Prestin as a potential biomarker in peripheral blood: differential expression across hearing loss subtypes and acute inner ear injury.

OBJECTIVE: This exploratory study investigated peripheral blood Prestin levels across different hearing loss subtypes to evaluate its potential as a biomarker for inner ear injury. METHODS: Plasma Prestin levels were measured via ELISA in 100 age-matched participants, comprising a healthy control group (HCG, n = 23) and patients with conductive (CHLG, n = 29) and unilateral sudden sensorineural hearing loss (USSNHLG, n = 48). Associations with hearing loss subtypes, initial audiogram configurations, and treatment outcomes were analyzed. RESULTS: Overall, Prestin levels varied significantly a

Association between ankylosing spondylitis and hearing loss: A Mendelian randomization study.

Observational studies have suggested a potential link between ankylosing spondylitis (AS) and hearing loss. However, this association remains controversial due to potential drug-induced ototoxicity from treatments. Therefore, we employed a Mendelian randomization (MR) approach to evaluate the association between AS and hearing loss from a genetic perspective. This MR study utilized summary data from genome-wide association studies on AS and hearing loss, including sensorineural hearing loss (SNHL) and conductive hearing loss (CHL). The primary analysis was conducted using the inverse-variance

Ocular Vestibular Evoked Myogenic Potential Response Patterns for Characterizing Vestibular Dysfunction in Peripheral Vestibular Disorders.

Ocular vestibular evoked myogenic potential (oVEMP) testing provides electrophysiological information on otolithic and superior vestibular pathway function, but its clinical interpretation often remains limited to a binary normal-or-abnormal judgment. This study developed and applied a structured oVEMP response-pattern protocol, together with an exploratory, study-specific composite abnormality-burden grade, to characterize vestibular dysfunction in patients with peripheral vestibular disorders. From February 2024 to December 2025, 110 adults with Ménière's disease, benign paroxysmal positiona

Beyond Excitotoxicity: Glutamate-Independent Mechanisms Underlie Noise-Induced Synaptopathy and Compromise Gene Therapy for Hereditary Deafness.

Noise-induced hearing loss (NIHL) is classically attributed to glutamate excitotoxicity, yet the precise requirement for synaptic glutamate release remains untested using genetic approaches. Here, we investigated noise-induced cochlear damage using Vglut3 knockout (Vglut3-KO) mice, which lack glutamatergic transmission from inner hair cells. Following 116 dB SPL noise exposure, wild-type (WT) mice exhibited permanent threshold shifts, persistent ABR wave I amplitude reduction, and significant loss of both ribbon synapses and outer hair cells (OHC) in the basal turn. Unexpectedly, Vglut3-KO mic

The cochlear macrophage in presbycusis: inflammaging, neuroinflammation, and targets for therapeutic modulation.

Age-related neurodegenerative diseases and age-related hearing loss are increasing in prevalence globally. Immune cells are novel mediators linking these diseases through shared mechanisms of chronic inflammation and maladaptive repair. In the cochlea, resident macrophages and infiltrating immune cells respond to injury, noise exposure, and aging by adopting activated phenotypes that disrupt synaptic integrity, impair myelination, and promote hair cell and neuronal loss. Similarly, in the central nervous system, microglia, the resident immune cells of the brain undergo age- and disease-associa

Clinical practice guidance for imaging selection in pulsatile tinnitus: an evidence-based expert consensus.

Pulsatile tinnitus (PT) is a rhythmic sound synchronized with the patient's heartbeat that represents a complex diagnostic challenge with a broad spectrum of vascular and non-vascular etiologies. Persistent symptoms can severely impact mental health, yet a specific causative abnormality can be identified via targeted radiological imaging in more than 75% of cases. Improper imaging selection frequently leads to misdiagnosis, reduced therapeutic efficacy, and increased healthcare costs. The choice of imaging protocol must be guided by a thorough initial clinical evaluation, including detailed me

Not All Transverse Sinus Stenoses Behave the Same: Distinct Flow Phenotypes on Phase-Contrast MRI.

Background Transverse sinus stenosis is usually assessed by its anatomical appearance, although stenoses with similar morphology may not have the same local flow behavior. This study evaluated peak velocity changes across transverse sinus stenoses using phase-contrast magnetic resonance imaging (MRI) and explored whether distinct hemodynamic patterns could be identified. Methods We retrospectively reviewed 183 consecutive patients who underwent brain MRI, magnetic resonance venography, and venous phase-contrast MRI for headache, vertigo, and/or tinnitus between September 2025 and June 2026. Emerging Therapies for Genetic Deafness: The Rise of Precision Gene Editing.

Hearing loss is a prevalent sensory disorder with profound social and health impacts. Although hearing aids and cochlear implants can significantly restore auditory perception, they do not address the underlying genetic causes. Inherited forms of hearing loss, often monogenic, are ideal candidates for gene therapy, particularly since over 150 genes have been linked to deafness. Advances in vector engineering and genome editing technologies now enable precise targeting of auditory cells within the inner ear. Technologies such as Clustered Regularly Interspaced Short Palindromic Repeats-Cas syst

Cellular and Tissue Mechanisms of Laser-Assisted Regeneration of Tympanic Membrane Perforation.

Chronic tympanic membrane (TM) perforations remain a significant clinical problem due to limited spontaneous healing and the drawbacks of surgical interventions. The present study aimed to evaluate the effects of low-intensity infrared laser irradiation on cellular mechanisms of regeneration and to assess its therapeutic potential in TM repair. Primary fibroblast cultures isolated from chinchilla tissue were exposed to laser radiation at wavelengths 1560 nm and 1450 nm. Cellular responses were analyzed using MTT assay, scratch test, Ki-67 immunostaining, and ELISA for collagen type II. In addi

Case Report: Olfactory loss as the sentinel manifestation of type 2 chronic rhinosinusitis without polyps: response to IL-4Rα inhibition with stapokibart - a case series.

Chronic rhinosinusitis without nasal polyps (CRSsNP) is traditionally associated with type 1 inflammation, but emerging evidence reveals a subset of patients with a type 2 endotype driven by IL-4 and IL-13, often presenting with prominent olfactory dysfunction and responding poorly to conventional therapy. We report three cases of type 2 CRSsNP with comorbid allergic rhinitis (aged 29-30 years, two males, one female) who presented with debilitating olfactory loss as the sentinel symptom. All had elevated type 2 biomarkers, absence of nasal polyps, and CT evidence of sinusitis. After failure of

Dietary Zinc Glycine Supplementation Attenuates Aging-Driven Cochlear Hair Cell Oxidative Injury and Hearing Loss.

Background: Micronutrient imbalance is now recognized as a modifiable risk factor driving the progression of age-related hearing loss (ARHL). Zinc, an indispensable dietary trace nutrient, acts as a structural cofactor and transcriptional regulator of a broad panel of antioxidant enzymes to sustain systemic and cellular antioxidant defense capacity. Nevertheless, the therapeutic value of zinc nutritional supplementation and its precise molecular mechanisms against age-dependent cochlear degeneration remain largely uncharacterized. Methods: We established aging mouse models to observe the pheno

Combined Bioinformatic Analysis and RNA Sequencing Uncover Key Tinnitus-Related Molecules in the Cingulate Gyrus.

Tinnitus arises from maladaptive plasticity and abnormal synaptic function, not only in auditory pathways but also in limbic regions. Among these, the cingulate gyrus plays a pivotal part in shaping the emotional burden and sensory filtering disturbances often seen in tinnitus patients. Yet, the specific molecular players operating within this region remain poorly understood, and most existing bioinformatic predictions have not been tested in living systems. Here, we combined multi-database mining with in vivo transcriptomic profiling to identify and validate key proteins and pathways linked t

Vestibular Event Monitoring in the Community and Emergency Room.

Patients who are experiencing an attack of acute vertigo often demonstrate nystagmus that could provide useful diagnostic clues that identify the underlying vestibular disorder. Such ictal nystagmus is encountered in primary care, emergency rooms and patients' home environments where methods of recording nystagmus (video nystagmography) are scarce. Investigators worldwide have endeavored to circumvent this difficulty by using novel service models or portable nystagmus recording devices. Here we describe the event-monitoring techniques used thus far and summarize the diagnostic information gath

Epithelial-mesenchymal Wnt crosstalk directs planar cell polarity in the developing cochlea.

Cochlear hair cells are precisely oriented by planar cell polarity. Wnt proteins direct planar cell polarity in vertebrates, but their roles in regulating cochlear planar cell polarity remain unclear. Here, we inhibit Wnt secretion by ablating epithelial Wntless and find shortened cochlea, mild hair cell misorientation and mislocalization of core planar cell polarity proteins, Fzd6 and Dvl2. Computational analysis predicts that epithelial Wnts (Wnt5a, Wnt7a, Wnt7b) and Wnt5a from surrounding periotic mesenchyme act on developing hair cells. Deletion of epithelial Wnt5a, Wnt7a, and Wnt7b additi
