Vernier acuity of normal and visually deprived cats.

This study examines the relationship between grating and vernier acuity in cats that were either normally reared, unilaterally amblyopic as a result of a period of monocular deprivation, or bilaterally amblyopic resulting from a period of reverse occlusion followed by binocular visual experience. Vernier acuity was assessed on a jumping stand by use of a vernier-grating stimulus similar to that devised for use with human infants. The vernier thresholds for normal cats were 1.2-1.3 min arc, values that were approx. 6 times better than their grating acuity, and hence may represent a true hyperac

Functional effects of bilateral form deprivation in monkeys.

Psychophysical methods were used to study the effects of binocular form deprivation, initiated at 1 month of age, on the visual sensitivities of young monkeys. All the monkeys reared with bilateral form deprivation for 7 weeks or longer had reduced spatial contrast sensitivity for both eyes. Although the contrast sensitivity deficits of the bilaterally form-deprived monkeys generally were larger for one eye than the other, the magnitudes of the deficits were small compared with those produced by similar periods of unilateral form deprivation. For other monocular vision functions investigated,

The long-term effectiveness of different regimens of occlusion on recovery from early monocular deprivation in kittens.

Although the behavioural effects of an early period of monocular deprivation imposed on kittens can be very severe, resembling an extreme form of the human clinical condition deprivation amblyopia, they are not necessarily irreversible. Considerable behavioural as well as physiological recovery can occur if normal visual input is restored to the deprived eye sufficiently early, particularly if the other (initially non-deprived) eye is occluded at the same time (reverse occlusion). However, past work has shown that in many situations the improvement in the vision of the initially deprived eye t

Binocular visual impairment in glaucoma.

Monocular and binocular vision was assessed in patients with glaucoma (n = 21), in patients with ocular hypertension (n = 20), and in age-matched visual control subjects (n = 20) using three tests: color vision with the Lanthony Desaturated D-15 test, low spatial frequency contrast sensitivity with the Pelli-Robson chart, and stereoacuity with the RANDOT test. No significant differences were found among the groups in the severity or type of color vision loss. Monocular contrast sensitivity testing showed considerable overlap among groups but a significant loss of contrast sensitivity in the gl

Binocular beat VEPs: losses of cortical binocularity in monkeys reared with abnormal visual experience.

Binocular beat VEPs were recorded from anesthetized macaque monkeys with diverse visual rearing histories, including surgically induced esotropia, optical prism dissociation, optical anisometropia, monocular form deprivation (MD), and normal rearing. Dichoptic visual stimulation was produced by temporally modulating the luminances of uniform fields presented to each eye. Five pairs of temporal frequencies were used, all of which had interocular differences of 2 Hz. While normally reared animals exhibited robust binocular beat responses strongly tuned to temporal frequency, the responses from m

Management of diplopia on down-gaze following orbital trauma.

Diplopia in the inferior field due to orbital trauma is a notoriously difficult and disabling problem. Even if a blow-out fracture is repaired by current methods, patients may still have diplopia. There also exists another group in whom no demonstrable radiological fracture can be found, and yet there is a severe down gaze deficit. The possible mechanisms of this phenomenon are discussed and the results of horizontal muscle transposition surgery in a series of nine patients is reported. The authors conclude that this procedure is a valuable method of increasing the useful field of binocular si

Long-term stability of prism correction of heterophorics and heterotropics; a 5 year follow-up. Part I: Heterophorics.

Twenty heterophoric and 10 heterotropic patients with long standing severe visual symptoms were corrected with prisms for permanent wearing using the full-correction method of H.-J. Haase. The patients were reexamined 1 and 5 years after completion of treatment. Stable corrections obtained by stepwise optical full-correction were also maintained during a follow-up period of 5 years. Sensory and motor adaptations were largely removed and asymptomatic binocular vision was obtained for nearly all patients. A few cases of partly irreversible amblyopia were indicated, but even in these cases largel

Intact "biological motion" and "structure from motion" perception in a patient with impaired motion mechanisms: a case study.

A series of psychophysical tests examining early and later aspects of image-motion processing were conducted in a patient with bilateral lesions involving the posterior visual pathways, affecting the lateral parietal-temporal-occipital cortex and the underlying white matter (as shown by magnetic resonance imaging studies and confirmed by neuro-ophthalmological and neuropsychological examinations). Visual acuity, form discrimination, color, and contrast-sensitivity discrimination were normal whereas spatial localization, line bisection, depth, and binocular stereopsis were severely impaired. Partial visual recovery in two fullterm infants after perinatal hypoxia.

Behavioural methods were used to assess the visual development of two infants with a history of severe perinatal hypoxia. Both infants were born fullterm and showed hypoxic-ischaemic encephalopathy and subsequent cerebral palsy. Low visual acuity, small and asymmetrical visual fields, asymmetrical binocular and monocular OKN, and absent visual threat response were demonstrated initially. Ophthalmological examination revealed strabismus, but no further abnormalities. Longitudinal assessments up to the age of two years showed impressive (partial) improvements of visual functions. The mechanism f

Unexpectedly large enhancement of a severely constricted field with reversed Galilean telescopes.

A 25-year-old female presented with severely constricted visual fields secondary to stroke associated with mitral valve prolapse. Repeatably measured central fields of less than 4 degrees diameter showed an unexpected enlargement up to 20 to 40 degrees diameter, when fitted with reversed full field 1.3x and 1.7x Galilean telescopes. Although initial tests indicated no psychogenic component, fields of approximately 20 degrees diameter were eventually obtained through an imitation telescope made with 2 plano lenses. This technique may be useful in determining whether there is a psychogenic compo

Evidence for nonlinear binocular interactions in human visual cortex.

Dichoptically presented uniform fields were sinusoidally modulated in luminance, with differing temporal frequencies between the eyes. This stimulus evokes a unique electrophysiological response termed "beats"; visual evoked potential (VEP) components reflecting nonlinear neural behavior and which could only arise from integrative binocular units. Individuals with strabismus or amblyopia, which accompany disrupted binocular function in the visual cortex manifest a severe reduction of this nonlinearity, suggesting an abnormality in the binocular cortical processes underlying this response.

The efficacy of optometric vision therapy. The 1986/87 Future of Visual Development/Performance Task Force.

Vision is not simply the ability to read a certain size letter at a distance of 20 feet. Vision is a complex and adaptable information gathering and processing system which collects, groups, analyzes, accumulates, equates, and remembers information. In this review, some of the essential components of the visual system and their disorders which can be physiologically and clinically identified, i.e., the oculomotor, the accommodative, and the fusional vergence systems have been discussed. Any dysfunctions in these systems, can lessen the quality and quantity of the initial input of information i

Acuity assessment of non-verbal infants and children: clinical experience with the acuity card procedure.

The acuity card procedure was used to assess the visual acuity of 510 neurologically normal and abnormal infants and children. Acuity estimates were obtained for 93 per cent of 842 binocular and 279 monocular tests. The observed development of binocular acuity of normal fullterm and preterm infants agreed well with previous reports using the traditional forced-choice preferential looking technique. Monocular tests seemed to support earlier suggestions that grating acuity may be relatively insensitive to strabismic amblyopia. Infants at risk of later neurological deficits but developing normall

Near vision stress: vergence adaptation and accommodative fatigue.

Changes in the visual system following prolonged near work were investigated. Fifteen young, normal subjects undertook a severe, two hour long, binocular near visual task at 20 cm without any breaks. Fusional stress was assessed by near 'phoria change. Refractive change was measured with an autorefractometer to investigate whether transient myopia occurred and its subsequent recovery. The near task caused vergence adaptation which was primarily due to the fusional stress of the task (accounting for 67% of its variance). The 'phoria change was to a lesser extent (40%) dependent on the accommoda

Complications of frontal defects. Immobilization of the eye.

A case where the severe course of an orbital phlegmon led to a functionally and cosmetically poor condition is presented. Thirty-five years later, it was possible to achieve a satisfactory result through several operations performed with interdisciplinary cooperation. The orbit and forehead were reconstructed with porous polyethylene, thus ensuring a cosmetically good result. In two operations, the left eye was freed from its upward fixation through cicatrectomy and mobilization of the mucles, so that the eyes were straight again in the primary position. There is a limited binocular visual fie

Reduced visual acuity in both eyes of monocularly deprived kittens following a short or long period of reverse occlusion.

While there can be substantial recovery of vision during reverse occlusion in the deprived eye of kittens that have been monocularly deprived from birth, in many situations this visual improvement does not appear to be retained following the introduction of binocular visual input. This study examines the consequences of periods of reverse occlusion whose onset and duration would be thought to maximize the opportunity for good and permanent recovery of vision in the initially deprived eye. Twenty kittens were monocularly deprived from near birth to either 4, 5, or 6 weeks of age and then revers

Monocular and binocular optokinetic nystagmus in humans with defective stereopsis.

In order to investigate the previously suggested relation between cortical binocular function and deficits in monocular optokinetic nystagmus (OKN), monocular and binocular OKN was examined in normal, stereodeficient, and stereoblind observers, using a variety of stimulation velocities and stimulus field sizes. Most stereodeficient and stereoblind observers showed monocular OKN deficits in one or both eyes, which took the form of either a directional asymmetry or a reduction of the response in both directions. The deficits were often more pronounced at higher stimulation velocities and with sm

Bilateral amblyopia after a short period of reverse occlusion in kittens.

The majority of neurones in the visual cortex of both adult cats and kittens can be excited by visual stimulation of either eye. Nevertheless, if one eye is deprived of patterned vision early in life, most cortical cells can only be activated by visual stimuli presented to the nondeprived eye and behaviourally the deprived eye is apparently useless. Although the consequences of monocular deprivation can be severe, they can in many circumstances be rapidly reversed with the early implementation of reverse occlusion which forces the use of the initially deprived eye. However, by itself reverse o

Amblyopia: a multidisciplinary approach. Proctor lecture.

Clinically different forms of amblyopia share as common denominators an inability to form well-defined and focused images in one or both eyes and, in the case of unilateral amblyopia, unequal visual input to the brain. This suggests two amblyopiogenic mechanisms which are effective, individually or in unison, in the various forms of amblyopia. The first is lack of adequate visual stimulation during infancy, causing visual deprivation. The second mechanism is based on abnormal binocular interaction. The clinical evidence and data from the animal laboratory will be reviewed in support of this du

Thirty-six patients with postoperative microstrabismus (24 of them without or with only a slight amblyopia, 12 with severe unilateral amblyopia) and three control groups (normal binocular, long-term monocular, and inexperienced monocular subjects) were tested by their ability to hit 14 vertical sticks set in varied positions in a rectangular board, without any time limit during the first test and with time limits in the second and third tests. The evaluation shows that all 36 patients achieved considerably better results than any of the monocular subjects, but did not score as highly as normal
