We operated on 59 eyes with advanced neovascular glaucoma (37 eyes in diabetic patients and 22 eyes in patients with central retinal vein occlusion) using a trabeculectomy technique that includes a simple silicone tube as the draining element. Mean (+/- 1 S.D.) intraocular pressure preoperatively was 57 +/- 8.7 mm Hg. During follow-up periods ranging from one to five years, the mean intraocular pressure decreased to 27 +/- 16.5 mm Hg. We examined the characteristics of ocular pain induced by green argon laser photocoagulation in 13 eyes of ten patients. Three cases of neovascular glaucoma associated with ocular hypoperfusion secondary to carotid artery disease are reported. In two cases, anastomosis of the internal and external carotid artery systems via a superficial temporal artery to the middle cerebral artery (Yasargil procedure) resulted in resolution of the rubeosis, relief of the ocular pain and improvement of the circulation of the globe. The third case, which did not have this procedure, resulted in a blind eye with severe rubeotic glaucoma.

A patient with refractory acute myelogenous leukemia was treated with high-dose cytosine arabinoside (3.0 g/m2 every 12 hours). Three aphakic male patients underwent successful scleral buckling surgery for unilateral rhegmatogenous retinal detachment. Within a two-week postoperative period, each developed severe ocular pain with massive exudative retinal and choroidal detachments resembling recurrent rhegmatogenous or traction retinal detachment or implant infection. Eight patients with chronic open-angle glaucoma treated regularly with different drugs (mainly timolol) and having a mean IOP of 20,4+/-1,2 mm Hg were subsequently treated for 9 months with Suprexon eye drops (guanethidine 1% + epinephrine 0,2%). The clinical features of a patient with atypical band keratopathy are described. Histochemical and electron probe analyses indicate that the granular deposits in Bowman's layer contain calcium and phosphate. An unusual feature in this patient was the presence of severe keratocyte degeneration; its possible role in the pathogenesis of this condition is discussed. Exfoliation of the calcified Bowman's layer appears to have been the basis for severe attacks of recurrent ocular pain.

Two patients, a 48-year-old woman and a 56-year-old man, had hypocomplementemic cutaneous vasculitis syndrome. The patients had ocular pain and photophobia and were found to have bilateral recurrent iridocyclitis. We treated the iridocyclitis with cycloplegia and topical corticosteroids, which relieved their symptoms and diminished the clinical signs. The bilateral iridocyclitis found in both patients may have been secondary to immune complex deposition in the eye.

Thirteen patients with moderately severe ophthalmopathy due to Graves' disease were treated by steroid therapy, supervoltage orbital radiotherapy, plasmapheresis and thyroxine replacement. All patients complained of chemosis, ocular pain and diplopia. The mean value of proptosis was 21.3 mm. Fifty hospitalized patients with moderate to severe ocular pain after retinal surgery were treated with the new, potent, oral analgesic, propiram fumarate. Most patients received total daily doses of 50 to 100 mg for one or two days. Pain relief obtained was excellent or good in 92.7% of the observations. Patient and physician acceptance was 96% and 92%, respectively. Only one side effect was seen. Our overall experience was propiram was favorable.

Three patients with leukemia developed corneal toxicity while receiving high doses (3 g/m2) of systemic cytarabine. We used graded radiofrequency heating to destroy pain fibers in the ophthalmic division of the trigeminal ganglion while sparing most of the touch fibers in nine patients (seven men and two women, ranging in age from 51 to 74 years) with chronic ocular pain. All nine experienced immediate and complete pain relief. Although one patient required a second procedure after 57 months, the other eight have been pain-free for 44 to 59 months. There were no serious complications. The corneal reflex was preserved and there was no damage to the optic or extraocular muscles.
