BACKGROUNDTraumatic optic neuropathy (TON) is a leading cause of blindness following closed traumatic brain injury, with no effective treatments available. Previous interventional clinical trials were complicated by its low prevalence, variability in neurodegenerative severity, and unavailability of reliable biomarkers.METHODSWe analyzed data from 1,226 patients enrolled in the prospective National Multi-Center Collaborative Clinical Research Program of China (2017-2024) to establish a clinical profile and identify noninvasive biomarkers for neurodegenerative severity. Subgroup analysis of pat

In this study, patients with inherited retinal dystrophies (IRDs) who visited Ningxia Eye Hospital from January 2015 to September 2023 were analyzed. Through Whole Exome Sequencing (WES) and Sanger verification, 17 probands carrying homozygous variants were detected. The association between the genotype and clinical phenotype of patients with homozygous variants was analyzed. Among all the patients, 3 patients (17.6%) had a family history of consanguineous marriage, and the onset age of 5 patients(29.4%) was less than 10 years. According to 12 patients (70.6% ), they had the best corrected vis

BACKGROUND: Injection sclerotherapy using sclerosants such as polidocanol has been widely employed for managing vascular disorders including chronic venous diseases and hemangiomas. Although sclerotherapy is considered a minimally invasive and generally safe procedure, rare but vision-threatening ocular complications have been reported. We present a unique scenario of progressive ophthalmic artery occlusion (OAO) associated with foamed polidocanol injection leading to irreversible blindness in a pediatric patient.

BACKGROUND: While historically amaurosis fugax was clinically categorized similarly to ipsilateral hemispheric transient ischemic attack (TIA) or stroke, it remains unclear whether transient monocular blindness confers comparable risk to patients in contemporary practice. The purpose of this analysis was to compare postoperative stroke risk among patients undergoing carotid endarterectomy (CEA) for amaurosis, compared with patients with TIA, stroke, or asymptomatic carotid stenosis.
METHODS: We studied patients who underwent CEA in the Vascular Quality Initiative (VQI) (2016-2024) and perform

Purpose: Monocular Elevation Deficiency (MED) is characterized by limited ability to elevate the affected eye in any gaze position, while maintaining normal duction in all other gazes. The type of surgeries described in MED include Knapp's surgery, vertical muscle recession and resection, contralateral superior rectus recession. Recently, the Nishida's procedure has also been explored as a treatment for isolated cases of MED. Hence, we conducted this study to evaluate the surgical outcome of Modified Nishida's procedure in cases of MED in a series of 15 patients. Methods: This prospective inte

We present the case of a 72-year-old male who developed Charles Bonnet Syndrome (CBS) following a right retinal artery occlusion. Complex visual hallucinations are the main symptom of CBS in persons with substantial vision loss. It has been conventionally associated with bilateral severe impairment, but recent observations indicate under-recognised monocular cases that offer equally notable incidences of CBS. This case highlights the importance of recognising that CBS can also occur in monocular blindness and its underdiagnosed prevalence in current literature. In order to avoid the misidentif

BACKGROUND: Central retinal artery occlusion (CRAO) is a subtype of ischaemic stroke that results in acute monocular vision loss. Although open-label studies and meta-analyses have suggested that early intravenous thrombolysis might improve visual acuity, no randomised controlled trials have yet confirmed this benefit. We aimed to compare the safety and efficacy of intravenous alteplase with oral aspirin in patients with CRAO treated within 4·5 h of onset of severe vision loss.
METHODS: THEIA was a multicentre, double-dummy, patient-blinded, assessor-blinded, randomised, controlled, phase 3 t

OBJECTIVE: Carotid artery stenosis may cause ocular ischaemic events (OIE) due to thrombo-embolic events or reduced perfusion to the ophthalmic arteries. While these patients have a lower risk of recurrent ipsilateral ischaemic events compared with patients with hemispheric events, the potential relationship between OIE and cerebral white matter hyperintensities (WMHs), as a manifestation of small vessel disease, has not been explored. This study investigated the presence of WMHs in patients with OIE compared with asymptomatic patients with carotid artery stenosis scheduled for carotid revascu

BACKGROUND: Infectious keratitis, commonly known as corneal infection, is a major cause of blindness, affecting approximately six million people globally and resulting in around two million cases of monocular blindness annually. The incidence varies widely worldwide, with higher rates in low- and middle-income countries due to various risk factors, including agricultural injuries and other accidental trauma, limited access to health care, and low levels of health literacy. Bacterial keratitis (BK) is the most prevalent form in higher-income regions, contributing to significant morbidity and he
