Type 2 Diabetes Mellitus With Complex Necrotizing Otitis Externa, Skull Base Osteomyelitis, and Cranial Nerve Palsies: A Case Report.

Necrotizing otitis externa (NOE) is a severe infection of the external auditory canal that can extend to the skull base. We report the case of a 67-year-old gentleman with insulin-dependent type 2 diabetes mellitus with suboptimal glycemic control who presented with a likely complex NOE. Initial findings included ciprofloxacin-resistant Pseudomonas aeruginosa, elevated inflammatory markers, and multiple cranial nerve palsies. MRI revealed the presence of granulation tissue at the osseocartilaginous junction. Histology from the right external auditory canal confirmed the presence of necrotizing

Bacterial Agents and Antimicrobial-Resistance Patterns in Canine Otitis Externa.

Canine otitis externa is a common and recurrent ear infection in dogs, often caused by bacterial pathogens, and complicated by increasing antimicrobial resistance. The analysis of bacterial isolates from dogs with otitis externa revealed a predominance of Staphylococcus pseudintermedius, representing 41% of all cases, followed by Staphylococcus aureus (23%), and Pseudomonas aeruginosa (19%). Other less frequently isolated organisms included Escherichia coli, Streptococcus canis, and Proteus mirabilis. These results highlight the significant role of S. pseudintermedius in the pathogenesis of ot

From the Ear to the Brain: Otitis Externa Complicated by Coalescent Mastoiditis Leading to Temporal Lobe Abscess and Wernicke's Aphasia.

Otitis externa is typically a benign infection of the external auditory canal; however, in high-risk individuals, particularly those with poorly controlled diabetes mellitus, it may progress to severe otogenic and intracranial complications. We report a case of a 59-year-old woman with type 2 diabetes and an HbA1c of 14%, reflecting longstanding uncontrolled disease related to medication nonadherence, who developed a temporal lobe abscess and Wernicke's aphasia following inadequately treated otitis externa. At her initial emergency department (ED) visit for left-sided otalgia, otoscopic examin

Comparative efficacy of selenoureido carbonic anhydrase inhibitors and azole antifungal drugs against clinical isolates of Malassezia pachydermatis.

BACKGROUND: Malassezia pachydermatis (MP) is implicated in severe dermatitis and otitis externa (OE) of companion animals and recently gained attention for its increasing resistance to azole compounds. For this reason, developing novel therapeutic strategies is of great interest. In a previous work, we used reference yeast isolates to evaluate several compounds bearing acyl/selenoureido moieties and primary/secondary sulfonamide groups for antifungal activity through organic selenium and carbonic anhydrase inhibition. OBJECTIVES: This work aimed to evaluate the antifungal efficacy of eight se

Otomycosis: a systematic review and meta-analysis of prevalence and causative agents in the era of molecular diagnostics.

BACKGROUND: Molecular identification of fungal agents in otomycosis can provide a more accurate diagnosis and differentiation of species compared to conventional morphological methods. Previous reviews mostly relied on studies using conventional methods to explore the prevalence and distribution of otomycosis etiologies. We aim to estimate the prevalence and distribution of causative agents in otomycosis cases confirmed with molecular methods. METHODS: We conducted a systematic search of PubMed, Scopus, Web of Science, and Google Scholar. We selected cross-sectional studies that reported caus

Emergence of Rhodotorula mucilaginosa among pet animals: a possible public health risk on the move.

Rhodotorulae are environmentally ubiquitous yeasts that were originally considered non-pathogenic. However, over the last fifty years, Rhodotorula mucilaginosa (R. mucilaginosa) has established itself as an emerging opportunistic pathogen incriminated in several systemic and localized infections in humans and animals. Most of the rhodotorulosis cases were linked directly or indirectly to an immunocompromising event in the affected cases. Nevertheless, recently, a few non-immunocompromised cases were reported. In the current study, performed throughout the period from April/2023 to April/2024,

Resolution assessment of super-resolution microscopy imaging: structural and technical dependencies for cell biology.

Angina pectoris is cardiac pain that is a common clinical symptom often resulting from myocardial ischemia. Spinal cord stimulation (SCS) is effective in treating refractory angina pectoris, but its underlying mechanisms have not been fully elucidated. The spinal dorsal horn is the first region of the central nervous system that receives nociceptive information; it is also the target of SCS. In the spinal cord, glial (astrocytes and microglia) activation is involved in the initiation and persistence of chronic pain. Thus, the present study investigated the possible cardiac pain‑relieving effec

Extensive Base of Skull Osteomyelitis: Why Does It Still Occur?

Skull base osteomyelitis (SBO) is a rare yet serious intratemporal infection that often masquerades as a skull base malignancy. It is most common in diabetic and immunocompromised patients. We present a case of an elderly diabetic patient with end-stage renal disease with progressive malignant otitis externa. The disease progressed to involve the base of the skull, causing multiple cranial neuropathies. Early initiation of intravenous (IV) antibiotics, along with supportive treatment, may improve the long-term prognosis of the disease. This case highlights the importance of keeping a high inde
