Borrelia miyamotoi exceeds Borrelia burgdorferi sensu lato in human exposure despite the opposite in ticks: an integrated serological, entomological and molecular study in southeastern Kazakhstan.

Lyme borreliosis (LB) and hard tick-borne relapsing fever caused by Borrelia miyamotoi are emerging tick-borne infections, yet in Kazakhstan routine LB diagnosis relies on single-tier enzyme immunoassay (EIA) without confirmatory testing, and B. miyamotoi is absent from national diagnostic protocols. In 2024-2026, 1300 participants were enrolled in the Ixodes persulcatus-endemic Almaty and Jetisu regions and the non-endemic Zhambyl and Turkistan regions, across diverse clinical and tick-exposure groups. All sera were screened for IgM and IgG to both pathogens by multi-antigen protein arrays, w

Fever from the shadows: Borrelia duttonii in a migrant in Italy.

In a striking diagnostic breakthrough, a Senegalese man arrived at SS. Annunziata Hospital in Chieti gripped by relentless high fever, throbbing headache, vomiting, right hypochondrium tenderness, lower limb myalgias, generalized lymphadenopathy, conjunctival injection, and photophobia-signaling a systemic infection with neurologic red flags. Labs showed thrombocytopenia, elevated CRP, procalcitonin, and indirect hyperbilirubinemia; lumbar puncture was negative. The game-changer came during peak fever: an extraordinarily rare visual spectacle on peripheral blood smear-motile spirochetes "danci

Listen to the Patient-She is Telling You the Diagnosis.

In this narrative, I illustrate the power of long-term primary care, knowing the patient and the land, and recognizing patterns others might miss. A teenage patient presented recurrent fevers and vague symptoms. What first appeared to be a viral illness ultimately revealed itself as tick-borne relapsing fever-"Cave Fever"-a rare but locally known condition. In a setting where relationships matter as much as laboratory results, diagnosis becomes an act of attentive presence. Sometimes, as Osler reminds us, the patient truly is telling us the diagnosis.

Neurological manifestations in a cat with tick-borne relapsing fever caused by Borrelia persica.

Tick-borne relapsing fever is an acute, potentially life-threatening, infectious disease affecting humans and animals. Relapsing fever (RF) caused by Borrelia persica has been reported to cause severe meningitis in humans, but neurological abnormalities associated with it have not been previously reported in cats. A 14-year-old female spayed domestic cat was diagnosed with RF borreliosis. It acutely developed general hyperesthesia and head tremors and deteriorated to having generalized tonic-clonic seizures within eight hours of the initial neurologic signs. These neurological abnormalities re

Serological evidence of Borrelia turicatae in Raccoons (Procyon lotor) from an endemic focus of tick-borne relapsing fever in Mexico.

Tick-borne relapsing fever is a neglected and overlooked disease. In Mexico, numerous historical reports document the distribution of Ornithodoros turicata, the vector of Borrelia turicatae, as well as human cases of infection. However, the enzootic cycle and reservoir hosts in Mexico remain unknown. Here, to detect previous infections with relapsing fever Borreliae in wild fauna a retrospective serological analysis was conducted with serum samples collected from raccoons trapped from 2022 to 2025 in the Navachiste region of Sinaloa, Mexico. Using a species-specific antigen, BipA from B. turic

Genomic and structural analysis of major antigenic proteins' variability in the human relapsing fever spirochete Borrelia persica.

Antigenic variation enables immune evasion of pathogens, yet the mechanisms for the rapid change in their antigens without loss of structural and functional features are not fully known. Tick-borne relapsing fever (TBRF) spirochetes of the genus Borrelia can be used as a model because their variable major proteins (Vmps) drive antigenic variation followed by recurrent waves of bacteremia. In this study we generated the genome of Borrelia persica LMU-C01 and evaluated Vmp structure across relapsing fever Borrelia species. Borrelia persica was found to have a genome of 1.7Mb with one linear chro

Human Exposure to Spirochete-Infected Ornithodoros turicata in a Residential Setting in the Austin, Texas Metropolitan Area.

A central Texas resident found an engorged Ornithodoros turicata tick attached to their ankle while at home. The tick tested positive for Borrelia turicatae, an agent for soft tick-borne relapsing fever. Three weeks earlier, the patient started experiencing febrile episodes, rash, and nausea, yet the etiological agent went undiagnosed.

Defining strain variation in the maintenance of Borrelia turicatae during hyperparasitism and transovarial transmission in the tick vector.

UNLABELLED: Tick-borne relapsing fever (TBRF) spirochetes are maintained in nature through vector-associated transmission routes primarily in argasid ticks. However, the extent to which strain-level differences influence the maintenance of TBRF spirochetes remains unclear. We evaluated two Borrelia turicatae strains, Bt -SSK1 and Bt -FCB, in Ornithodoros turicata . Both strains were acquired by female ticks, and after mating we unexpectedly observed that they were hyperparasitized by uninfected and unfed male O. turicata. Bt - SSK1 was maintained in males while Bt- FCB was not. Transovarial tr

Abundant ticks but limited Borrelia spp. pathogens in Mojave desert tortoise habitat.

Ornithodoros ticks, a genus of soft ticks in the family Argasidae, occur worldwide including throughout deserts in the Southwestern United States. In the Mojave Desert, Ornithodoros parkeri and O. turicata are common ectoparasites of Mojave desert tortoises (Gopherus agassizii). These two tick species are vectors of Borrelia spp. that cause tick-borne relapsing fever (TBRF) in people. However, the distribution of O. parkeri and O. turicata and TBRF group Borrelia infection prevalence remain poorly known in the Mojave Desert. These ticks are commonly found in desert tortoise burrows. In this st

Blood smear examination matters: an incidental case of tick-borne relapsing fever in a traveller returning from Senegal.

Relapsing fever is caused by various species of Borrelia bacteria and is traditionally categorized into louse-borne relapsing fever (LBRF) and tick-borne relapsing fever (TBRF). The epidemiological patterns differ according to the vector: LBRF is typically linked to epidemic outbreaks, while TBRF usually presents with an endemic distribution. Even though TBRF is endemic also in the Mediterranean Basin, diagnosis is mainly incidental in migrants or visiting friends and relatives travelling from sub-Saharan Africa. We report a case of TBRF with aseptic meningitis caused by Borrelia crocidurae/du

First cluster of relapsing fever compatible with louse-borne transmission in southern Saudi Arabia: An eight-case series from Najran.

Louse-borne relapsing fever (LBRF), caused by Borrelia recurrentis, is a neglected vector-borne infection associated with overcrowding, migration, and poor hygiene conditions. We describe eight spatiotemporally linked cases of Borrelia spp. relapsing fever compatible with louse-borne transmission identified in Najran, southern Saudi Arabia. This retrospective case series included eight patients diagnosed between March 2013 and August 2013, identified during routine malaria smear examination. Seven patients were undocumented East African migrants who had recently entered through the Yemeni bord

Borrelia theileri infection in beef and dairy cattle in Israel: molecular detection and co-infection with bovine piroplasmids.

BACKGROUND: Tick-borne relapsing fever is an infectious disease caused by spirochetes of the genus Borrelia, leading to fever and spirochetemia in humans and animals. Borrelia theileri is the agent of bovine relapsing fever, which is transmitted by ixodid ticks of the genus Rhipicephalus and has been reported to cause mild disease in cattle. It has been infrequently reported in several countries over a broad geographic area; however, it has not been reported in Israel previously. METHODS: Blood samples were collected from beef and dairy cattle in 23 locations in Israel. Real-time and conventi

Prevalence, distribution, and phylogenetics of the tick-borne relapsing fever spirochete Borrelia turicatae in the soft tick Ornithodoros turicata americanus in Florida.

In the southwestern United States, the occurrence and potential for disease spread of Tick-Borne Relapsing Fever (TBRF) has been studied. In Florida, Borrelia pathogens that cause TBRF have only been found in two domestic dogs (Canis lupus familiaris), yet the soft tick vector, Ornithodoros turicata americanus, is commonly found throughout the state. The goal of our study was to provide the first large-scale investigation of this disease system in the southeastern US. Our objectives were to: 1) describe the occurrence and prevalence of Borrelia spp. in ticks found throughout their distribution

Quantitative polymerase chain reaction from malaria rapid diagnostic tests to detect Borrelia crocidurae in Mali.

BACKGROUND: Tick-borne relapsing fever (TBRF) is caused by Borrelia species transmitted to humans by soft ticks of the genus Ornithodoros. Very little is currently known about the morbidity of this disease in Mali, despite the risk of human co-infection with malaria. The lack of appropriate diagnostic services or technical expertise to differentiate suspected malaria from other causes of febrile illness of unknown origin means that this tick-borne disease remains neglected and under-diagnosed in febrile patients in Mali. Our study investigated the detection of Borrelia crocidurae DNA in febril

Exposure in Horses to Human Tick-Borne Relapsing Fever Agent Borrelia persica, Israel, 2025.

Human tick-borne relapsing fever caused by Borrelia persica is common in western Asia. A survey of 301 horses in Israel revealed 9.96% seropositivity toward B. persica antigens; 1 horse (0.33%) was also PCR positive for B. persica DNA. Phylogenetic analysis supported a transmission cycle involving ticks, humans, and horses.

Lyell´s syndrome associated with the use of topical agents: case report.

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc
