Tolosa-Hunt syndrome and orbital myositis have common features such as ocular pain, ophthalmoplegia and exophthalmos. Both syndromes are thought to be caused by a granulomatous inflammation involving the cavernous sinus area in the former and the orbital cavity in the latter. The question whether these two conditions represent different presentations of a single disease, or they belong to different entities has not been settled. We studied the effects of intra-arterial chemotherapy (IAC) with a new nitrosourea (hydroxyethyl-chloroethyl nitrosourea: HeCNU) on the visual system of 68 patients with malignant gliomas. The intra-arterial chemotherapy was given as a complementary treatment of glioma after surgery (19 patients), after tumor recurrence (28 patients) and as the preliminary treatment before radiotherapy (21 patients). We studied the prevalence of migraine in low-tension glaucoma (LTG) and primary open-angle glaucoma (POAG). Seventy seven Japanese patients with LTG, 73 with POAG, and 75 normal subjects were randomly selected and tested with a headache questionnaire. Endophthalmitis has typically been described as occurring within the first 24-48 hours postoperatively and having a rapid progression. Classical signs and symptoms include intense ocular pain, decreased visual acuity, and exaggerated inflammatory response not commensurate with the level of the patient's recuperative process. In 101 eyes with either anterior or retrobulbar optic neuropathy of ischemic or inflammatory origin, ocular pain occurred significantly more often with retrobulbar optic neuropathy. The association of pain with posterior optic nerve lesions supports Whitnall's hypothesis that the pain of optic nerve inflammation is caused by traction of the origins of the superior and medial recti on the optic nerve sheath at the orbital apex. Eye pain reflected neither severity nor origin of optic neuropathy.

Alkali burns are more common in dogs than acid burns. The sources of alkali in this study were soap, cement, and mortar dust. Noncontact transscleral Nd:YAG laser thermal mode cyclocoagulation is known to reduce intraocular pressure (IOP). Thirty-five eyes were treated at a mean of 2.5 Joules and followed for a mean of 28 months. Eighteen patients with acute optic neuritis underwent optic nerve magnetic resonance imaging (MRI) before and after injection of gadolinium-diethylene triamine pentacetic acid (Gd-DTPA). We examined a 67-year-old man with autosomal-dominant polycystic kidney disease who had right ocular pain and visual loss. Escherichia coli was grown from the vitreous aspirate. A case of haematogenous Serratia marcescens endophthalmitis in an HIV-infected intravenous drug addict is described. The patient was admitted with fever, ocular pain and visual loss in the right eye following an i.v. injection of pulverized buprenorphine. A vitreous humor culture grew S. marcescens. Combined penetrating keratoplasty was performed on 10 cases of complicated bullous keratopathy and followed up 6 months to 3 years with satisfactory results in that (1) the extreme ocular pain was totally eliminated; (2) 8 of the 10 grafts remained crystal clear, and (3) the corrected visual acuity in 5 of the 10 patients was improved from preoperative hand motion or finger counting to 0.1-0.8. Therefore, the author deemed the combined penetrating keratoplasty the treatment of choice for bullous keratopathy.

A 34-year-old man in good general health became bilaterally totally blind by an unusual retinochoroidal degenerative disease 3 years after the initial onset of visual symptoms. The advanced stages of the retinochoroidal disease were associated with bilateral progressive iris necrosis and severe ocular pain. The constellation of findings and course of this disease represent a clinical entity that has not been previously described in the literature.

We studied 39 blind painful eyes in 39 patients who were treated with retrobulbar injection of absolute (96%) alcohol for their severe ocular pain at the King Khaled Eye Specialist Hospital from January 1984 to January 1987.
