Unsuccessful Healing of Tympanic Membrane Perforations Following Myringoplasty-A Nationwide Retrospective Study.

INTRODUCTION: Risk factors for unsuccessful healing of a tympanic membrane perforation following myringoplasty lack consensus. Large cohort studies are missing. METHODS: Data were collected for myringoplasties (with/without concomitant ossiculoplasty) performed the years 2014-2019 with a follow-up visit using the Swedish Quality Register for Ear Surgery (SwedEar). Risk factors for unsuccessful surgical outcome, defined as non-healed tympanic membrane at follow-up, were calculated using logistic regression. RESULTS: The study population included 3622 cases and 12.8% had a non-healed tympanic

Executive Summary of the Clinical Practice Guideline on Tinnitus Update.

OBJECTIVE: Tinnitus is the perception of sound without an external source. More than 50 million people in the United States have reported experiencing tinnitus, resulting in an estimated prevalence of 10% to 15% in adults. The focus of this guideline is on tinnitus that is both bothersome and persistent (lasting 6 months or longer), which often negatively affects the patient's quality of life. The target audience for the guideline is any clinician, including allied health professionals, involved in managing patients with tinnitus. The target patient population is limited to adults (18 years an

Determining the Impact of Race and Ethnicity on Diagnosis and Management of Vestibular Schwannoma.

OBJECTIVE: While prior literature has demonstrated that patients of lower socioeconomic groups may have larger tumors on presentation and poorer outcomes, there is limited literature on the impact of race and ethnicity on management of vestibular schwannoma (VS). The objective of this study was to determine whether race and ethnicity impacted treatment choice and postoperative management of VS. METHODS: Retrospective review of patients diagnosed with VS (ICD9 225.1, ICD10 D33.3, and ICD 10 H93.3) who presented to a tertiary care center between January 1, 2018 and October 31, 2023 was complete

Clinical management of vestibular schwannoma - what's the evidence?

BACKGROUND: Clinical management of vestibular schwannomas (VS) encompasses three main strategies: observation ("wait-and-scan"), stereotactic radiosurgery, and microsurgery. Despite extensive literature, a consensus on the optimal modality is lacking due to the scarcity of high-level evidence. METHODS: This narrative review explores the current understanding of the most significant clinical and radiographic outcomes following wait-and-scan, radiosurgery, and microsurgery. This is not a systematic review of the literature. RESULTS: Observational studies indicate that approximately one-third o

Primary Renal Glomus Tumor of Uncertain Malignant Potential: A Rare Case Report and Literature Review.

Renal glomus tumor (GT) is a rare neoplasm originating from glomus cells within the kidney. The subtype designated as uncertain malignant potential (UMP) is particularly rare, with limited literature available. Its clinical presentation and imaging features closely resemble those of renal cell carcinoma (RCC), making accurate preoperative diagnosis difficult. This case report aims to describe the clinical, radiological, and pathological features of a renal GT of UMP and to summarize previously reported cases to improve recognition of this rare entity. We report a case involving a 36-year-old m

Petrous apex metastasis revealing prostate cancer: a rare case report and review of literature.

INTRODUCTION AND IMPORTANCE: Temporal bone malignancies are rare, representing only 0.2% of head and neck malignancies, with an annual incidence of 1 to 6 cases per million. Metastatic involvement of the petrous apex is particularly uncommon, with prostate cancer accounting for approximately 8.6% of temporal bone metastases. We report an extremely rare case of metastatic prostate cancer presenting with otologic symptoms and a destructive petrous apex lesion in a patient with no prior oncological history. CASE PRESENTATION: A 73-year-old male presented with right-sided tinnitus, vertigo, and h

Papilledema as the Initial Manifestation of Styloidogenic Jugular Venous Compression Syndrome.

Styloidogenic jugular venous compression syndrome (SJVCS) is a rare variant of Eagle Syndrome caused by compression of the internal jugular vein (IJV) by an elongated styloid process. We describe a 47-year-old woman in whom papilledema was incidentally detected during a routine optometric examination. Initial neuroimaging did not adequately image the jugular veins in the neck but demonstrated unremarkable intracranial appearances. The patient remained asymptomatic with a normal lumbar puncture opening pressure. Over 3 years, the papilledema gradually worsened in the absence of symptoms of rais

The impact of a psychedelic drug on olfactory search behavior by mice.

Animals use their sense of smell for survival and well-being. Any disruption to olfactory perception can lead to devastating consequences and decreased quality of life. Psychedelics alter perception in humans, yet little is known about the impact of psychedelics on olfactory behaviors. Using an olfactory search task, we investigated the impact of the psychedelic 2,5-dimethoxy-4-iodoamphetamine (DOI) on olfactory search behavior of mice. We found that DOI decreases search accuracy, alters movement, and increases sniff rate. These findings suggest that DOI disrupts olfactory-guided behavior, elu

A population code for semantics in human hippocampus.

Motivated by the successes of large language models (LLMs), we hypothesized that semantic information in the brain may be expressed in distributed patterns of neural activity. We recorded responses of hippocampal neurons while individuals listened to narrative speech. Here, using an encoding model approach, we find robust encoding of semantics, even after controlling for phonemics and grammar. Individual words had complex selectivities that included multiple words in multiple semantic categories. Similar to embedding vectors in LLMs, distance between neural population responses correlated with

The Role of A Surgeon in Restoring the Senses & in Biomedical Engineering.

This review is based on my 2025 Jepson Lecture and outlines my personal journey to restore hearing in people with a severe sensorineural hearing loss. My dream started as a 5-year-old as I told my kindergarten teacher when I grow up 'I want to fix ears' because my father is deaf. The journey resulted in a career in surgery followed by auditory neuroscience, engineering and audiology. While the key stages in the development of the multi-channel cochlear implant are highlighted, their importance in discovering the first restoration of a human sense, and in understanding brain function is present

Differential cross-modal functional plasticity in deaf individuals: A meta-analysis of neuroimaging studies.

A manifest condition of brain plasticity is cross-modal plasticity in deafness, wherein auditory-deprived cortices are recruited by intact sensory modalities. While cross-modal plasticity is well documented, the underlying principles remain debated, centering on the functional change and functional preservation principles. To quantitatively evaluate these principles, we conducted the largest coordinate-based meta-analysis to date, synthesizing 47 studies (801 deaf and 880 hearing participants) spanning linguistic and non-linguistic tasks. Deaf individuals exhibited consistent cross-modal activ

Auditory- and Visual-Speech Evoked Activation of the Superior Temporal Gyrus Contralateral to the Cochlear Implant Predicts Speech Recognition Outcomes.

OBJECTIVES: Speech recognition outcomes vary widely in cochlear implant recipients, but the reasons for this variability are not well explained by patient history factors alone. Neuroplastic reorganization after both deafness and cochlear implant switch-on can affect how new recipients adapt to electrical hearing. In this study, we evaluate the associations between speech recognition outcomes and stimulus-evoked activation of cortical language processing areas. DESIGN: Forty-three newly implanted post-lingually deaf adult cochlear implant recipients (17 female, mean age at implantation: 58.5

[Methodological implications of the minimal clinically important difference in patient-reported outcome measures in the field of cochlear implant provision. German version].

To determine the minimal clinically important difference (MCID), patient-reported outcome measures (PROMs) are presented that cover the dimensions 1. subjective hearing ability, 2. hearing-specific quality of life, and 3. generic quality of life, which are largely mentioned in corresponding guidelines and recommendations for device provision and aural rehabilitation in Germany. In the absence of recognized MCID thresholds in the field of cochlear implant (CI) provision in Germany, a methodological framework for future clinical studies was developed based on a two-stage anchor-based retrospecti

Viewpoint on Crossmodal Neural Plasticity.

Crossmodal neural plasticity, the brain's ability to adapt in response to sensory deprivation, serves as a primary model for understanding the multifaceted interplay between intrinsic neurobiological constraints and environmental experience in brain development. In this viewpoint, Olivier Collignon and Ella Striem-Amit engage in a collaborative dialogue exploring the fundamental principles of brain organization in individuals born with blindness, deafness, or without hands. The discussion evaluates current shifts in the field, including the move toward high-resolution neuroimaging, deep data a

Case Report: Streptococcus suis-associated meningoencephalitis with profound bilateral labyrinthine deafness and intra-operative cochlear fibrosis treated by cochlear implantation.

Streptococcus suis is a common zoonotic pathogen that usually causes monomicrobial meningitis. Occult bloodstream detection of Klebsiella pneumoniae, together with proven central-nervous-system S. suis infection, followed by irreversible cochlear fibrotic changes requiring cochlear implantation, appears exceptionally rare. We report a 52-year-old male with chronic heavy alcohol intake and occupational swine-exposure history who developed severe sepsis, recurrent suppurative meningoencephalitis caused by S. suis, and permanent bilateral labyrinthine-related deafness. Automated Question-and-Answer-Based Speech-In-Noise Hearing Test Using a Large Language Model.

A conversational speech-in-noise test administered without human supervision is desirable to reduce the need to attend the clinic. This study aimed to create and evaluate a novel question-and-answer-based speech-in-noise test called 'CHAT', where stimuli were synthesised using text-to-speech (TTS), responses captured using automatic speech recognition (ASR), and responses evaluated using a large language model (LLM). Three tests were compared: the newly developed CHAT, the Adaptive Sentence List (ASL) and the Four Alternative Auditory Feature Test (FAAF). CHAT used synthetic speech while ASL a
