Temporomandibular joint (TMJ) herniation into the external auditory canal is a rare condition associated with anterior canal wall defects, most commonly due to a patent foramen of Huschke, trauma, or infection. The clinical presentation is variable and often non-specific, which may lead to misdiagnosis. High-resolution computed tomography is crucial for accurate diagnosis and effective surgical planning. While open surgical repair remains standard, minimally invasive approaches have not been widely reported. An 81-year-old male presented with right-sided otalgia exacerbated by mandibular movem

OBJECTIVE: The goal of this investigation was to characterize the microbiological profile and antimicrobial susceptibility patterns of pathogens isolated from AOE patients of 2 vehicle-controlled Phase III clinical trials and evaluate pathogen eradication rates achieved with finafloxacin otic suspension. In vitro MIC comparisons to other fluoroquinolones (ciprofloxacin, ofloxacin) are reported as supplementary susceptibility data and do not reflect direct randomized clinical comparisons.
METHODS: A total of 2683 bacteria isolated from 1234 AOE patients enrolled in 2 Phase III clinical studies

INTRODUCTION: Otitis externa is thought to affect 10% of people at some stage, and can present in acute, chronic, or necrotising forms. Otitis externa may be associated with eczema of the ear canal, and is more common in swimmers, humid environments, people with absence of ear wax or with narrow ear canals, hearing-aid users, and after mechanical trauma.
METHODS AND OUTCOMES: We conducted a systematic review and aimed to answer the following clinical questions: What are the effects of empirical and prophylactic treatments for otitis externa? We searched: Medline, Embase, The Cochrane Library,

BACKGROUND: Malignant otitis externa is a life-threatening infection in which facial nerve palsy is a morbid complication. This TriNetX study identified predictors of facial nerve palsy in malignant otitis externa.
METHODS: Retrospective analysis compared malignant otitis externa-only (n = 12 032) versus malignant otitis externa plus facial nerve palsy (n = 719) cohorts for demographics, biochemical markers and co-morbidities using t-tests and odds ratios (p < 0.05).
RESULTS: Male gender (p = 4.65 × 10-15) and Hispanic and/or Latino ethnicity (17.66 vs 13.02 per cent, p = 0.0014) predicted f

OBJECTIVE: Malignant otitis externa (MOE) is a potentially life-threatening infection of the external auditory canal (EAC), often with skull base extension. Vascular complications are exceptionally rare, with only six reports of petrous internal carotid artery (ICA) pseudoaneurysm following MOE.
METHODS: We present the first reported case of a nasopharyngeal hematoma as a complication of MOE.
RESULTS: An 82-year-old male with poorly controlled diabetes presented with 2 months of recurrent left ear otalgia, purulent otorrhea, and facial nerve palsy. He was commenced on 6 weeks of antimicrobia

We performed a 15-year retrospective audit of all cases of malignant otitis externa (MOE) referred to Alice Springs Hospital in Central Australia from 2009 to 2024, the largest Australian series of MOE. Data on demographics, risk factors, microbiological culture results, management and outcomes were collected, identifying 64 cases of MOE, with the incidence increasing over time. The incidence of MOE was associated with Indigenous status and the presence of diabetes mellitus and chronic kidney disease. Pseudomonas, Staphylococcus aureus and fungal pathogens predominate as causal agents. Complic

BACKGROUND/OBJECTIVES: Skull base osteomyelitis and necrotising otitis externa require prolonged antibiotic therapy, yet determining optimal treatment cessation timing remains challenging. Conventional imaging modalities demonstrate persistent abnormalities beyond infection resolution, confounding treatment decisions. This systematic review evaluated the diagnostic test accuracy of 18F-fluorodeoxyglucose positron emission tomography (18F-FDG PET) for treatment response monitoring in skull base osteomyelitis and necrotising otitis externa.
METHODS: We conducted a systematic review following PR

We report on a patient with severe progressive osteomyelitis of the lateral skull base, despite targeted long-term antibiotic therapy. Successful treatment and recovery of the facial nerve palsy were achieved through additional surgical debridement and insertion of synthetic calcium sulfate beads as a local antibiotic carrier to the skull base. We describe the characteristics of this novel biodegradable long-term release matrix and the surgical technique in detail. The patient is free from recurrence 2 years postoperatively.

Staphylococcus caprae, a non-aureus staphylococcus primarily associated with goats, is typically regarded as a commensal organism. While its role in veterinary contexts is well established, its emergence as a human pathogen in healthcare settings has gained increasing recognition. It has been implicated in infections such as bacteremia, otitis externa, osteoarticular infections, and prosthetic device-related complications. However, infective endocarditis caused by S. caprae remains extremely rare, with only six cases reported in the literature since 1995. This report presents a case of native

A limited number of antibiotics are available to treat pathogenic Pseudomonas spp. infections in animals and among currently available antipseudomonal options, only fluoroquinolones can be administered topically, orally and parenterally. We hypothesised that canine Pseudomonas spp. isolates showed higher rates of resistance against fluoroquinolones compared to other antipseudomonal antibiotics. A systematic review and meta-analysis were conducted to determine prevalence and distribution patterns of antimicrobial resistance in Pseudomonas isolates from canine infections. Extracted data were str

Malignant otitis externa (MOE) can rarely progress to skull base osteomyelitis (SBO) with catastrophic vascular complications. We describe a 68-year-old man with uncontrolled diabetes (HbA1c 9%) who presented with left otalgia, purulent otorrhea, and hearing loss. Initial workup showed CRP 23 mg/L, leukocytes 8,000/µL, and ear pus culture positive for Pseudomonas aeruginosa. Computed tomography (CT) and magnetic resonance imaging (MRI) demonstrated MOE with temporal bone osteomyelitis. Despite prolonged intravenous antibiotics (imipenem, colistin) and topical ofloxacin, symptoms persisted. Thr

Malignant external otitis (MEO), also known as necrotizing otitis externa, is an aggressive infection of the external auditory canal and skull base, typically affecting elderly diabetic or immunocompromised patients. We present a case of an 88-year-old non-diabetic woman diagnosed with MEO whose management was complicated by a severe allergic reaction to systemic antibiotic therapy. The patient initially presented with severe otalgia and purulent otorrhea of the right ear, unresponsive to conventional treatment. Otoscopic examination revealed granulation tissue at the osseocartilaginous juncti

An immunocompetent, non-diabetic man in his 90s presented to A&E with headache and sudden-onset left abducens nerve palsy. He was treated for recurrent left otitis externa six months previously, with a normal CT of the petrous bone at the time.Ear appearances were unremarkable, but CT petrous bones revealed extensive skull base osteomyelitis extending to the petrous apex. MRI skull base and nuclear medicine technetium-99m hexamethylpropyleneamine-oxime white cell scanning confirmed active osteomyelitis.Necrotising otitis externa (NOE) is a complication of otitis externa and is more common in t

Necrotizing otitis externa (NOE) is a severe and potentially life-threatening infection of the external auditory canal, particularly affecting elderly diabetic or immunocompromised patients. Pseudomonas aeruginosa is the most frequently isolated pathogen. Among fungi, Aspergillus and Candida spp. are the most commonly implicated. Nevertheless, Scedosporium sp. has also emerged as a notable pathogen in NOE. We report the case of a 67-year-old woman with poorly controlled diabetes who presented with persistent right-sided otorrhea and otalgia, complicated by facial paralysis. Imaging revealed bo

This article explains the biochemical basis of the synergistic effect of oxythiamine (OT) and ketoconazole (KTC) against Malassezia pachydermatis yeast, which was isolated from dogs exhibiting clinical signs of otitis externa. All strains were incubated on MLNA medium supplemented with OT, KTC, or a mixture of both compounds. We found that the ergosterol content was reduced by the compounds tested, both separately (20%-50%) and in combination (80%). OT alone and in combination with KTC reduced NADPH levels. However, we found no differences in acetyl-CoA levels under the influence of the compou

Bradycardia-induced cardiomyopathy is a reversible form of myocardial dysfunction caused by prolonged bradyarrhythmia, most commonly due to high-grade atrioventricular block or sinus node dysfunction. Although bradycardia-induced cardiomyopathy is well recognized in human medicine, it remains underreported in veterinary literature. This case series describes 3 dogs diagnosed with 3rd-degree atrioventricular block that presented with clinical signs including exercise intolerance, syncope, and cardiomegaly. Each dog underwent comprehensive diagnostic evaluation, including physical examination, t

Otodectes cynotis is the agent of otodectic mange, a disease affecting wild and domestic felines, canines and mustelids. It is a highly contagious and pruritic condition and a major cause of otitis externa in dogs. This study was designed to verify the efficacy of NexGard® PLUS, a combination of afoxolaner, moxidectin and pyrantel pamoate, against natural O. cynotis infestations in dogs. It was a blinded, randomised, single-centre, negative controlled clinical efficacy study. Twenty-four naturally infested dogs were allocated to three groups of eight dogs: an untreated control group, a group t
