Two patients, a 48-year-old woman and a 56-year-old man, had hypocomplementemic cutaneous vasculitis syndrome. The patients had ocular pain and photophobia and were found to have bilateral recurrent iridocyclitis. We treated the iridocyclitis with cycloplegia and topical corticosteroids, which relieved their symptoms and diminished the clinical signs. The bilateral iridocyclitis found in both patients may have been secondary to immune complex deposition in the eye.

Thirteen patients with moderately severe ophthalmopathy due to Graves' disease were treated by steroid therapy, supervoltage orbital radiotherapy, plasmapheresis and thyroxine replacement. All patients complained of chemosis, ocular pain and diplopia. The mean value of proptosis was 21.3 mm. Eleven patients (84.6%) showed some improvement, but the effect was mostly on the symptoms of orbital soft tissue involvement. The effect on proptosis was rather unsatisfactory. Fifty hospitalized patients with moderate to severe ocular pain after retinal surgery were treated with the new, potent, oral analgesic, propiram fumarate. Most patients received total daily doses of 50 to 100 mg for one or two days. Pain relief obtained was excellent or good in 92.7% of the observations. Patient and physician acceptance was 96% and 92%, respectively. Only one side effect was seen. Our overall experience was propiram was favorable.

Three patients with leukemia developed corneal toxicity while receiving high doses (3 g/m2) of systemic cytarabine. Symptoms began five to seven days after initiation of treatment with high doses of systemic cytarabine and consisted of ocular pain, tearing, foreign-body sensation, photophobia, and blurred vision. All three patients developed bilateral conjunctival hyperemia and fine corneal epithelial opacities and refractile microcysts that were more numerous in the central than in the peripheral cornea. We used graded radiofrequency heating to destroy pain fibers in the ophthalmic division of the trigeminal ganglion while sparing most of the touch fibers in nine patients (seven men and two women, ranging in age from 51 to 74 years) with chronic ocular pain. All nine experienced immediate and complete pain relief. Although one patient required a second procedure after 57 months, the other eight have been pain-free for 44 to 59 months. There were no serious complications. The corneal reflex was preserved and there was no damage to the optic or extraocular muscles.

Electrical stimulation in the treatment of postoperative ophthalmologic pain was applied in 25 patients. In 21, stimulation was applied by means of subcutaneously implanted wires and in four patients the therapy was administered by externally placed electrodes. The subcutaneous electrodes were introduced near the supraorbital and infraorbital nerves at the end of the surgical procedures, and stimulation was immediately begun. Eye care professionals can play an important role in the early detection of multiple sclerosis (MS) since the disease often has ocular manifestations such as optic neuritis. In conjunction with optic neuritis, symptoms such as ocular pain, acuity reduction, visual field loss, blurred or edematous discs, nystagmus, and diplopia may be present. Although MS still has no cure, early identification can aid in beginning the treatment of the symptoms.

Sixteen patients whose chief complaint was ocular or periocular pain were diagnosed as having occipital neuritis. All but one had significant relief of pain from one or more injections of an anesthetic in the vicinity of the greater occipital nerve. They were subsequently treated with anti-inflammatory measures. From these cases definitions of significant tenderness and success of treatment were developed. These observations form a basis for a current double-blind study designed to establish a relationship between ocular pain and occipital neuritis.

Thirty-six patients with benign intracranial hypertension (BIH) were reviewed. Follow-up was obtained on 33 patients (91%) after a mean period of 7 1/2 years. Precipitating factors were found in 27 patients (75%). On admission, 5 patients had retro-ocular pain, especially on eye movements, a complaint not yet described in BIH. The diagnosis of rhinocerebral mucormycosis is most often made at autopsy. We report a series of nine patients in whom the diagnosis was established premortem. Two patients developed Mooren's ulcer following cataract extraction. The first case was complicated by ocular trauma 8 years prior to surgery and by vitreous loss at the time of surgery. We followed 210 cases of juvenile rheumatoid arthritis closely for eleven years. Thirty-six of the 210 patients (17.2%) developed iridocyclitis. Iridocyclitis was seen most frequently in young female patients (0 to 4 years) with the monoarticular or pauciatricular form of the arthritis.
