Nasal Neurostimulation as an Adjunct Novel Treatment for Oculofacial Pain in Herpes Zoster Ophthalmicus-Related Postherpetic Neuralgia.

OBJECTIVES: This study describes the effect of nasal neurostimulation in five cases of postherpetic neuralgia (PHN) after herpes zoster ophthalmicus. MATERIALS AND METHODS: This retrospective case series included five symptomatic patients with PHN who were receiving conventional pain therapy and underwent a single session of nasal neurostimulation for additional oculofacial pain relief. Neurostimulation was performed either through intranasal or extranasal application. Pain intensity was assessed before and after treatment using the visual analog scale (VAS; 0-10). RESULTS: Three patients re

Symptom overlap in chikungunya and dengue: A diagnostic challenge during outbreaks.

Distinguishing dengue from chikungunya at presentation is difficult in co-circulation settings due to substantial symptom overlap. In November 2023, São José do Rio Preto, São Paulo, Brazil, a dengue-endemic city, faced its first recognized chikungunya outbreak, challenging frontline triage and surveillance. We conducted a hospital-based retrospective cross-sectional study (November 2023-August 2024), integrating clinical variables with RT-qPCR confirmation to compare syndromic profiles and severity markers between dengue and chikungunya. Neurological cases were captured through active surveil

Acute retinal necrosis presenting exudative retinal detachment: a case report.

BACKGROUND: Acute retinal necrosis (ARN) is a severe, rapidly progressive viral retinitis that is commonly complicated by rhegmatogenous retinal detachment in its late stage. However, the presentation of ARN with exudative retinal detachment (ERD) in the early phase is exceptionally rare, particularly when caused by varicella zoster virus (VZV) in an adult patient. This report highlights this atypical presentation, which initially occurred without definite evidence of retinal necrosis, posing a diagnostic challenge. CASE PRESENTATION: A 43-year-old woman presented with acute blurred vision, e

Case report: Central retinal artery occlusion following scleral buckling surgery with secondary angle closure and recurrent acute ocular hypertension.

Central retinal artery occlusion (CRAO) is a rare but vision-threatening complication following retinal detachment surgery. We report a man in his late 60s who underwent combined phacoemulsification with intraocular lens implantation and segmental scleral buckling for macula-sparing rhegmatogenous retinal detachment caused by inferotemporal ora dialysis. Shortly after surgery, he developed recurrent ocular pain with marked intraocular pressure (IOP) spikes, anterior chamber shallowing, iris bombe, and secondary angle closure. Approximately 3 h post-operatively, visual acuity deteriorated to no

Syphilitic Panuveitis Presenting as Bilateral Granulomatous Uveitis in an Immunocompetent Patient: A Case Report.

Ocular syphilis is a re-emerging, sight-threatening manifestation of Treponema pallidum infection that can mimic a wide range of inflammatory eye diseases. We report the case of a 31-year-old immunocompetent woman who presented with a two-month history of progressive bilateral blurred vision, ocular pain, redness, photophobia, and floaters. Ophthalmic examination revealed bilateral granulomatous panuveitis with mutton-fat keratic precipitates, anterior chamber inflammation, vitritis, optic disc hyperemia, and multifocal chorioretinal lesions. Dermatologic examination demonstrated non-pruritic

Primary tubercular chorioretinitis: A case report.

Intraocular tuberculosis often evades diagnosis owing to its nonspecific clinical features and frequent absence of systemic signs. We describe a rare case of primary tubercular chorioretinitis in an immunocompetent woman in her early 20s who presented with acute vision loss, ocular pain, and redness in the left eye. Multimodal imaging demonstrated active inflammation arising within-not at the margin of-pre-existing focal chorioretinal atrophy, accompanied by retinal vasculitis and choroidal hypoperfusion. Systemic workup revealed no extrapulmonary involvement; however, interferon-gamma release

Panophthalmitis With Orbital Cellulitis Following Glaucoma Drainage Implant Surgery in an Adult Patient.

BACKGROUND Glaucoma drainage implants, including the Paul glaucoma implant, are widely used in the management of refractory glaucoma when medical therapy and conventional filtering surgery prove insufficient. Although associated with favorable outcomes, rare but serious postoperative complications such as endophthalmitis and panophthalmitis can occur, resulting in significant visual morbidity. CASE REPORT We describe the case of a 77-year-old man with chronic angle-closure glaucoma who developed acute panophthalmitis with orbital cellulitis 12 days following uneventful Paul glaucoma implant su

Clinicopathological Patterns, Associations, and Management of Orbital Involvement in Sinonasal Malignancies: A Retrospective Study.

Background Sinonasal malignancies are uncommon head and neck tumors that frequently present at an advanced stage due to their deep anatomical location and nonspecific early symptoms. Orbital involvement represents an important indicator of locally aggressive disease and has direct implications for staging and treatment planning. This study was conducted to analyze the clinicopathological profile, radiological patterns of orbital invasion, and management strategies in patients with sinonasal malignancies involving the orbit. Methods A retrospective observational study was carried out in the Dep

Iturin A disrupts Cryptococcus neoformans membrane integrity, attenuates virulence, and synergizes with amphotericin B.

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

A Rare Pediatric Giant Cell Tumor of the Clivus Bone, H3.3 p.Gly35Trp-mutated: Case Report and Mini-review of the Literature.

Introduction: Giant cell tumor of bone (GCTB) is a rare, typically benign neoplasm that primarily affects long bones in adults, with clival involvement being extremely rare, particularly in pediatric cases: a mini-review shows a total of 28 described cases, of which only 5 were truly pediatric (within 14 years of age). Surgery is the treatment of choice, and Denosumab is reported to be the most effective drug therapy. To date, the GCTB's molecular hallmark is the somatic mutation p.Gly34Trp, at the H3F3A gene (H3.3 p.Gly34Trp mutation), but in this case, the mutation H3.3 p.Gly35Trp was identi

Nociplastic pain among individuals with chronic ocular surface pain: One cause for "pain without stain"?

Chronic ocular surface pain (COSP) refers to interrelated symptoms such as burning, aching, and irritation and can occur as an isolated condition or comorbid with numerous ocular disorders, including dry eye syndrome Treatments for COSP are largely aimed at the ocular surface and modulating pain arising from damaged corneal nerves; however, the average impact of these treatments on COSP are low to absent. A potential explanation for this is that, in a subset of patients with COSP, individuals have amplified and/or dysregulated neural signaling and sensory processing within the central nervous

Prevalence and risk factors for the development of chronic postoperative pain after cataract surgery in the Age-related Eye Disease Study (AREDS).

Chronic ocular pain impacts quality of life and is often linked to ocular surgery. We assessed the prevalence of chronic postoperative pain (CPOP) after cataract surgery and associated risk factors using a secondary cohort post-hoc analysis of data from the Age-Related Eye Disease Study (AREDS), a multicenter, controlled, randomized clinical trial of antioxidant vitamins and minerals. Ocular pain was determined from item 4 of the National Eye Institute Visual Function Questionnaire (NEI-VFQ-25), administered between 1997 and 2005. We included participants who underwent cataract surgery during
