Carotid endarterectomy specimens surgically removed in one piece were kept intact, processed pathologically, and serially sectioned at 8 microns. There were 34 cases with multiple hemispheric transient ischemic attacks (TIAs), 23 cases with multiple episodes of transient monocular blindness (TMBs), 33 asymptomatic cases, and 51 cases with prolonged or persisting neurologic deficits. In 205 patients with carotid transient ischemic attacks (TIAs) who underwent full angiographic and cardiac investigations, we found that ipsilateral carotid bruit and triggering of TIA by exertion or standing up suggested a carotid lesion. Angina pectoris or palpitations at onset of TIA suggested a cardiac source of emboli. Studies of visual evoked potentials in head injuries have all utilized the flash stimulus. We studied the efficacy of monocular pattern-shift visual evoked potentials (PSVEPs) in eliciting residual dysfunction of the visual pathways in 33 patients 6 to 24 months after head injury. Those with ocular trauma or ocular pathology were not included. Abnormal PSVEPs were seen in one-third of 33 head-injured patients. Only 1 (11%) of 9 patients with mild cognitive impairment had abnormal PSVEPs compared with 7 (39%) of 18 with moderate and 3 (50%) of 6 with severe cognitive impairment.

It has been accepted that transient monocular blindness (TMB) is highly suggestive of an ipsilateral carotid artery lesion in patients with arterial risk factors. We confirmed this in 55 patients, who had an internal carotid artery severe stenosis, occlusion or ulceration on angiography in more than 50 per cent of the cases. Thirty-six patients with postoperative microstrabismus (24 of them without or with only a slight amblyopia, 12 with severe unilateral amblyopia) and three control groups (normal binocular, long-term monocular, and inexperienced monocular subjects) were tested by their ability to hit 14 vertical sticks set in varied positions in a rectangular board, without any time limit during the first test and with time limits in the second and third tests. Visual acuity of infants and young children with ophthalmologic disorders was assessed by adapting a transformed up-down staircase to preferential looking (PL) procedures. Eighty-five percent of pediatric patients between 11 days and 5 years of age were tested successfully. We examined monocular spatial vision of strabismic amblyopes by measuring errors of relative directionalization (specifying whether or not two targets are in vertical alignment) and partitioning (equating left- and right-field spaces). We studied 11 patients with the uncommon finding of traumatic chiasmal syndrome after closed head trauma. Visual field defects varied from complete monocular blindness and contralateral temporal hemianopia to subtle bitemporal arcuate scotomas. The degree of visual loss was not necessarily related to the severity of the craniocerebral trauma. Diabetes insipidus was present in half of these patients, but unlike the visual abnormalities, this complication was transient.

Fourteen clinically affected and 40 asymptomatic members of a six generation family with Leber's optic neuropathy have been studied clinically and by recording pattern-reversal visual evoked potentials. While 12 of the affected members had suffered the typical sequential bilateral failure of vision, in 2 the condition was still monocular after periods of 12 and 14 years. The most commonly used methods for the treatment of amblyopia are based on dosed obstruction of the eye with the better function. Occlusion therapy is still the treatment of choice for severe (eccentric fixation) amblyopias. Examination during an episode of transient ischemic attack (TIA) was performed on 121 of 1,307 patients suspected of having a single type of TIA. This examination supported the diagnosis of TIA in 79 patients and contributed to the establishment of a diagnosis other than TIA in 42.
