PURPOSE/BACKGROUND: Following laser panretinal photocoagulation (PRP) for proliferative diabetic retinopathy, patients are at risk of failing the UK driving visual field test due to loss of peripheral field. The threshold of stereopsis was measured by static stereopter for 111 children with amblyopia aged from 4 to 13 years. They were divided into three groups: 38 cases ranging in age from 4 to 5 years, 43 cases 6 to 7 and 30 cases 8 to 13. Fifty six cases were unilateral and 55 bilateral amblyopia. Outcome, prognosis and course of pigmentary glaucoma were studied in 38 patients (75 eyes). The mean age of the subjects at the time of diagnosis was 34.0 years and at the time of the study 44.1 years. The follow-up time ranged from 2 to 30 years with a mean follow-up time of 10.1 years. We studied 20 patients with acute optic neuritis prospectively for 12 months. Visual fields, color vision, and VEPs to 15' checks were initially abnormal in all patients. Visual acuity was abnormal in 90% and contrast sensitivity in 95% of patients. We devised a graded visual impairment scale (GVIS) to include all visual functions tested. Complete recovery of visual function occurred in 65% of cases. Recovery in the majority of patients was rapid and complete within the first 2 months. Visual acuity, the character of fixation, the angle of deviation, refraction, monocular determination of accommodation and blood supply of the ciliary body have been studied in 50 schoolchildren (100 eyes) with convergent strabismus and amblyopia of various degree. Children with amblyopia of high and moderate degrees have shown a sharp fall or absolute absence of the positive part of relative accommodation and reduced blood supply of the ciliary body. Between the mentioned indices and degree of amblyopia there exists a positive connexion.

Orthoptic therapy was instituted in a 29-year-old patient having moderate amblyopia, constant small-angle esotropia, and large and steady eccentric fixation. This combination of factors, especially the age, pointed toward a poor prognosis for attainment of markedly improved vision function. Rate of recovery of several monocular vision functions was monitored during one year of orthoptic therapy. Results showed substantial improvement in most areas, thus providing evidence of neural plasticity at multiple sites in the visual pathways in this adult amblyope.

Studies of visual evoked potentials in head injuries have all utilized the flash stimulus. We studied the efficacy of monocular pattern-shift visual evoked potentials (PSVEPs) in eliciting residual dysfunction of the visual pathways in 33 patients 6 to 24 months after head injury. Those with ocular trauma or ocular pathology were not included. Abnormal PSVEPs were seen in one-third of 33 head-injured patients. Only 1 (11%) of 9 patients with mild cognitive impairment had abnormal PSVEPs compared with 7 (39%) of 18 with moderate and 3 (50%) of 6 with severe cognitive impairment.

A case of recurrent postprandial amaurosis fugax (AF) associated with periorbital pain is described. Clinical and angiographic examination revealed moderate atherosclerosis of the cerebral vessels and narrow-angle glaucoma. Cerebral postprandial hypoperfusion combined with increased intraocular pressure probably precipitated the painful monocular blindness. The attacks of AF subsided partially after treatment of the glaucoma.
