 6/ 18) w

Grating acuity and visual field development in infants following perinatal asphyxia.

Grating acuity and visual fields were assessed in 66 children who had had perinatal asphyxia (ASPH). Also tested were 41 healthy preterm children (H-PT). Subjects were tested at birth to one month, and four, nine, 12, 17, 24, 30 and 36 months corrected age. The mean acuity scores of the ASPH group were lower than those of the H-PT group at most test ages, and significantly so at 30 and 36 months. The mean visual field size of the infants in the ASPH group was significantly smaller than that of the H-PT group at nine, 12, 30 and 36 months. No significant effects were found for preterm vs term b

The fragile X female: a case report of the visual, visual perceptual, and ocular health findings.

BACKGROUND: Fragile X syndrome is one of the most frequently encountered etiologies of X-linked mental retardation. Even though it is second only to Down syndrome in prevalence among children with chromosomal abnormalities, few studies have investigated the visual, visual perceptual and ocular health anomalies associated with this syndrome. No studies have been published that report on these areas for females with fragile X. This case report describes a 10-year-old white female with fragile X syndrome who exhibited numerous oculo-visual and perceptual abnormalities. METHODS: A 10-year, 7-mont

Visual impairment in severe and profound sensorineural deafness.

The frequency of reversible and irreversible visual impairment was determined in children with severe and profound sensorineural deafness, as subnormal vision can adversely affect their educational and social development. Eighty three of 87 such children attending an audiology service were examined to assess the incidence and severity of visual impairment. Each child underwent a detailed ophthalmic assessment. The criteria for visual impairment were visual acuity < 6/9 Snellen or equivalent and/or abnormal binocular vision. Forty five had a normal ophthalmic examination (54.2%). Twenty nine ha

Eclamptogenic Gerstmann's syndrome in combination with cortical agnosia and cortical diplopia.

Cortical blindness is defined as a loss of vision due to bilateral retrogeniculate lesions (geniculocalcarine blindness). Gerstmann's syndrome is a combination of disorientation for left and right, finger agnosia, and profound agraphia, alexia, and acalculia. It is due to a lesion in the left angular gyrus, situated at the confluence of the temporal, parietal, and occipital lobes. We report on a patient who suffered from severe underdiagnosed eclampsia and who developed bilateral extensive medial temporal, parietal, and calcarine ischemic infarctions during an eclamptic fit. In addition, ische
