Clinical recovery of severe infantile Angiostrongylus cantonensis meningoencephalomyelitis treated with pulse methylprednisolone: A case report and review of literature.

BACKGROUND: Angiostrongylus cantonensis (A. cantonensis) is the leading cause of eosinophilic meningitis worldwide. Although infection is often self-limiting in adults, infants may develop severe meningoencephalomyelitis, associated with high morbidity and mortality. The optimal corticosteroid regimen for infants with severe disease and extensive central nervous system involvement remains unclear, and standard-dose corticosteroid therapy may be ineffective in acute fulminant cases. CASE SUMMARY: A 10-month-old female infant was admitted with fever and vomiting, followed by rapid progression t

Neurological complications of Zika virus and their impact on quality of life: a narrative review.

BACKGROUND: Zika virus (ZIKV), known for its alarming health consequences, is primarily transmitted by infected Aedes spp. mosquitoes but can also spread through sexual contact and from mother to child during pregnancy. While most infections are mild or asymptomatic, ZIKV has been linked to severe neurological disorders, such as Guillain-Barré syndrome (GBS) in adults and congenital Zika syndrome (CZS) in infants. While the substantial economic burden and the range of these disorders have been described, the potential long-term effects and the impact on quality of life (QoL) remain inadequatel

Critical airway obstruction associated with extensive subglottic inflammatory slough and oedema following severe acute respiratory syndrome SARS-CoV-2 infection in a child.

SARS-CoV-2 infection in children is typically mild, although COVID-19-associated croup can be severe. We report a patient in middle childhood with impending critical airway obstruction due to severe subglottic oedema and extensive inflammatory slough/crusting following SARS-CoV-2 infection. Subsequent airway assessment in the operating theatre revealed critical subglottic narrowing. During transport, nebulised epinephrine was initiated; however, the patient developed hypoxaemia (SpO2 80%) with loss of consciousness. On arrival, video-laryngoscopic removal of obstructive slough using Magill for

Rezpegaldesleukin treatment of moderate-to-severe atopic dermatitis (REZOLVE-AD): final results from the 16-week induction period of an international, double-blind, placebo-controlled, randomised phase 2b study.

BACKGROUND: Rezpegaldesleukin is an interleukin-2 receptor agonist that selectively expands and enhances the function of regulatory T cells (Tregs), offering a novel therapeutic approach for autoimmune and inflammatory diseases such as atopic dermatitis. We aimed to compare the efficacy and safety analyses of rezpegaldesleukin with placebo in adults with moderate-to-severe atopic dermatitis naive to biological treatments. METHODS: REZOLVE-AD was a phase 2b, randomised, double-blind, placebo-controlled study conducted in 107 community research centres or hospitals in ten countries (Australia,

Mitochondrial Quality Control Links Exercise to Sterile Inflammation in the Cardiovascular System: A Narrative Review.

Preservation of mitochondrial integrity has emerged as a central hub in the anti-inflammatory effect of exercise. This narrative review advances a framework in which mitochondrial damage-associated molecular patterns (mtDAMPs) serve as the mechanistic bridge between exercise and inflammation. Mitochondrial dysfunction releases mtDAMPs, including mitochondrial DNA (mtDNA), reactive oxygen species, cardiolipin, N-formyl peptides, and ATP, which activate cGAS-STING, the NLRP3 inflammasome, TLR9, AIM2, ZBP1, and NF-κB signaling. Crosstalk among these pathways allows mild mitochondrial damage to es

Beyond Rett syndrome: a case series expanding the neurological spectrum associated with pathogenic MECP2 variants.

BACKGROUND: Although pathogenic variants in MECP2 are classically associated with Rett syndrome (RTT), increasing evidence suggests that they can underlie a broader spectrum of neurological phenotypes. Clinical manifestations may vary according to sex, variant type, residual protein function, and pattern of X-chromosome inactivation. METHODS: We describe five unrelated individuals carrying pathogenic MECP2 variants identified through multiplex ligation-dependent probe amplification, chromosomal microarray analysis, and next-generation sequencing. Clinical, neuroradiological, neurophysiologica

Xanthomatous pleuritis in a child with recurrent pneumonia and patent ductus arteriosus: A rare pediatric case report and literature review.

BACKGROUND: Xanthomatous pleuritis is an exceptionally rare benign inflammatory condition characterized by aggregates of lipid-laden macrophages within the pleura. To the best of our knowledge, only three adult cases have been reported worldwide, associated with chronic pleural inflammation or infection. Pediatric pleural xanthomas are exceedingly uncommon, and xanthomatous pleuritis has not been previously reported in a child. CASE PRESENTATION: We report a 3.5-year-old girl with recurrent pneumonia, chronic respiratory symptoms, and Failure to Thrive (FTT). Chest X ray (CXR) revealed cardio

Association Between Laryngopharyngeal Reflux and the Severity of Obstructive Sleep Apnea.

IMPORTANCE: Laryngopharyngeal reflux (LPR) is frequently encountered in patients with obstructive sleep apnea (OSA). However, its relationship with OSA severity remains uncertain. Clarifying this association may improve reflux phenotyping and guide otolaryngology-focused evaluation in OSA populations. OBJECTIVE: To evaluate the association between LPR-related parameters and OSA severity and the relationship between velum obstruction grade and LPR-related measures. DESIGN: Prospective observational study. SETTING: Single tertiary academic medical center (Peking University Third Hospital), ou

Peritoneal Tuberculosis in a Female Patient Mimicking Peritoneal Carcinomatosis: A Case Report.

Peritoneal tuberculosis (PTB) is a rare extrapulmonary disease. The clinical and radiological findings closely resemble those of peritoneal carcinomatosis, leading to diagnostic confusion. Herein, we present a case of PTB in a patient who underwent general surgery with a preliminary diagnosis of peritoneal carcinomatosis. A 47-year-old woman presented with abdominal pain for three months. Abdominal evaluation revealed generalized tenderness and mild defenses. Contrast-enhanced abdominal CT demonstrated free fluid in the peritoneal cavity, especially in the lower abdomen, irregular infiltration

Beyond Hemorrhage: A Case of Acute Dengue Infection Presenting with Budd-Chiari Syndrome and Bilateral Sensory Polyneuropathy.

Budd-Chiari syndrome (BCS) is a rare hepatic vascular disorder characterized by hepatic venous outflow obstruction with an incidence of 1-2 per million population annually. While dengue fever is primarily associated with hemorrhagic complications, thrombotic events are rarely reported. To our knowledge, this represents the first documented case of BCS occurring in association with dengue fever. A 22-year-old male from a rural area presented with a 4-day history of abdominal pain, fever, vomiting, and loose stools. Physical examination revealed hepatomegaly, ascites, and mild dehydration. The diagnostic value of interleukin-6, procalcitonin, and C-reactive protein in the severity of acute appendicitis in children.

BACKGROUND: Our aim was to explore the diagnostic potential of IL-6, PCT, and CRP for the severity of acute appendicitis (AA) in children. METHODS: We carried out a retrospective study on 412 children (≤ 18 years) with surgically - confirmed AA and 160 children with non-AA abdominal pain as controls (October 2024-September 2025). AA cases were divided into mild (simple/suppurative) and severe(gangrenous/perforated) types based on pathology. We compared the biomarkers (IL-6, PCT, CRP, WBC, N%) among groups and used ROC curves to evaluate their diagnostic performance. RESULTS: There were no si

A Unique Case of Psychogenic Binocular Diplopia.

Diplopia is a common chief complaint in the ophthalmology clinic, and it is important to differentiate diplopia requiring urgent work-up versus benign forms of diplopia that can be managed conservatively. This case demonstrates a 39-year-old male who reported the ability to move each eye independently resulting in chronic binocular diplopia. He reported having an extensive work-up done by an outside ophthalmologist that returned unremarkable. His glasses on presentation had a mild myopic correction in both eyes (OU), 1.50 prism diopters (PD) base up (BU) in the right eye (OD), and 0.50 PD BU i

A visual evoked potential and optical coherence tomography cross-sectional study in patients with Behcet's disease.

INTRODUCTION: Previous studies have reported conflicting results regarding visual evoked potential (VEP) and OCT abnormalities in patients with Behçet's disease (BD). This study aimed to address these discrepancies by investigating subclinical optic neuropathy and retinal dysfunction in Behçet's disease using VEP and OCT. METHODS: Seventy-five patients with BD and 40 age- and sex-matched healthy controls underwent comprehensive neurological and ophthalmological examinations. VEP was used to assess P100 latency and amplitude, while peripapillary retinal nerve fiber layer (RNFL) and ganglion ce

Maintenance of Minimal Manifestation Status with Individualized-Interval Efgartigimod in Generalized Myasthenia Gravis: Study Protocol for a Single-Arm, Open-Label Clinical Trial.

INTRODUCTION: Efgartigimod, a human immunoglobulin G1 Fc fragment targeting the neonatal Fc receptor, improves symptoms in generalized myasthenia gravis (gMG). It remains unknown whether minimal manifestation (MM) status achieved with efgartigimod can be durably maintained over the long term, and whether subcutaneous (SC) self-administration reduces treatment burden and supports quality of life. We designed a prospective study to evaluate maintenance of MM with low-dose oral prednisolone (PSL) ≤ 5 mg/day (MM-5 mg) under interval-adjusted efgartigimod administration. METHODS: This single-arm,

Efficacy of picosecond Nd:YAG 1064 nm laser in melasma treatment: a real-world retrospective analysis.

BACKGROUND: Melasma is a challenging hyperpigmentation disorder, especially prevalent in individuals with darker skin tones. Traditional treatments, like hydroquinone and chemical peels, have limitations due to high recurrence rates and post-inflammatory hyperpigmentation (PIH) risk. Picosecond lasers have emerged as promising alternatives for treating melasma in patients with Fitzpatrick skin types III-IV. This study aims to evaluate the efficacy and safety of the 1064 nm Nd:YAG picosecond laser for melasma in a Vietnamese population. METHODS: This retrospective study analyzed 29 female pati

The impact of amblyopia, reduced viewing conditions and binocular vision on reading ability: a narrative review.

Amblyopia is a neurodevelopmental disorder of the visual cortex which leads to issues in visual acuity, contrast sensitivity and eye movement patterns, all characteristics which, along with other deficits may negatively influence reading skills. Individuals with reduced reading capabilities due to amblyopia or other binocular vision disorders may have difficulties navigating various social aspects of daily life including employment and academics. Reading is an active viewing task which involves multiple oculomotor and cognitive processes. In this review, we introduce how a lack of binocular vi

When to Continue, Pause, or Stop SGLT-2 Inhibitors in Heart Failure: A Complication-based Clinical Guide.

Sodium-glucose cotransporter 2 (SGLT2) inhibitors have become an essential part of guideline-directed medical therapy for heart failure due to their proven ability to reduce hospitalizations and improve survival, independent of diabetes status. Despite these benefits, their use can be complicated by genitourinary side effects such as genital mycotic infections, urinary tract infections, and, rarely, serious complications including urosepsis, Fournier's gangrene, and euglycemic diabetic ketoacidosis. These events often leave clinicians uncertain about whether therapy should be continued, paused

Lifetime trauma, recent socioeconomic stressors, and cumulative PTSD prevalence: a multi-country population-based study.

BACKGROUND: Posttraumatic stress disorder (PTSD) research has largely focused on exposure to DSM-defined traumatic events, while comparatively little attention has been paid to the role of ongoing socioeconomic stressors. Few contemporary, population-based studies have examined how lifetime trauma (e.g., physical violence) and recent socioeconomic stressors jointly relate to PTSD risk, or whether the association between socioeconomic stressors (e.g., losing a job) and PTSD differs by trauma burden among trauma-exposed individuals. METHODS: We analyzed data from the Global Social Determinants
