Relapsing fever in children--demographic, social and clinical features.

Louse-borne relapsing fever (LBRF) is an acute febrile illness endemic Ethiopia. To date reports of childhood LBRF are few. The demographic, social and clinical features of eighty children with LBRF admitted to Ethio-Swedish Children's Hospital, Addis Abeba between 1989 and 1991 is presented. The mean age of patients was 8.8 years (range 4 months to 15 years). The male to female ratio was 1.2:1. Seventy-seven (97%) patients came from Addis Abeba. They came from poor families living in overcrowded homes. Fever, headache, right upper quadrant pain, chills and rigors were common symptoms. Fever a

Pathophysiology of the Lyme disease spirochete, Borrelia burgdorferi, in ixodid ticks.

The pathophysiology of Borrelia burgdorferi, the Lyme disease spirochete, is unique in tick/vector relationships, differing substantially from that of other spirochetes, e.g., Borrelia duttonii, the agent of tick-borne relapsing fever, and Borrelia recurrentis, the agent of louse-borne relapsing fever, in their respective vectors. Following ingestion by a tick, B. burgdorferi lodges in the midgut diverticula, in some instances penetrating the gut wall and invading various tissues. Certain investigators suggest that transmission of the spirochete occurs via infectious saliva, although, in light

Clinical presentation of louse-borne relapsing fever among Ethiopian refugees in northern Somalia.

Louse-borne relapsing fever (LBRF) is still endemic among Ethiopian populations. In order to assess the clinical presentation of LBRF in an Ethiopian refugee camp in northern Somalia, a referral system was organized for all pyrexias of unknown origin. Among the 134 patients referred, 37 showed Borrelia in fresh and stained blood smears. Common clinical features were: high fever (above 39 degrees C in 73% of the cases), headache and general body pain (88%), liver tenderness (62%), petechia (54%), nausea and vomiting (46%), chills and rigors (30%) and epistaxis (11%). Jaundice was absent. No fat

Cross-reactivity in serological tests for Lyme disease and other spirochetal infections.

Serum specimens from 163 persons with Lyme disease, tick-borne or louse-borne relapsing fever, yaws, syphilis, leptospirosis, or Rocky Mountain spotted fever were analyzed to assess the specificity of indirect fluorescent antibody (IFA) tests, an enzyme-linked immunosorbent assay (ELISA), and microscopic agglutination (MA) procedures. Strong cross-reactivity occurred when sera from individuals with Lyme disease, tick-borne relapsing fever, and louse-borne relapsing fever were tested against heterologous Borrelia antigens. Antibodies to Borrelia burgdorferi bound to Treponema pallidum in immuno

Glucose and insulin homeostasis during the Jarisch-Herxheimer reaction.

Plasma concentrations of glucose and insulin were measured in ten patients during the Jarisch-Herxheimer reaction of tetracycline-treated louse-borne relapsing fever. Plasma glucose fell significantly in eight of the ten patients associated with the peak of the reaction, but plasma insulin remained low. Glucoregulation by insulin was therefore normal. This evidence questions the role of macrophage mediator-induced pancreatic insulin release in causing hypoglycaemia in borreliosis or bacterial endotoxicosis.

Meptazinol diminishes the Jarisch-Herxheimer reaction of relapsing fever.

Naloxone, an opioid antagonist, and meptazinol, an opioid antagonist with agonist properties, were tested in a double-blind placebo-controlled trial in 24 Ethiopian patients with louse-borne relapsing fever. The potentially fatal Jarisch-Herxheimer reaction (J-HR), which invariably follows tetracycline treatment of the disease, was unaffected by naloxone, 30-40 mg intravenously, but was diminished by meptazinol, 300-500 mg intravenously. Compared with naloxone and placebo, meptazinol reduced the clinical severity of the reaction, significantly delayed and shortened its chill phase, delayed the

Pathophysiology and immunology of the Jarisch-Herxheimer-like reaction in louse-borne relapsing fever: comparison of tetracycline and slow-release penicillin.

Twelve men with louse-borne relapsing fever were treated with single doses of procaine penicillin plus aluminum monostearate (PAM) intramuscularly or of tetracycline intravenously. All patients experienced a definite Jarisch-Herxheimer-like reaction. Fever and spirochetemia were significantly prolonged and peak temperature was lower and occurred later in the PAM-treated group. Peak pulmonary ventilation, metabolic rate, and arterial PO2 were significantly higher in the tetracycline-treated group. Circulatory changes were similar in the two groups but were prolonged in the PAM-treated patients.

Antibiotic treatment of louse-borne relapsing fever in Ethiopia: a report of 377 cases.

Single-dose non-random, antibiotic treatment was evaluated in 377 Ethiopians with louse-borne relapsing fever. Oral doses of tetracycline hydrochloride 500 mg, doxycycline 100 mg, erythromycin base 500 mg, chloramphenicol 500 mg, or a single intramuscular injection of 1,200,000 units of penicillin aluminum monostearate (PAM) were equally effective treatments. All drugs induced a Jarisch-Herxheimer reaction, which was clinically less severe in patients given PAM. The duration of spirochetemia after treatment was much longer after PAM treatment, however. Three critically ill patients died shortl

Multifocal neuropathy and vocal cord paralysis in relapsing fever.

A 19-year-old soldier developed multifocal neuropathy following an infection with tick-borne Borrelia. He suffered from involvement of the right accessory nerve, the right brachial plexus and the left recurrent laryngeal nerve. As was expected, all these complications were reversible. In contrast to louse-borne relapsing fever, neurological manifestations in the tick-borne variants are not common. To the best of our knowledge, vocal cord paralysis has never been described before.

Louse-borne relapsing fever in the Sudan. A historical review and a clinico-pathological study.

A small outbreak of louse-borne relapsing fever in Khartoum (May-June 1974) provided material for a clinico-pathological study. The history of the disease in the Sudan is reviewed and the clinical and laboratory findings in 32 patients are presented. Fever, headache, jaundice, epistaxis and hepatosplenomegaly were the commonest clinical findings; thrombocytopenia was detected in 93% of cases. Although elevated levels of fibrin degradation products were found in most patients, disseminated intravascular coagulation could not be diagnosed. Hepatocellular derangement was found in 68% of cases, wh

Louse-borne relapsing fever: I. A clinical and laboratory study of 363 cases in the Sudan.

Louse-borne relapsing fever seems to have become endemic in the southern Sudan. The epidemic history of the disease in the Sudan is reviewed. We have studied 363 Sudanese patients involved in an outbreak of louse-borne relapsing fever in Khartoum (Sudan) between January and June 1974. 318 of the 363 patients were new immigrants from the soughern Sudan to Khartoum. The clinical presentation varied. The common clinical fetures of the disease were: fever (94%), headache (85%), hepatosplenomegaly (74%), body and joint pains (66%), abdominal pain and tenderness (63%), jaundice (46%) and epistaxis (

Louse-borne relapsing fever: II. Combined penicillin and tetracycline therapy in 160 Sudanese patients.

One hundred and sixty patients with louse-borne relapsing fever were treated with a combination therapy of procaine penicillin and tetracycline. Fortified procaine penicillin B.P. was given as 400,000 units in the first day. This was followed the next day by 2 g tetracycline orally in divided doses for seven days. A mild rise in temperature was observed in 22 (13-7%) patients within four hours of administering penicillin. Rigors and hypotension occurred in one patient. Relapse occurred in two patients, of whom one had concomitant typhoid and one visceral leishmaniasis. The combined therapy has

Clinical pathology of the Jarisch-Herxheimer reaction.

The Jarisch-Herxheimer reaction is a complication that can follow treatment of several infectious diseases. Its most severe form is in louse-borne relapsing fever; in this syndrome the reaction can cause death. Information from studies in Ethiopia during the past eight years is presented, and clinical, physiological, pathological, and immunological features of the reaction are described. Possible causative mechanisms of the reaction are discussed, especially in relation to the role of endotoxin, and an attempt is made to consider this reaction in relation to other endotoxin-associated states.

Experimental louse-borne relapsing fever in the grivet monkey, Cercopithecus aethiops. I. Clinical course.

Fifteen grivet monkeys, Cercopithecus aethiops, were infected with an Ethiopian strain of Borrelia recurrentis, the causative agent of louse-borne relapsing fever. An initial spirochetemia occurred in all. Inactivity, fever, and leukocytosis accompanied the infections. Eight of the monkeys experienced 1 relapse and 1 monkey had 2 relapses. The relapses tended to be less severe than the initial infections. The only deaths, however, occurred in 2 of the 3 monkeys with severe relapses. The clinical course of the experimental infection in grivet monkeys closely resembles that of natural infections

Nineteen grivet monkeys, Cercopithecus aethiops, were infected with the spirochete of louse-borne relapsing fever, Borrelia recurrentis, and killed at various intervals following inoculation: 4 during the initial spirochetemia; 4 during remission; 3 during relapse; 2 that were dying following severe relapses; and 6 during convalescence. The histologic changes of histiocytic myocarditis, multiple microabscesses replacing the nodular white pulp of the spleen, and hepatitis with foci of midzonal necrosis were similar to those seen in humans. These changes are rarely seen in other infectious disea

Experimental louse-borne relapsing fever in the grivet monkey, Cercopithecus aethiops. III. Crisis following therapy.

Eight grivet monkeys infected with Borrelia recurrentis received tetracycline (12.5mg/kg body weight) on the 3rd or 4th day of spirochetemia. Leukopenia, fever, hyperpnea, and tachycardia developed within 2 hours as spirochete counts fell to undetectable levels. These events closely simulated the crisis in human louse-borne relapsing fever in both timing and extent.
